Leptospirosis
typically presents with fever and thrombocytopenia, with or without jaundice. Acute necrotising pancreatitis and acalculous cholecystitis are rare presentations of this spirochetal infection. Here is the case of necrotising
pancreatitis
and acalculous cholecystitis associated with
leptospirosis
in an elderly patient.
Leptospirosis
was diagnosed by serological tests and abdominal CT imaging. The patient was successfully treated medically with intravenous antibiotics (imipenem and ceftriaxone) and proper hydration.

Leptospirosis
is a zoonosis caused by the spirochete
Leptospira interrogans
. Most cases of
leptospirosis
are mild to moderate, and self-limited. The course of disease, however, may be complicated by multiorgan dysfunction such as in Weil's disease. We present a case of Weil's disease with
pancreatitis
in a young Caucasian man residing in Hawai'i. Although
leptospirosis
is common in Hawai'i, few patients present with
pancreatitis
. This report of
leptospirosis
-induced
pancreatitis
should help raise awareness of clinicians to assess for
pancreatitis
when evaluating a patient with
leptospirosis
and acute abdominal pain.

Leptospirosis
is an ubiquitous anthropozoonosis particularly frequent in the tropical climate zone. It is caused by gram negative, strictly aerobic bacteria belonging to the order Spirochaetales and to the family Leptospira whose transmission usually occurs through contact with water sources contaminated with animal urine (carriers). Acute pancreatitis is a rare complication of
leptospirosis
; it is associated with high mortality rates. We report the case of a 66-year old patient in whom the diagnosis of
leptospirosis
was made after a rare complication of
pancreatitis
. Our patient was admitted with septic shock requiring hemodynamic and respiratory support with renal dialysis. The outcome was favorable.
