IUDs are being investigated in the Contraceptive Clinic at Charing Cross Hospital in London. An interesting trend in the incidence of pelvic inflammation (
PID
) was found when the Lippes loop and the Birnberg bow were compared. 149 cases of women who had a Lippes loop were followed up at Charing Cross and at the Family Planning Association clinic. 128 users of the Brinberg bow were followed up. 27 users of the Lippes loop had
PID
and 5 had possible

. The figures for the Birnberg bow were 1 with

. The diagnosis was made on clinical grounds. The symptoms were usually minimal, such as backache, dysmenorrhea, premenstrual backache, pelvic discomfort, and vaginal discharge. A few of these cases were very ill with high pyrexia and severe pain necessitating admission to the hospital and may even present as pelvic
peritonitis
. Although this is a small series, several cases of
PID
of varying degrees have been observed in women wearing an IUD with a cervical appendage or having had 1 previously removed. Some of these subacute cases were missed because physicians were prepared to accept a certain degree of pelvic discomfort as part of the reaction of the uterus to an IUD.
PID
does occur in women with an IUD and there appears to be a significant predilection for those women using the Lippes loop, suggesting that organisms gain entry into the upper genital tract up the treads through the cervix.

Laparoscopy, is technique, indications, contraindications, and complications as well as the author's personal experience with it are described. It is a new procedure for diagnosis and treatment of intraabdominal and pelvic conditions. It is a safe and effective method of tubal ligation with shorter hospitalization time. The complete procedure for laparoscopy is discussed in the article. Most patients are able to leave the hospital the day of surgery, and the clips are removed in the office in 72 hours. Laparoscopy is used in sterilization and diagnostically in cases of infertility, pelvic pain, congenital anomalies, second look procedures, and removal of IUD. It should not be used in patients in whom anesthesia is contraindicated, or those with intestinal obstruction,

, and extensive abdominal scarring. The recovery rate is virtually 100% within 24 to 48 hours following laproscopy. Complications in the author's experience with laparoscopy include, perforation of inferior epigastric artery, postoperative
