A 65-year-old male was operated for esophageal carcinoma. Transhiatal esophagogastrectomy with jejunostomy feeding tube was done. Orals were started on the 12th postoperative day. The jejunostomy feeding tube was removed on the 20th postoperative day. Immediately after removal of the feeding tube, a 10-12 cm ascaris was seen emerging through the jejunostomy tract. Ascaris lumbricoides can cause a variety of complications like intestinal obstruction, perforation, biliary obstruction, pancreatitis, liver abscess, cholangiohepatitis, volvulus, and gangrene, etc. Although the above-mentioned complications have been frequently reported, ascaris exit through the feeding jejunostomy tract is very rare. This case is reported here to emphasize the importance of this complication of wandering ascariasis.

Ascaris lumbricoides is a comman intestinal helminths in humans. It is a parasite which commonly affects society with a low socioeconomic status, especially in tropical and rural areas. Ascaris lumbricoides infestation can lead to serious complications because of the mobility of the worms. The parasite can cause a variety of complications like intestinal obstruction, perforation, biliary obstruction, pancreatitis, peritonitis, liver abscess, cholangiohepatitis, volvulus, and gangrene, etc. A 59-year-old female patient hospitalized with the diagnosis of mesenteric ischemia was operated on for jejunal resection. On the 6th postoperative day, a worm was noticed emerging through the nasogastric tube. Ascaris lumbricoides was determined as a result of the examination microbiology laboratory. The patient was treated successfully with one dose of albendazole 200 mg 1x2. Our case describes a clinical situation of ascariasis observed after jejunal resection and emphasizes the importance of remaining aware of this rare complication of ascariasis.

Infection with Ascaris lumbricoides is common in tropical and subtropical regions of the world. Prevalence of ascariasis is related to poverty, poor hygiene and poor fecal sanitation. This helminth usually lives harmlessly in small intestine but can also cause intestinal obstruction or perforation peritonitis which is common in childhood. Ascaris can also migrate through ampulla of vater to produce cholangitis, pancreatitis,cholecystitis and rarely hepatic abscess. Ascaris induced hepatic abscess can sometimes present with acute abdomen (like acute pyogenic abscess). We present a young adult with Ascaris induces hepatic abscess who presented and a case of acute abdomen in emergency department.

A 46-year-old previously fit and well man, travelled to Jhelum, Pakistan for 6 weeks in early 2015. Four weeks after returning to the UK, he developed sudden onset epigastric pain, vomiting and fever. C reactive protein was 232 mg/L and amylase was 2061 U/L. He was treated conservatively as pancreatitis, with peripancreatic streaking and left colic gutter fluid on a CT abdominal scan. He was discharged and later attended the Hospital for Tropical Diseases, after coughing up an 8 cm male ascaris worm. He was treated for acute ascariasis infection, complicated by resolved mild-moderate pancreatitis, a common complication in the developing world but rarely seen among travellers.

Ascariasis lumbricoides is one of the most common helminthic infestations in humans. Despite the fact that the prevalence of ascariasis in developed countries has been decreasing, biliary ascariasis can cause serious complications, such as acute cholangitis, pancreatitis, and liver abscess. Here we presented a rare ascariasis-related complication-hepatolithiasis.A 60-year-old female patient had symptoms of recurrent cholangitis. Abdominal computed tomography scan revealed left intrahepatic duct stones with left liver lobe atrophy. Endoscopic retrograde cholangiopancreatography was performed, but the stones could not be removed due to left main intrahepatic duct stenosis. The patient was treated with left hemi-hepatectomy. Unexpectedly, Ascaris ova were found on the histopathological examination. She received antihelminthic therapy orally and was on regular follow-up without any complications.Our study indicates that clinicians should be aware of biliary ascariasis in patients with hepatolithiasis, though not living in endemic areas.

Ascariasis is the commonest helminthic disease to infect humans. Due to their wandering nature, the roundworms from the second part of the duodenum migrate through the biliary opening into the hepatobiliary and pancreatic ducts. Ascariasis is the most common parasitic cause of pancreatitis in endemic region. Pancreatitis can result due to pancreatic ascariasis, biliary ascariasis or both. Ascaris is a common cause of acute pancreatitis in developing countries. The mechanism of ascariasis induced acute pancreatitis include obstruction of papilla of Vater, invasion of common bile duct, or pancreatic duct (PD). PD ascariasis is a rare diagnosis. Endoscopic ultrasound is a highly accurate method to diagnose the aetiology of idiopathic acute pancreatitis with reference to biliary and pancreatic ascariasis. Treatment usually consist of endoscopic removal of worms with dormia basket or forceps on side viewing endoscopy. Ascaris induced pancreatitis is generally mild and worm extraction is associated with rapid relief of symptoms. We present a case of PD ascariasis diagnosed on endoscopic ultrasound.

Ascariasis is a soil-sourced, second most common parasitic infection worldwide. Because of its wandering nature, it migrates from the intestine to other organs of the body like the lungs and biliary system. This results in complications such as biliary colic, acute cholecystitis, pyogenic cholangitis, liver abscesses, pancreatitis and loeffler's pneumonia. We report a unique case of an 8-year-old boy who presented with upper gastrointestinal bleed and chest infection. A 25-year-old female presented with a sore throat, fever and epigastric pain after coming from India. She had an obstructive pattern of liver function tests and an ultrasound scan abdomen showed features suggestive of acute cholecystitis and pancreatitis, with no obvious gallstones. She made no improvement with intravenous antibiotics. Subsequently, a contrast CT demonstrated a curvilinear structure within a thickened gallbladder and common bile duct. After input from multiple specialities, she had a MR cholangiopancreatography (MRCP) which further confirmed the curvilinear filling defect impacted in the gallbladder neck and proximal common bile duct. Biliary ascariasis with mild pancreatitis was diagnosed and was successfully treated with mebendazole. At one-month outpatient follow-up, her liver function tests and ultrasound scan had returned to normal. This case report discusses the radiological findings seen with biliary ascariasis using a range of imaging modalities. It also highlights the importance of the multidisciplinary team in managing a patient who presents a diagnostic challenge, in order to achieve the best patient outcome.
