This paper describes a modified preauricular surgical approach to the
TMJ
used in more than 272 joint surgeries since October 1984. During this time, only four surgical procedures have resulted in postsurgical
paresis
of the frontal branch of the facial nerve. The technique is a modification of the preauricular approaches described by Al Kayat, Bramley, and Dolwick. Relevant anatomic studies that relate to the frontal branch of the facial nerve and associated fascial layers are discussed. From findings of current anatomic dissections, a safe preauricular approach to the

is described, with emphasis on following certain landmarks.

In this paper the surgical management of
TMJ
dysfunction-ankylosis and arthropathies-is described. The surgical techniques and the necessity of wide exposure are pointed out. Only thus are correct reshaping of the condyle, repair of the disc and radical resection of ankylotic bone tissue possible. The indication for prosthetic substitution to be interpositioned in cases with discal atrophy and perforation is discussed. There were no cases of facial
paresis
and no recurrence of ankylosis. The results, in regard to the functional aspects, confirm the efficiency of the methods described.
