The association between cumulative load (biomechanic load and exposure time integral over the entire work experience) and back pain was investigated in a group of institutional aides with physically stressful jobs. A questionnaire/interview was conducted with 161 of these institutional aides. The point prevalence of back pain in this sample was 62%.
Men
had worked a mean duration of 14.3 years and women 11.6 years at the time of the onset of the first
pain
episode. Every job performed was analyzed by the use of a two-dimensional static mathematical model. The compression and shear at the thoracolumbar and lumbosacral discs were computed by the use of a biomechanic model. Cumulative compression and shear were significantly higher in institutional aides with
pain
compared with those without
pain
(P less than 0.05-0.01). The

group was similar to the no-

group in age, weight, and height.

Referred limb symptoms (RLS) in chronic low back pain patients without signs of root affections were studied in 212 men and 126 women, aged 36-55 years, who were at work, but suffered from chronic or recurrent low back pain. RLS during the past few months were experienced by 17% daily and 22% occasionally. Previous RLS were reported by 34%, whereas 27% had never had such symptoms. There was a 3:4 distribution between symptoms in right and left legs, and 30% claimed symptoms in both legs. The distal extension of RLS into the limbs was as follows: thigh 18%, leg 37%, foot 20%, and toes 26%. The nature of RLS comprised the following:

15%, and weakness 10%. Occurrence of RLS was not related to age. In both men and women, RLS correlated with subjective disability as well as with

on bendings and palpation of lumbar spine and muscles.

with previous and present RLS had greater external rotation of the hips, but otherwise no specific physical measurements were related with RLS. RLS of both legs in women and of distal extension in men showed more findings related with back pain.

With few exceptions, use of multiple analgesic drugs achieves dubious increases in analgesia while placing elders at increased risk of the many potential adverse effects of analgesic drugs. The potential for duplication of analgesic therapy among the elderly is great due to prevalent painful chronic conditions and the variety of prescription and nonprescription analgesic remedies available. The prevalence of multiple analgesic product use and patterns of concurrent use of different analgesic categories was investigated in a geographically defined population of persons 65 years of age and older. The demographic characteristics of users of multiple analgesic drug products were examined, as were their smoking status, alcohol use, lifetime history rates of major illnesses, physical functioning,
pain
experiences, memory performance, and depressive symptoms. A substantial proportion of analgesic users reported taking multiple products in the preceding 2 weeks (14.4% of female and 10.5% of male analgesic users).
Men
who reported

in the preceding year were more likely to use multiple analgesic products. Women who experienced
pain
or limited physical functioning, or who had higher depressive symptom scores or a life-time history of ulcers were most likely to use multiple analgesic products. Thus, although some users of multiple analgesic products reported significant

, several other factors were shown to be related to the phenomenon of multiple use.

Before and after the Qi Gong, there is a normal +4 response to the Bi-Digital O-Ring Test at these acupuncture points. Similar changes were also observed on acupuncture points CV17 (Shan Zhong), CV 22 (Tian Tu), Yin Tang (at an area just between the eyebrows: the pituitary gland representation area, colloquially known as the "third eye") and GV20(Bai Hui), the entire pericardium meridian & triple burner meridian, their acupuncture points, the adrenal glands, testes, ovaries and perineum, as well as along the entire spinal vertebrae, particularly on and above the 12th thoracic vertebra, medulla oblongata, pons, and the intestinal representation areas of the brain located just above and behind the upper ear. Using these findings as criteria for evaluating the effectiveness of reaching the Qi Gong state, we were able to reproduce during the experimental trials similar changes in ourselves and the patient being treated with therapeutic effects comparable to those of the Qi Gong Master. Beneficial effects of external Qi Gong treatment given by a Qi Gong practitioner 1 to 3 times for 10-20 seconds each (although most Qi Gong masters take 3-20 minutes per treatment) often resulted in improvement of circulation and lowering of high blood pressure, as well as relaxation of spastic muscles, relief of
pain
, and enhanced general well-being, all of which resemble acupuncture effects. In order to reproduce the same procedure with others, we selected 4 children ranging between 8 and 11 years of age who had no knowledge of Qi Gong or Oriental medicine. One of these four children, the 8 year old, was able to consistently reach the same Qi Gong state after less than a half day and another child, 11, after less than 2 days. Within a week, the other two were sometimes able to reproduce the Qi Gong state but not always. Previous studies have indicated that the shock absorbency of the heel pad is significant in the pathophysiology of
pain
conditions connected with heel strike. The purpose of this study was to develop a simple clinically usable method for quantification of the heel pad shock absorbency. The results of our efforts was the HPC-device which consists of a fixing device and a transducer. The HPC-device was found suitable in the evaluation of the heel pad force/deformation characteristics. The reproducibility of heel pad deformation was 0.96. Tests of six cadaver heel pads revealed a high correlation between the HPC result and the shock absorbency found by drop tests. In a normal material (200 heel pads), it was found that the shock absorption decreased with age and that men had significantly higher shock absorbency than women.
Men
also had significantly thicker heel pads than women. However, there was no linear correlation between the thickness of the heel pad and the shock absorbency, although the thinnest heel pads had the lowest shock absorbency. The HPC-device was found clinically usable and will be used in further testing of the significance of heel pad shock absorption for the development of heel strike dependent over-use injuries.

Patients with intermittent claudication were treated with six months intensive physical training. They were instructed to walk with a speed of 6 km/h until they got complaints and then to rest for some time. This sequence was repeated until men below 65 years had walked 2 km.
Men
above the age of 65 years and women had to walk 1.5 km. This training session was performed 3 times a day. Every two months the claudication distances in a corridor and on a treadmill were measured. Out of 95 patients 65 completed the training. In 38 patients (59%) there was no substantial change in walking distance. Fourteen patients (22%) doubled their walking distance, seven (11%) could walk more than 1000 m, but their complaints remained. Six (9%) could walk more than 1000 m without claudication

. Most patients gained their result during the last 2 months of the program. After the training 48% of the patients were satisfied with their walking distance.

Low back disorders and trunk muscle function were studied by questionnaire, interview, clinical examination, and performance tests in 902 employees in the metal industry. The survey was repeated after a 10-yr interval for 654 persons. For those who were allowed to participate in the muscle function tests, the prevalences of clinical findings in the low back and of chronic lumbosacral disease were inversely associated with muscle function in both cross-sectional studies. This association seems not to be explained by the interference of
pain
with muscle function tests. There was no association between muscle function at baseline and either the 10-yr incidence of chronic low back disease or the development of low back symptoms.
Men
with poor muscle function at baseline had a slightly elevated risk for the appearance of clinical findings at follow-up. There was an inverse association between the clinical findings at baseline and muscle function at follow-up. The association between low back disorder and trunk muscle function seems to result from an effect of the former on the latter, but poor muscle function may also contribute to the risk of disorder.

The cause of femoral lysis after cemented total hip arthroplasty is unknown. Clinical aspects are poorly understood, and there have been only a few reports, with small numbers of cases, in the literature. The author analyzed common factors and clinical aspects in 51 cemented hip arthroplasties with lysis around the femoral component, in an effort to understand the cause of lysis. The process frequently starts where metal abuts against bone or where cement is deficient or fractured.
Men
are more commonly affected than women (4:1).
Pain
is the only symptom, but lysis can occur without
pain
. Lysis is progressive and always culminates in implant loosening. Progression is usually slow but can be alarmingly rapid. Twenty-six percent of cases in this series had associated acetabular loosening. After bilateral hip arthroplasty, lysis can be bilateral but usually is not. The major causative factor may be microfragmentation of any of the component parts of the artificial joint complex.
