Spontaneous
peritonitis
in adults has not been previously reported to occur in association with ascites of neoplastic origin. We report a patient with metastatic gastric adenocarcinoma in whom spontaneous streptococcus pneumonia
peritonitis
occurred as a terminal event. Massive metastases may have resulted in significant loss of the hepatic reticuloendothelial system and impaired the capability of the liver to function as an effective bacterial filter, predisposing the patient to bacterial peritonitis.

The results of surgical treatment of 765 patients with colonic
cancer
are analysed, The resectability was 68.7%, postoperative mortality--in 16.3%. The most frequent causes of lethal issues were
peritonitis
and acute cardiovascular disturbances. The survival of 5 years and longer after radical operations was noted in 53.4 +/- 3.4%. The survival rate in patients with
cancer
of the right colon is reliably higher than that with the left colon involved. Hemicolectomy yielded better results compared with segmental resections. The worst results of the treatment were noted in stage T4--T3. The presence of metastases in regional lymph nodes reduces 3 times the number of patients survived for 5 years after radical surgery, compared with those without regional metastases. Late results of the treatment in mucous and solid cancers were several times worse than in adenocarcinoma.

Evidence is presented concerning 94 patients with
malignancy
of bladder in whom diversion of urine was performed after cystectomy for exclusion of the bladder. There were 80 men and 14 women, ages ranging from 30 to 81 years. In 35 of these 94 cases ureterosigmoidoanastomosis, in 38 cases ureterocutaneostomy, in 15 Portilla's operation, in 2 Bricker's method and in 4 nephro-pyelostomy were performed. For the prevention of bleeding cystectomy was preceded by bilateral ligation of the internal iliac artery in 14 cases. Continuous postoperative follow-up of the patients (from a few months to 14 years) and analysis of the early and late results allow to regard the combined technique of Coffey II-Nesbit-Goodwin as the method of choice having the slightest risk of
peritonitis
, intestinoureteral reflux and other complications inherent in other procedures. Of the existing methods of ureterocutaneostomy the authors have chosen Le Dentu's method with some modifications. They consider ureterostomy to be the simplest and quickest method owing to the higher location of ureterostoma. Portilla's method is no longer used because of its unfavourable long-term results. Of 94 patients with cancer of the bladder, who had been subjected to different methods of urinary diversion in the years 1954-1974, 16 have survived up till now. There are 10 survivors after ureterosigmoidoanastomosis, 5 after ureterocutaneostomy and 1 after Bricker's operation.

An eight-year experience at Charity Hosptial and Touro Infirmary indicates that 10 percent of splenectomies were performed for reasons other than trauma or
malignancy
. Fifty-nine per cent of therapeutic splenectomies were done for congenital spherocytosis or idiopathic thromboytopenic purpura. All patients having spherocytosis and 57 per cent having ITP had favorable response following splenctomy. Seven surgical death occurred following 69 splenectomies (mortality 10%). Three deaths were from pulmonary emboli, three from hemorrhagic disorders and one from
peritonitis
. Eighteen postopertive complications occurred, the most frequent being thromboembolic disease. Prophylactic anticoagulants were not used in any patient.

When operating as an emergency for a perforation of the colon should one carry out a by-pass operation, colostomy or restore intestinal continuity? 36 colonic perforations are reported here, as a complication of
cancer
in 50 p. 100 of cases, or sigmoiditis in 38 p. 100 of cases. The perforation was situated on the sigmoid in 77 p. 100 of cases. 13 by-pass operations were carried out with a 23 p. 100 mortality, 12 colectomies without anastomosis with a 50 p. 100 mortality, and 2 colectomies with restoration of continuity, 9 p. 100 mortality. The overall mortality was 27.7 p. 100 much less than reported by other authors but, nevertheless partly due to the severity of
peritonitis
due to the presence of feces and, partly due to the operation adopted. The present attitude of the authors depends on the merits of each case. A large incision is made and the peritoneum thoroughly cleaned. If the surgeon is experienced, colonic resection is advisable for perforated cnacer and certain cases of sigmoiditis. Restoration of continuity depends, above all, on the anatomical condition of the colon above the lesion. Hartmann's resections or by-pass operations thus still have indications.
