Although there is increasing awareness of the short-term psychological and social adaptations to childhood sexual abuse, little is known about the long-term effects of such abuse, particularly its effect on subsequent medical utilization and the experience and reporting of physical symptoms. We re-analyzed data from a previous study of 100 women scheduled for diagnostic laparoscopy (50 for chronic pain, 50 for tubal ligation or infertility evaluation) who received structured, physician-administered psychiatric and sexual abuse interviews. Women were regrouped by severity of childhood sexual abuse, and we compared the groups with respect to lifetime psychiatric diagnoses and medically unexplained symptom patterns. Unadjusted odds ratios showed that risk for lifetime diagnoses of major depression, panic disorder, phobia, somatization disorder and drug abuse, and current diagnoses of major depression and somatoform pain disorder were significantly higher in the severely abused group compared with women with no abuse or less severe abuse. Logistic regression analysis demonstrated that number of somatization symptoms, lifetime panic disorder and drug dependence were predictive of a prior history of severe childhood sexual abuse. Psychiatric disorders and medical symptoms, particularly chronic pelvic pain, are common in women with histories of severe childhood sexual abuse. Clinicians should inquire about childhood sexual and physical abuse experiences in patients with multiple medical and psychiatric symptoms, particularly patients with chronic pelvic pain.

The prevalence of genital actinomyces infection and possible routes of transmission in IUD users were analyzed in a high-risk population of predominantly indigent Mexican-American family planning clients. The Papanicolaou (Pap) smears of 12 (9%) of the 134 IUD users cultured were positive for actinomyces-like organisms. The IUD involved was the Lippes Loop in 7 cases, the Cu 7 in 2 women, the Dalkon Shield in another 2 cases, and the Saf-T-Coil in the final case. The duration of IUD use ranged from 1-10 years, with a mean of 6 years. Oral-genital sexual contact was the sexual preference in 3/4 of the women with actinomyces infection compared with under 2/3 of the general clinic population. Most of the women with actinomyces-like organisms had a concomitant condition, including gastroenteritis, cholecystitis, scabies, schizophrenia, drug abuse, anemia, herpes genitalis, venereal disease, and urinary tract infection. All of these women complained of vaginal discharge with pelvic pain during their initial visits. Eikenella corrodens was recovered in 1 of these cases and had an overall prevalence of 0.17% in the clinic population. Eikenella corrodens is found in the mouth, on dental plaques, and is not yet recognized as a normal inhabitant of the vagina or gastrointestinal tract. Oral-genital contact appears to be the mode of transmission of both actinomyces and Eikenella corrodens.

Criminal statistics say that 300,000 children are sexually abused in the Federal Republic of Germany every year: 70-75% are abused by their own fathers or another psychological parent. Most victims are girls aged 7-12 years. Sexual abuse during childhood can lead to severe psychosomatic dysfunctions both in children and adults. Possible long-term results are depression, anxiety, emotional and cognitive problems, personal dysfunction, eating and sleeping disorders, alcohol or drug abuse, relationship problems, social maladaptation, and somatizations. Many urological dysfunctions without organic findings can be caused by sexual abuse. Among others, chronic pelvic pain (CPPS), enuresis, incontinence, and sexual dysfunction can occur. When children or adults see the urologist because of their symptoms there is always the danger of reproducing the abusive event by invasive diagnostic methods.Sometimes harming themselves the patients bring this situation about unconsciously. With the following article we want to heighten the awareness among urologists.
