Leptospirosis, the most common vasculitic
zoonosis
in the world, is characterized with jaundice and acute renal failure. However,
pancreatitis
is an uncommon complication of leptospirosis. In this report, an acute pancreatitis case due to Weil's disease has been presented. A 31-year-old female patient with high levels of glucose, blood urea nitrogen, creatinine, creatine kinase, bilirubin, amylase and lipase, has been diagnosed to have leptospirosis by the high positive result (1/800) of microscopic agglutination test against Leptospira interrogans serogroup icterohemorrhagiae. The patient has been treated with supportive and symptomatic therapy, and with penicillin G for leptospirosis. Following triple hemodialysis, all the blood biochemistry tests returned to normal on the tenth day of therapy. Leptospirosis is a disease caused by pathogenic spirochetes of genus Leptospira. It is considered the most common
zoonosis
in the world. Acute pancreatitis is a rare complication of leptospirosis (25%). We present the case of a 34-year-old male patient with severe leptospirosis complicated with acute renal failure. After 9 days from the onset of the disease, the patient developed acute necrotizing
pancreatitis
, infected from the very beginning, associated with multiple organ failure, septic shock and severe anemia. The diagnosis was clinically and biologically stated and confirmed by CT-scan. The patient underwent surgery for infected necrotizing acute pancreatitis of the head and neck of the pancreas, with left retroperitoneal expansion down to the left iliac fossa. We performed a necrosectomy with the evacuation of the tisular debris, multiple drainage of the peritoneal cavity, followed by an open abdomen with synthetic mesh. The postoperative evolution was difficult but constantly progressive. Two reinterventions were necessary. The patient left the hospital in good condition after 75 days postoperatively.

Scrub typhus is a
zoonosis
transmitted by a trombiculid mite which introduces bacteria of Orientia tsutsugamushi by its bite. The acute febrile illness is characterised by eschar at the site of the bite with maculopapular rashes and local and/or generalised lymphadenopathy. The disease is endemic in the tsutsugamushi triangle. Sikkim, a small Himalayan north-eastern state, is also not unaffected where outbreaks of the disease have been reported. The clinical spectrum of the disease ranges from mild to fatal depending on the virulence of the bacterial strain, susceptibility of the host and promptness of the treatment. In severe cases, there can be multiple organ involvement.
Pancreatitis
is a serious and unusual complication of this disease, which was seen in our presentation. A 22-year-old man, diagnosed to have scrub typhus, developed

Hydatid disease or echinococcosis, a systemic
zoonosis
is caused by Echinococcusgranulosus larvae. This is a common disease found all over the world, especially in the Mediterranean region. We report a 40 year old male with no known comorbids who came with complaints of fever with rigors and chills, right hypochondriac pain, and yellow discolouration of the sclera. A CT scan abdomen with endoscopic retrograde cholangiopancreatography (ERCP) gave a diagnosis of hydatid cyst of the liver with
pancreatitis
, cholangitis and jaundice due to involvement of the biliary tree and common bile duct ERCP was done and a stent was placed after which the patient was referred to general surgery department where the resection of cyst was performed under general anaesthesia.

was managed conservatively. We could not find any case reported in the literature, which showed manifestation of hydatid cyst of liver with
pancreatitis
, cholangitis and jaundice simultaneously, which made us report this case.

Scrub typhus, a re-emerging acute febrile
zoonosis
can present as an undifferentiated fever often complicated by potentially fatal systemic involvement. Acute pancreatitis is a rare complication of scrub typhus which can result in a marked elevation of serum lipase. We report three cases of scrub typhus who presented with acute undifferentiated fever, where a marked elevation of serum lipase occurred without any evidence of acute pancreatitis, or other causes of elevated lipase levels.
