We have studied 12 reports of
pneumonitis
associated with nilutamide (Anandron) and notified to the French regional
ADR
monitoring centers between November 1987 and June 1990. The mean age of the patients was 71.5 +/- 9.5 years, and 35% (5/12) had a history of lung disease. All 12 patients developed dyspnea, cough and fever, 4.7 +/- 6 months after starting nilutamide. Ten patients (83%) required admission to hospital. The symptoms revolved in 11 cases when the drug was withdrawn. In the other case, they disappeared when another drug was stopped and the dose of nilutamide was reduced. Six patients were treated with glucocorticoids. The severity of nilutamide-associated
pneumonitis
appears to be related to the time between the onset of dyspnea and consultation.

A 47-year-old man was admitted with a cough on January 4, 1986. A chest X-ray film showed a mass shadow in the left lower lung, which was revealed to be a bronchogenic cyst by CT scanning and ultrasonography. Thoracotomy was performed on March 3, 1986 because cytologic tests on the fluid in the cyst suggested malignancy. A cyst, two tumors on the diaphragm and pleural thickening were revealed. Microscopic examination showed a benign bronchogenic cyst and a mixed-type malignant mesothelioma. In spite of chemotherapy (
ADR
, Cis-DPP, 5-fluorouracil) and immunotherapy (OK-432, PSK), the pleural thickening progressed, as was demonstrated by CT scanning and ultrasonography. Although cardiac tamponade due to invasion by the malignant mesothelioma developed, this was improved by cardiocentesis. The patient died of
pneumonia
on March 28, 1987. We studied the concentration of mineral fibers in lung and tumor tissues of this case by Energy Dispersive X-ray Analyser because asbestos or non-asbestos inorganic fibers might cause malignant mesothelioma. This case of malignant pleural mesothelioma accompanied by a bronchogenic cyst is very rare.

Chronic doxorubicin (adriamycin;

) intoxication was produced in Beagle dogs by weekly IV injections (1 mg/kg of body weight) for 20 weeks (cumulative dose 400 mg of

/m2). Group A (5 dogs) were given
ADR
only; group B (5 dogs),
ADR
and daily supplements of 0.5 mg of L-thyroxine (T4) (twice the recommended maintenance dose); and group C (5 dogs),
ADR
and 2.0 mg of T4 daily (8 times the recommended maintenance dose). The control group D (5 dogs) was given 0.5 mg of T4 daily. The
ADR
-induced cutaneous lesions (alopecia and melanosis), body weight loss, hematologic alterations, and myocardial microscopic and ultrastructural alterations (sarcoplasmic vacuolation and myocytolysis) were present with equal severity in the 3
ADR
-treated groups, with or without T4 supplements. Six of the 15

-treated dogs died during the study: 3 dogs had

-treated dogs after 28, 56, 84, 112, and 140 days and were attributed to faulty extrathyroidal conversion of T4 to T3. Serum T4 concentrations were high in dogs given 2 mg of T4/day (group C) after 28, 56, 84, and 112 days, but were low terminally as those dogs became debilitated from chronic
ADR
toxicosis. Unusual clinical signs or cardiac alterations of hyperthyroidism did not develop in the dogs given T4 supplements (groups B, C, and D). Our findings indicate that supplements of T4 do not alter the development of the cardiac and extracardiac lesions of chronic
ADR
toxicosis in dogs and indicate that the
ADR
-induced lesions probably are not mediated via hypothyroidism.

We describe a 35-year-old male admitted to the intensive care unit (ICU) for acute exacerbation of chronic obstructive pulmonary disease (COPD). He developed ventilator-associated
pneumonia
caused by multidrug-resistant Pseudomonas aeruginosa and was treated with imipenem and colistin without any renal toxicity. The patient was readmitted to the ICU for a 2nd and a 3rd exacerbation of COPD and was again treated with imipenem and colistin. In both episodes, he developed rapid worsening in renal function, which improved following colistin withdrawal. Use of the Naranjo
ADR
probability scale indicated a probable relationship between the renal failure and the colistin therapy. In addition, the time course of events suggested that colistin was the cause of acute interstitial nephritis in this patient. We conclude that our patient had a possible acute allergic reaction to colistin since the 1st introduction was not associated with any renal toxicity and renal failure was observed on the 1st day of the 2nd and the 3rd initiation of colistin therapy, respectively.

Hepatoblastoma (HB) frequently presents at an advanced and unresectable stage. Transcatheter arterial chemoembolization (TACE) had been attempted to improve the feasibility of removing the bulky tumor in the authors' hospital and the results were presented here to evaluate the effectiveness and therapeutic role of TACE in the HB infants. Eight patients (6 boys, 2 girls), ranging in age from 2 months to 12 months, had unresectable HB based on clinical manifestation, B-ultrasound (B-US), chest X-ray film, computed tomography (CT), blood chemistry, and serum alpha-fetoprotein (AFP), and were subjected to TACE 1-3 times. On each TACE, Adriamycin (
ADR
, 20 mg/m2), vincristine (VCR, 1.5 mg/m2), and cisplatin (CDDP, 40 mg/m2) dispersed in 5-10 mL lipiodol were infused into the tumor, and stainless-steel embolization coils were released into the main feeding artery until completely embolized. Then, all the patients were reexamined once a month. Digital subtractive angiography was performed and the therapeutic strategy of further TACE or surgery was individualized in terms of the changes of tumor stain and the newly forming feeding artery. Six children (75%) had a marked response after the first TACE and were judged as being surgically resectable, but one boy died of
pneumonia
just before the scheduled operation and another boy preferred further TACE. The other 2 patients had only a partial response and required further TACE before the operation. Thus 6 children eventually underwent complete surgical resection and 1 boy achieved successful disappearance of tumor after 3 episodes of TACE alone. Seven children had an excellent recovery and remained tumor-free for 15-49 months. The results indicate that TACE is an effective and useful preoperative therapeutic choice for unresectable hepatoblastoma, and can improve the resectablity of the bulky tumor and the survival rate of HB patients. Multiple TACE could enhance the therapeutic effect and should be considered if indicated.
