A rare case of cerebral alveolar
hydatid disease
in a 41-year-old female is presented. The larval mass was subtotally removed and the presence of alveolar hydatid cysts established histologically. Postoperatively, slight mental disturbance persisted but
paresis
did not develop.

The authors present 27 cases of cerebral
hydatid cyst
(CHCy) treated between 1980 and 1992. These cases of CHCy represent 2.8% of all cases of expansive nontraumatic lesions in children. Most of the patients were between 6 and 10 years of age. There was a substantial prevalence of male patients-18 cases (66.6%). Most of the children with CHCy were from rural areas. The cysts were all located in the cerebral hemispheres (none in the posterior fossa). Usually two or three lobes were affected and the cysts were most often retrorolandic. Only 8 patients (29.6%) also had pulmonary or hepatic infestation. All patients were operated on immediately the diagnosis was established. Operative mortality was very low (1 patient died immediately after surgery). There have been 11 cases of recurrence; all these patients have been reoperated on. Neurological sequelae were mainly partial and general seizures. Epilepsy developed postoperatively in five patients.
Paresis
, which usually disappears in time, was the most common motor disturbance. Two other important postoperative complications were subdural effusion and ventricular dilatation (six cases). There have been five cases of multiple recurrences (four of these patients have since died). Treatment with albendazole was used in three cases without significant results.

Hydatid disease
in childhood is a serious health problem in Turkey and other places where the parasite is endemic. An 8-year-old girl from a rural area was admitted with headache, recurrent abdominal pain, abdominal distension and hypo-aesthesia and
paresis
of the right arm. Computerised tomography (CT) scans demonstrated a large intracranial
hydatid cyst
. A
hydatid cyst
was detected in the liver also. Her mother had been treated previously for disseminated
hydatid disease
. The cerebral cyst was removed but only subtotal excision of the hepatic cyst was possible. She was treated with albendazole, has fully recovered and, after 12 months of follow-up, there is no sign of recurrence.

This is a case report of 7 years old boy who presented to us with acute onset of pain and coldness in left leg that appeared during a game and started 3 hours prior to admission in our hospital. Developmental and past medical histories were nonspecific. Physical examination revealed absence of all pulsations in left lower extremity in addition to presence of paralysis of left peroneal nerve. Complete diagnostic investigation was carried out including Seldinger angiography and occlusion of left external iliac artery was confirmed. Embolectomy was performed and
hydatid cyst
that caused arterial obstruction has been removed from left external iliac and common femoral artery. Follow up examination after 7 days confirmed presence of palpable pedal pulsation with slight lingering
paresis
of left peroneal nerve that improved with physical rehabilitation exercises. The aim is to present an extremely rare case of primary arterial
echinococcosis
and to stress upon necessity of including it into differential diagnosis of acute arterial occlusion in regions where this illness is endemic.

Simultaneous intradural, extradural, vertebral and paravetebral invasion of
hydatid cyst
, pathologic fracture, and multiple vertebral involvement are all rare encountered conditions in echinococcal infestation. A 48-year-old man who had experienced a falling down trauma, 10 years ago, and at that time, because of L1 burst fracture, undergone on Harrington rod placement, admitted in our ward for newly started urinary retention and mild
paresis
of lower extremities. With imaging analysis and during surgery, we discovered the extension of echinococcal spinal infestation extra- and intradurally after a decade of extinction. We performed a double stage circumferential reconstruction and adjuvant long term chemotherapy. We closely monitor our patient neurologically and radiologically and believe that aggressive surgical treatment and sustained cyclical albendazole therapy can increase the quality of life and life expectancy.

The involvement of spinal column in cyst
hydatid disease
is rare and hard to treat. The gold standard treatment is total removal of the cysts without rupture. However, recurrence after surgery is almost inevitable and reoperations carries technical difficulties and higher morbidity. We present a 69-year-old woman with two cystic masses at the T12 level, which compress the spinal cord causing severe
paresis
in her left leg. Under local anestesia, the cysts were aspirated and irrigated with 20% hypertonic saline solution via bilateral T12 transpedicular route. We aimed to report that percutaneous CT guided treatment should be considered as an alternative therapeutic option in case of recurrent spinal cyst hydatid.

Hydatid disease
is a parasitic infection whose etiologic agent is
Echinococcus
granulosus. Human is an accidental intermediate host and the most common site is the liver. The brain involvement is unusual and up to 75% of cases are described in the pediatric population. We present six children with cerebral
hydatid disease
admitted to the Pediatric Hospital J.P. Garrahan. All had neurological involvement on admission. The images showed single cystic lesion in the brain. They did not present involvement in other organs. Serology was negative in all cases. Medical and surgical treatment in all cases. The clinical outcome was favorable without sequelae in five of them and one had a residual
paresis
right faciobrachiocrural. This infection should be considered in the differential diagnosis of cystic tumor lesions of the central nervous system.
