From 1966 up to 1975 164 patients with a hernia incarcerata were seen. 21,3% of them died, in most cases of delayed diagnosis bowel resection was necessary. The prognosis depends on
ileus
(11,8%) and multimorbidity of the aged. The death was mainly related to
peritonitis
(43%) and pneumonia (26%). Only no delay in diagnosis, early surgery, intensive pre- and postoperative care will improve the results.

The acute enteritis in the postneonatal period with or without dyspepsia appeared with an incidence ranging from 18,9% to 27,6% during twenty years period (1956--1975) observations in a general children's autopsy material. Bacteriological and serological analysis may be of success and complete the pathologic-anatomical diagnoses. Infective enteritis often is accompanied by pneumonia and/or otitis media. The acute enteritis can always be detected as an important feature in regard to the cause of death. Most children die within less than twenty-four hours from hospital admission. The hemorrhagic, ulcerous and necrotic form of the enterocolitis predominate the perinatal and neonatal period, frequently combined with
peritonitis
and bowel perforation. Bacteriological examinations should be obtained. This form of the acute enteritis and enterocolitis of the newborns must be taken into consideration of the

symptoms differential diagnosis.

The author believes that the vascular spasm does not play any part in the occurrence of functional

. Humoral factors, such as the loss of electrolytes and serotonin, which is of a specific mediatory effect upon the intestine in particular, are by far more important. It seems that some other humoral factors, e.g. bacterial toxins and endotoxins, formed during the inflammatory process, play their part too. In a period of 21 years we observed 28 tumours of the small bowel (14 benign, 14 malignant), what comes up to 0.85% of all tumours of the GI-tract seen at our hospital. The low rate of incidence of malignomas of the small bowel is supposed to be in relation with the carcinogenic indifference of its contents. 60% of the cases underwent operations for acute complications, such as
ileus
or
peritonitis
. Early diagnosis of these tumours is difficult due to their uncharacteristic symptoms and the lack of radiographic signs. Only 4 out of the 14 cases with malignomas underwent radical surgery. The prognosis of malignant tumours of the small bowel is not a good one due to delay of diagnosis, early metastising and local involvement of the mesentery. The 5 year survival rate was 22%. Solitary benign tumours of the small bowel after their removal have an excellent prognosis.

Sequential chemotherapeutic regimens, primarily used in the treatment of hematopoietic malignancies, and employing ara-C as a basic antineoplastic agent induce mucosal alterations in the entire gastrointestinal tract. These are characterized by surface and glandular epithelial atypia, immaturity, and necrosis. Glandular regeneration is characteristically delayed leading to a state of intestinal aproliferative cytopenia. Other toxic intestinal changes include telangiectasia of blood vessels and the formation of intramural hematomas. Intestinal infections develop frequently and are complicated by
peritonitis
, liver abscesses, pneumatosis cystoides in testinalis and sepsis. These intestinal lesions are accompanied by a predictable clinical syndrome which begins concomitantly with ara-C infusions and is characterized by diarrhea,

, abdominal pain, hematemesis and melena, severe hypokalemia, hypocalcemia and a protein-losing enteropathy. Additional toxic manifestations induced by ara-C include transient weight gains, fever elevations and severe bone marrow depression. The genesis of the intestinal lesions is linked to the three day dose schedule of ara-C infusions which insures both arrest of the cycling intestinal cells in the S-phase and a high cytotoxic index. The severity of these lesions is markedly augmented by prior treatment with ara-C and cyclophosphamide which causes synchronization and probable recruitment of intestinal stem cells, respectively.

Later barium suspensions permit to obtain pictures of good quality. Middle and lower segments of the small bowl are best analysed with duodenal intubation. Pneumocolon gives often the best results in controlling ileocolic anastomosis.

A pedunculate patch, obtained from a disconnected segment of jejunum, was implanted into the longitudinal incision of the ductus choledochus. Intra-operatively, the findings were based upon direct cholecystocholangiography; postoperatively, upon laboratory tests. After an observation period of 12 months a new cholangiography of the transplant was made along with a selective picture of the mesenteric arteries. The choledochus duct with the transplant was examined histologically. Three dogs died immediately postoperatively of
peritonitis
due to suture insufficiency. Five dogs died of invaginate
ileus
. These animals, as well as the remaining 8 dogs showed macroscopically and microscopically a smooth and uneventful healing of the transplant at the end of the observation period. Radiographies showed free flow of the biliary duct system. The selective mesenteric arteriographies evidenced beginning anastomosis between the transplant and the capillaries of the bile duct. Based on the results presented, the pedunculate jejunal graft can be considered well suited as an expansion graft for the bile duct.

From 1958 to 1977 a total of 390 patients underwent surgery for colonic obstruction of neoplastic origin. The series includes 218 women and 172 men, average age 64 years. 135 patients (35%) came to death. The incidence of left colon carcinoma considerably exceeds the carcinoma of the right side (73 or 27% accordingly). The operative methods used are grouped under three headings: primary resection (200 cases = 51%; mortality rate 31%), staged resection (52 cases = 13%, mortality rate 33%), palliative interventions (138 cases = 36%; mortality rate 40%).
