A pilot study was conducted, in which photodynamic therapy (PDT), a technique in which malignant cells are destroyed by light after being previously photosensitized by a chemical compound, was tried in a group of 14 patients with recurrent or residual colorectal cancer in the pelvis. Three of the six patients with unresectable pelvic recurrences experienced a significant relief of pain after PDT. In two of the five patients who had an incomplete resection of their pelvic recurrences, there was also a substantial relief of
pelvic pain
after surgery and PDT. In one of these patients subsequent biopsies proved the disappearance of the residual pelvic microscopic disease after several sessions of PDT. Three patients had a recurrence from a squamous cell carcinoma primary of the anal canal. All recurrences were amenable to surgical resection. In one of the patients, PDT was used in an attempt to sterilize an area of residual tumor that was located over the left ischial tuberosity. The patient experienced good relief of pain, but died of her disease 7 months after PDT. In the other two patients, PDT was used as an "adjunct" after resection of their recurrences. One of these patients was free of disease and died of an unrelated cause 12 months after PDT. The other is alive and well. This study demonstrated that PDT can be safe and tolerable in patients with pelvic
malignancies
. PDT is capable of tumor destruction, can be used repeatedly in areas previously exposed to ionizing radiation, and may have a role in the prevention and management of pelvic-perineal recurrences from colorectal cancer.

Metastatic choriocarcinoma was suspected in a 39-year-old woman who presented 7 months postpartum with fatigue,
pelvic pain
, a massive pleural effusion, and a positive urine pregnancy test. Subsequent evaluation resulted in discovery of the isolated production of the free beta-subunit of chorionic gonadotropin (CG-beta) by a widespread, poorly differentiated epidermoid carcinoma. Chemotherapy was ineffective, the woman died, and at autopsy the primary site of the tumor could not be determined. The patient's serum (185 ng/ml) and a tumor metastasis (720 ng/g) contained large amounts of immunoactive material that diluted in parallel to CG-beta standard, but neither chorionic gonadotropin (CG), its alpha subunit, nor other placental proteins were detected. A monoclonal antibody that recognizes free CG-beta, but not intact CG, was instrumental in implicating an ectopic source of the CG-beta before a tissue diagnosis was obtained. When the patient's serum was chromatographed on a dextran gel, the CG-beta immunoactivity eluted in a position of higher apparent molecular weight than either standard CG or CG-beta, suggesting that this neoplasm secreted an altered molecular form of the CG-beta subunit.

Since the establishment of the department at a hospital in Valparais o, Chile, the number of births has declined, as well as the number of abortions (monthly births, from 102 to 70, abortions from 24.7 to 13.8, that is, a decrease of 36.4% and 43.4%, respectively). IUDs, oral contraceptives and, in some cases, condoms have been prescribed. Complications due to IUDs generally concerned expulsion, metrorrhagia (both curable and requiring removal of IUD), infection and intrauterine perforation (few cases) and

. The pregnancy rate per 100 years of observations was .80 with the Lippes IUD (more used) and 2.03 with the zipper type. Expulsions amounted to 5.4%. After repositioning, the final expulsion rate was only 1.25%. With respect to oral contraceptives, the findings coincide with those already widely published. The sequential treatment appears to be tolerated considerably better. Since 1970 all patients were subjected to cytological tests, irrespective of age and of apparent good health. A series of 289 patients who were diagnosed with 342 benign ovarian tumors (BOT) at laparotomy have been reviewed. Mean age was 36.7 years (S.D. +/- 13.9), mean parity was 1.5 (S.D. +/- 1.6); 236 (81.7%) patients had a single unilateral ovarian tumor; 29 (10%) patients had bilateral ovarian tumors. Benign cyst teratoma was diagnosed in 89 (26%) tumors, and it was the most frequent histological type. In eight patients (2.8%) the post-operative pathologic report was compatible with
malignancy
, despite the lack of suspicion at laparotomy. Mean maximal diameter of BOT was 9.1 cm, and almost 60% of the mucinous cystoadenomas were larger than 16 cm. Benign cyst teratoma and serous cytoadenoma had a medium size, about 6-10 cm. Lower abdominal-

was the symptom most frequently reported by patients with BOT. Calcifications were most frequently seen in benign cyst teratoma. Tumors of 11-15 cm in size were the tumors most frequently torsioned. Twenty four (8.3%) patients had their surgical operation done during pregnancy; benign cyst teratoma was the tumor most frequently associated with pregnancy.

