As accepted as the condition of
pelvic pain
and pelvic joint instability (PPPJI) is in pregnancy, as controversial and doubted is the same diagnosis in non-pregnant women. Women suffering severe incapacitating PPPJI for years following parturition have recently founded an association. This offers a unique opportunity to investigate their characteristics. 153 of the 215 members answered a questionnaire. The majority (110) suffered PPPJI in first pregnancy with onset preceding 20 weeks gestation in 84 women. Onset was significantly earlier in users of oral contraceptives than in non-users (16.3 weeks and 20.5 weeks, respectively). The 153 women did not differ from the controls with regard to life-style, diseases prior to reproduction or reproduction. Their 358 deliveries were characterized by a marked excess of post-term deliveries, a raised proportion of infants of 4000 g or more, an extremely low sex-ratio of offspring (0.84) with 54.5% female births, and a significantly raised congenital hip
dysplasia
rate, consistent with elevated oestrogen and relaxin levels.

A study of 153 women with persistent
pelvic pain
and pelvic joint instability (PPPJI) following parturition, revealed that the women differed from the controls by a significantly lower age at menarche. Precocious puberty is associated with a fibrous
dysplasia
which resembles that seen in some PPPJI women patients at operations. Precocious puberty is known to be associated with hypothalamic dysfunction. Following cessation of lactation, 70 of 153 women did suffer galactorrhea and breast discomfort, suggesting a possible affection of the hypothalamic-pituitary system. Age at menarche was higher in users of oral contraceptives (OC) than in non-users, but onset of PPPJI was significantly earlier by gestation, and galactorrhea, and breast discomfort more frequent (60% affected as compared to 30% of non-users), suggesting that use of OC affects the hypothalamus in a manner similar to that associated with very early puberty. The widespread use of contraceptives is important if it truly leads to an increased proportion of PPPJI among reproducing women. Onset of puberty, controlled by the hypothalamus, coincides with the final step in brain development--elimination of some 40% of neuronal synapses. Age at puberty has declined by some 4 years in 100 years, and it is still falling. It cannot be excluded that in some very early maturers, redundancy of neuronal synapses persists, and that this is associated with hypothalamic dysfunction.

Adamantinoma is a rare primary tumour of long bones containing mesenchymatous and epithelial cells. There has been some controversy over the pathogenesis. We observed a localization in the right tibia. A 22-year-old patient from Algeria was seen for spontaneous progressively increasing pain in the upper part of the right tibia. The patient's general health had deteriorated somewhat, with fever. On examination there was ulceration of the skin, costal and

and inflammatory right inguinal lymph nodes. Radiography revealed lateral metaphyseal lytic image with interruption of the cortex also seen on magnetic resonance imaging which revealed invasion of the soft tissue and multiple bilateral pulmonary nodules. Bone scintigraphy showed several zones of hyperfixation. The diagnosis of adamantinoma was confirmed by pathology examination of the biopsy specimen. On surgical exeresis, the capsule of the knee joint was found to be involved without invasion of the knee joint. Node dissection showed inguinal and popliteal invasion. Macroscopically, the surgical specimen was a red-whitish osteolytic tumour. Microscopically, the tumour was composed of hyperchromatic epithelial cells in an abondant fibrous stroma. Immunohistochemical studies were negative for vimentine, cytokeratin and factor VIII. Adjuvant chemotherapy was based on a sarcoma protocol. Unfortunately, after two cycles, white cell counts fell sharply and multiple skin nodules appeared together with progression of the bone metastases. The chemotherapy was modified without any therapeutic effect and the patient died in February 1993. An epithelial origin would appear most probable, but at least two groups of adamantinoma can be described: one with typical epithelial differenciation and one overlapping to the differential diagnosis of osteofibrous
dysplasia
. Although considered as a low grade malignant tumour, we emphasize the aggressive forms with local relapse or metastatic resistance to chemotherapy. Treatment relies on wide surgery and prognosis is generally good. Neither chemotherapy nor radiotherapy has stood the test of time in cases with metastasis.

Researchers examined results of cervico-vaginal smears of 350 women aged 23-45 years fitted with IUDs at various family planning clinics in Kuala Lumpur, Malaysia, to examine abnormalities in their genital tract. All the women had undergone preinsertion cervico-vaginal smears. They used the IUD for 1-8 years. Around 66% exhibited symptoms after IUD insertion. 40% had vaginal discharge, especially mucous. 3% had
pelvic pain
and intermittent low grade fever, suggesting pelvic inflammatory disease. 80% had an increase in the number of leukocytes in their blood. 42% had an increase in the number of histiocytes with multinucleate giant forms. The following microorganisms were present: Gardnerella vaginalis (42%), Trichomonas vaginalis (32%), Candida (28%), Actinomyces-like organisms (2%), and non-pathogenic Amoeba (0.6%). Both endocervical and squamous columnar cells exhibited morphological atypias (inflammatory, degenerative, or reparative changes). 70% of atypias were benign and varied from mild to severe. 14 women (4%) had cervical intra-epithelial neoplasia (CIN). 3% of the women had atypical single cells. The IUDs were removed from all of these women. 6 months after IUD removal, the cervixes with mild
dysplasia
had reverted to normal. Two women with severe
dysplasia
underwent cervical biopsy, which revealed a CIN III lesion. 28% of smears had abnormal or irritated glandular epithelial endocervical and endometrial cells with hyperchromatic nuclei, an increased nucleo-cytoplasmic ratio, and bubble-gum vacuolation of the cytoplasm. 31% of the women had normal or inflamed out-of-phase (beyond day 11 of the menstrual cycle) endometrial cells. 80% of these 109 women had menorrhagia or intermenstrual bleeding. The researchers recommend that serious epithelial atypias be followed up and the IUD be removed. IUD removal allows clinicians to determine whether atypias will regress in the absence of an IUD or are truly neoplastic.

We report the case of a 24-year-old woman with an 8-month history of deep
pelvic pain
and postcoital bleeding. Examination revealed desquamation of the vaginal epithelium with tender fissured plaques in the vagina, initially thought to be vaginal intraepithelial neoplasia. Histology showed squamous mucosa with suprabasal acantholysis and hyperkeratosis, and no evidence of viral infection,
dysplasia
or malignancy. These findings were consistent with acantholytic dermatosis (AD), a rare lesion that resembles Hailey-Hailey and Darier disease histopathologically, but can be distinguished on a clinical basis. Vulval cases of AD are well recognized, but to our knowledge, this is the first reported case involving the vaginal epithelium alone.
