Men and women who came to clinics in Boston underwent pharyngeal examinations, and pharyngeal specimens were obtained for cultures for Neisseria gonorrhoeae, Mycoplasma hominis, and Ureaplasma urealyticum. Fifty-one (4.9%) of 1,037 participants had
gonococcal
pharyngeal infection. M. hominis and U. urealyticum were recovered from the pharynges of 149 (14.3%) and 154 (14.8%) of 1,044 participants, respectively. The history of ever having performed fellatio was associated with pharyngeal infection with N. gonorrhoeae (P less than 0.02), M. hominis (P less than 0.05), and U. urealyticum (P less than 0.006). A history of fellatio was also associated with a history of a recent sore throat. There was, however, no association between pharyngeal infection with N. gonorrhoeae, M. hominis, or U. urealyticum and a recent sore throat. Cunnilingus was not associated with symptoms or signs or
pharyngitis
or with the isolation of gonococci or genital mycoplasmas from the pharynx. The
pharyngitis
associated with fellatio remains a microbiologic enigma.

A single intramuscular injection of 0.2 g of spectinomycin hydrochloride is highly effective for the treatment of uncomplicated anogenital infections with Neisseria gonorrhoeae. Spectinomycin hydrochloride is indicated for uncomplicated anogenital
gonococcal
infections in men and women who cannot receive penicillin or probenecid, and is the drug of choice for the retreatment of patients with uncomplicated anogenital
gonococcal
infection who have not responded to other antibiotics. A single dose of spectinomycin does not appear to be reliably effective in the treatment of
gonococcal
pharyngitis
. Preliminary reports indicate that multiple-dose schedules of spectinomycin will prove to be effective in
gonococcal
pelvic inflammatory disease and disseminated
gonococcal
infection. In the dosage used for
gonococcal
infections, spectinomycin has little effect on infection with Treponema pallidum. Evaluation of this antibiotic against other microorganisms suggests little utility for this agent in conditions other than
gonococcal
infections.

In many cases
gonorrhea
is overlooked and not treated due to an unusual course of the disease. The longer this infection exists the greater is the danger of dissemination of the gonococci.
Gonorrhea
, if not diagnosed, may cause a series of local and general clinical manifestations, such as: the dermatitis-arthritis syndrome whose pathogenesis is not yet completely clarified: the Fitz-Hugh-Curtis syndrome which is often seen in females with chronic
gonococcal
adnexitis;
gonococcal
pharyngitis
which nowadays occurs more often due to less conventional sexual practices of contemporary society;
gonococcal
conjunctivitis which represents the best known extragenital gonococcosis. This paper deals with the clinical manifestations and their treatment.

The arthritis of rheumatic fever in adults may mimic acute
gonococcal
arthritis. To further characterize this clinical picture the features of six patients presenting with the migratory polyarthritis of acute rheumatic fever have been analyzed. There were two men and four women, ranging in age from 18 to 43; in five the initial clinical impression was
gonococcal
arthritis. In all patients, however, the diagnosis of acute rheumatic fever was subsequently established. Characteristics of acute rheumatic fever not commonly described included tenosynovitis in all six and an erythematous rash in three. In three patients synovial fluid WBC exceeded 28,000/cu mm, with greater than 90% neutrophils. In five of the six,
gonococcal
arthritis was ruled out by appropriate studies and by failure to respond to antibiotics in all patients. The one patient with serum antigonococcal antibodies had
gonococcal
pharyngitis
and acute rheumatic fever.

Sera from 103 fasting individuals 3 to 76 years of age and free of clinical infectious disease and sera from 183 patients with infectious disease were assayed for serum total non-esterfied fatty acids (tNEFA) and compared. Data were also separated into five groups according to age of donor: 3--7, 8--19, 20--35, 36--60, and 61--76 years. The mean group serum levels of tNEFA increased with age. Among patients with infectious diseases sixty-five were diagnosed as having hepatitis, 41 with infectious mononucleosis, 18 with cellulitis, 12 with pulmonary tuberculosis, 11 with non-pneumococcal pneumonia, 9 with pneumococcal pneumonia, 8 with
pharyngitis
, 6 with pyelonephritis, 6 with aseptic meningitis, 4 with Gram-negative sepsis, and 3 with encephalitis. The sera from 23 non-fasting patients with
gonorrhea
were also tested. The serum tNEFA levels were found to be altered, in fact depressed from normal group values, only in patients with pneumonia or tuberculosis. This depression may be related to aberrant pulmonary metabolism during pneumonia.

A 19-year-old man was admitted to the hospital with acute onset of fever, sore thoart, and leg pain. He gave a history of recent sexual activity and was found to have an exudative
pharyngitis
, two pustular skin lesions, and a hot, swollen left knee. Laboratory studies were consistent with an acute septic arthritis and the diagnosis of the
gonococcal
arthritis-dermatitis syndrome was entertained. He responded to high doses of penicillin, and Neisseria meningitidis, group B, subsequently was cultured from the blood and pharynx. The same organism was isolated from the throats of three of 11 asymptomatic contacts.

Despite the fact that
gonorrhea
is our most common reportable infectious disease, little is known about natural and acquired resistance to Neisseria gonorrhoeae. With the chimpanzee model, which mimics human
gonococcal
infection in signs, symptoms, and host response, a natural resistance to
gonococcal
challenge was found. One aspect of this natural resistance became evident when the cervix and oral pharynx resisted more gonococci than the urethra. Natural resistance was also shown when environmental factors were found to influence resistance to
gonococcal
pharyngitis
. In addition to natural resistance a postinfection-acquired immunity to the gonococcus was demonstrated. Following
gonococcal
pharyngitis
, this anatomical location successfully resisted more gonococci than were initially resisted. Similarly, more gonococci were successfully resisted in rechallenging the urethra. These findings are related to the clinic situation and suggest possible new approaches to
gonorrhea
control.

Today, there is an apparent increase in oral genital activity among heterosexual groups not only in the United States but also globally. As a result,
gonococcal
pharyngitis
was rediscovered, and primary syphilis of the oral cavity has been recognized with increasing frequency. In addition, physicians are seeing a wide variety of traumatic lesions of the genitals from "hickeys" of the labia to dental imprints and ulcerations of the glans penis. Our patient exhibits an interesting phenomenon recognized readily elsewhere on the skin but infrequently diagnosed on the glans penis.

In a prospective survey of 106 male homosexuals, investigations included smears and cultures of material taken from the tonsillar areas. A diagnosis of
gonococcal
pharyngitis
was made in six cases. This is the largest series so far reported from England. The prevalence of gonorrhoea nd less conventional sexual practices must prompt us to investigate
gonococcal
infection in sites remote from the ano-genital region.

A 15-year-old boy had acute
gonococcal
flexor tenosynovitis of the middle finger associated with symptomatic
gonococcal
pharyngitis
. The patient had a history of blunt trauma to the hand. The need for a careful history in any sexually active adolescent is emphasized. Acute flexor tenosynovitis may be the only manifestation of disseminated
gonococcal
infection.