Sixty-two patients with intractable
pelvic pain
secondary to
malignancy
arising in the pelvis received a total of 72 percutaneous retrograde arterial infusions with nitrogen mustard. Fifty-three (74%) injections resulted in marked relief of pain for an average period of six to eight weeks. Five (7%) produced no pain relief. Toxicity was mild. There were two severe complications and no deaths related to the procedure or the drug in this study. Percutaneous retrograde arterial infusion with nitrogen mustard is suggested as the preferred procedure in the attempt to control

Clinical and pathological features of 41 homologous uterine stromal tumors classified by the criteria of Norris and Taylor were reviewed. These included stromal nodules (12 cases), endolymphatic stromal myosis (20 cases), and stromal sarcoma (nine cases). Patients ranged in age from 18 to 79 with a median of 45 years. Symptoms were similar in all three forms and included vaginal bleeding (63%),
pelvic pain
(11%), or an abdominal mass (6%), while 26% of patients were asymptomatic. Tumors characteristically resembled endometrial stroma histologically, but variations included hemangiopericytoma and "sex-cord-like" patterns, hyalinization, and foam cells. A sex-cord-like pattern was seen only with stromal nodules and endolymphatic stromal myosis. Necrosis was seen in all three lesions, whereas nuclear anaplasia was seen only with stromal sarcoma. Follow-up ranging from 2 months to 12 years (median interval: 5 years) was obtained in 23 cases. No recurrences were recorded among six stromal nodules. Uterine carcinosarcoma is a rare and rapidly fatal

. The records of 49 patients with carcinosarcoma were studied; from these studies emerged a symptom complex of vaginal bleeding and abdominal or
pelvic pain
. Eleven patients had been previously irradiated at an average of 16.4 years before they developed uterine carcinosarcoma. Celiotomies revealed more extensive tumor than could be determined by pelvic examination under anesthesia. The majority of the patients died from local recurrence in the pelvis rather than from distant metastases. The overall 5-year survival was 6%. Celiotomies are recommended to stage carcinosarcomas and guide treatment decisions.

Ascites is a rare but important complication of endometriosis because it mimics ovarian cancer. Most cases occur in nulliparous young black women and present with massive ascites. Treatment is effected by ablation of ovarian function by surgery, radiotherapy, or suppression of endometriosis by endocrine therapy. The pathogenesis is unknown. In this paper, we present a case report and review of the other 19 cases in the literature. Because of the age of most of these women, endocrine therapy is preferred rather than castration. Previously irradiated recurrent pelvic
malignancy
is refractory to most treatment modalities. Ten patients with local recurrences (six with rectal cancer; three, anal cancer; and one, anorectal melanoma) were treated with a total of 17 courses of isolated pelvic perfusion chemotherapy (12 with multiple agents) using standard hemodialysis technology. Aortic and inferior vena caval occlusion was maintained via transfemoral balloon catheters, with a single intraoperative balloon disruption. Mean pelvic-systemic drug exposure ratios were 9.8:1 for fluorouracil, 4.8:1 for cisplatin, and 4.4:1 for mitomycin C. Results were three partial responses (two patients subsequently underwent resection) and three minor responses, all in patients with a visible tumor.
Pelvic pain
was relieved in six of eight symptomatic patients (mean duration, 4 months). Using limited access, this procedure produces high pelvic-systemic concentration gradients, prolonged palliation for recurrent pelvic cancers, and increased resectability in selected patients.

To determine the subjective experiences of the effects of oral contraceptive (OC) usage upon well-being, questionnaires were distributed to 200 female students and staff members of an Australian university. 145 completed questionnaires provided information on 1) sociodemographics (mean age of 26); 2) past (45%) and current (55%) OC usage, including discontinuation because of side effects and the reason for using OCs; 3) the difference in the subject's premenstrual psychological state when using and not using Ocs (all significant results showed perceived deterioration); 4) changes with OC use in physical premenstrual symptomatology (all were negative except for the beneficial effect on

before and during menstruation); 5) the effect of OC use on overall physical and emotional well-being (a significantly deleterious effect was reported for these factors and for eating behavior, and duration of use did not reduce side effects); 6) whether the respondent believed the OC was damaging her body in any way (73% were concerned with longterm fertility, increased risk of

, had nonspecific fears, or feared cardiovascular complications); 7) any beneficial effects other than contraception attributable to OC use (83% reported such effects as reduced pain, a more regular cycle, and decreased menstrual flow); and 8) whether the women believed that medical staff had adequate knowledge of the side effects of OCs (46% believed that medical staff did not have adequate knowledge). None of these findings could be linked to type of OC used. These preliminary findings require replication in a larger study.
