Postoperative intra-abdominal and pelvic adhesions are the leading cause of infertility, chronic
pelvic pain
, and intestinal obstruction. It is generally considered that some people are more prone to develop postoperative adhesions than are others. Unfortunately, there is no available marker to predict the occurrence or the extent and severity of adhesions preoperatively.
Ischemia
has been thought to be the most important insult that leads to adhesion development. Furthermore, a deficient, suppressed, or overwhelmed natural immune system has been proposed as an underlying mechanism in adhesion development. The type of surgical approach (laparoscopy or laparotomy) and closure of peritoneum in gynecologic surgeries and cesarean section have been debated as important factors that influence the development and extent of postoperative adhesions. In this article, we have reviewed the current state of adhesion development and the effects of barrier agents in prevention of postoperative adhesions.

Causes and mechanisms of chronic
pelvic pain
/chronic prostatitis (CPP/CP) syndrome are not well investigated yet. It is suggested that pain in an inflammatory form of this syndrome may be due to tissue inflammation. In the non-inflammatory form pain may result from neurogenic factors. Standard diagnostic methods in the absence of inflammation often fail to detect pathological changes in the prostate. We made duplex mapping of the prostate and obtained quantitative characteristics of arterial hemodynamics of the prostate. Control values of arterial prostatic blood flow were developed after examination of healthy volunteers. These values were compared with similar values of arterial blood flow in CPP/CP syndrome. We found a significant decrease of the peak systolic velocity in patients with non-inflammatory CPP/CP syndrome. There is a direct correlation between intensity of pain syndrome and peak systolic velocity of arterial blood flow. Thus, pain may arise from chronic prostatic

. Although ultrasound has been the study of choice for the diagnosis of adnexal torsion, patients with
pelvic pain
may often be imaged first with computed tomography given the prevalent use of computed tomography in the emergency department. Therefore, it is essential to be familiar with the computed tomography signs of adnexal torsion. A retrospective study of 40 patients with adnexal torsion with and without an adnexal mass was conducted to evaluate interobserver reliability for the computed tomography signs of adnexal torsion. With the exception of deviation of the uterus to the twisted side, interobserver reliability was fair for all computed tomography signs of adnexal torsion. There was no difference in interobserver reliability for most signs between patients with a mass and those without. However, for a twisted vascular pedicle, there was moderate agreement in patients with a mass and no agreement for patients without a mass. Although many computed tomography signs of adnexal torsion have been described, interobserver reliability for most of these signs was found to be fair.

Acute pelvic pain during pregnancy makes the differential diagnosis more challenging. We here report two cases of adnexal torsion during the second trimester of pregnancy in order to draw attention to this diagnosis and to highlight the importance of early treatment to avoid irreversible damages due to
ischemia
which can be fertility-threatening. The first patient, G1P0, 20 weeks pregnant, initially presented with appendix syndrome. Exploration with a small McBurney incision showed a right ovarian necrosis, hence ovariectomy was performed. The postoperative course was uneventful. The second patient, G2P2, 26 weeks pregnant, presented to the emergency departments with acute left iliac fossa pain. Laparotomy revealed the torsion of a hydatid of Morgagni whose necrotic appearance due to twisting required hydatid ablation. No postoperative complications were noted in the two patients. Adnexal torsion is an emergency condition that should not be ignored in the case of acute
pelvic pain
in pregnant women. Conservative treatment represents the gold standard and proper management is necessary to avoid possible maternal and fetal complications.
