The material includes 11 traumatic lesions of the kidney after needle biopsy, one hemangioma of the kidney with arteriovenous fistula and 4 renal exclusions for malignant hypertension. In traumatic lesions of the kidney after needle biopsy, embolization was followed by 90,9 p. cent excellent, permanent results without any marked loss of renal parenchyma and with simple post-operative follow-up. A hemangioma with congenital arteriovenous fistula required complex embolization owing to multiple pedicles and caused devascularisation of part of the lower pole of the kidney. The clinical result was satisfactory, but an arteriographic control carried out as a routine 3 months later after the embolization showed the development of newly formed vessels around the renal pelvis. However, no relapse of the hematuria occurred after a follow-up of 2 years. During the same period, spontaneous disappearance of 3 hemangiomas of the kidney sent up for embolization was noted, which led us to treat only hemangiomas which bled and prefer embolization in order to keep the kidney intact. In malignant hypertension, embolization proved efficacious and lasting in one case of small unilateral kidney. Carried out in two stages in one case of chronic nephritis with small kidneys, it proved efficacious on one side, insufficient on the other and had to be followed by nephrectomy. Finally, embolization of a remaining kidney due to chronic glomerulonephritis, technically incomplete, was followed by an uncontrollable attack of hypertension with later death of the patient. The blood supply of the kidney must thus be interrupted completely and embolization should not be undertaken unless it appears technically possible. The degree of pain following embolization suggests the possibility of peridural anesthesia.

The effects of tiapride were studied in 180 patients, including 165 with cephalalgia originating in various causes and 15 with other types of pain. 110 of the 165 patients with cephalalgia completed the study; results were good or excellent in 78 (71%), with no differences related the the cause of the headache. 13 of the 15 patients with other types of pain completed the study, with good or excellent results in 10. Tiapride was given in a daily dose of three tablets a day in outpatients, and two daily intramuscular injections in hospitalized patients. Tolerance was excellent in 109 of the 123 patients (88%). Recorded side-effects were drowsiness in 4 patients, asthenia in 3, ebrious manifestations in 3, amenorrhea-galactorrhea in 2 and constipation in 2.

Missing limb pain remains a preoccupying and worrisome problem for amputees. Approximately one-third of amputees experience missing limb pain. Although it often abates with time, this pain disables the patient in the first few months following amputation. We carried out a therapeutic trial of oral tablets of tiapride, which was given to twenty patients in three daily doses of 50 to 100 mg each. Tiapride was the sole medication in most instances. Analgesia appeared after an interval of 24 hours to 15 days. Clinical tolerance was excellent. Efficiency in terms of resolution of pain is observed in most cases.

This work is based on a review of 120 peripheral bone metastases of the upper limbs found in the literature and 10 others selected from a personal series of 469 cases of bone metastases. The authors stress the scarcity of these lesions (2%), metastases arising in bones of the forearm are usually part of a widespread osseous involvement, generally of mammary or prostatic origin. Metastases to the bones of the hand are more peculiar: 1) Those from bronchogenic carcinoma account for 50%. 2) Resultant soft tissue necrosis creates swelling, redness, heat and pain, which can mimic acute osteomyelitis. 3) X-rays reveal isolated osteolysis affecting metacarpals, phalanges, or more rarely carpal bones. Distal location of those metastases does not modify therapeutic management amputation is the best effective palliative treatment and must be discussed every time there is a concomitant skin ulceration, or if bone metastasis is unique or if the primary tumor is cured or accessible to treatment.

A new case of analbuminemia in a fifty-four-year-old French woman is described. The patient was admitted to a hospital cardiology department for thoracic pain. Consanguinity between the patient's parents was established. Proteins were studied both during the acute phase and the subsequent remission. Total serum protein concentrations were stable, around 5.3 g. Hypoalbuminemia was compensated initially by alpha-2-globulins and subsequently by beta globulins. Increases in serum cholesterol and serum beta lipoproteins were recorded. Hydroelectrolytic balance was normal. Orthostatic renin and aldosterone release was significantly increased, denoting response to lowered hydrostatic pressure. A minor increase in serum thyroxin was recorded, probably because of the increase in thyroxin binding globulin.

Articular manifestations occur in approximately 1% of cases of leprosy, sometimes at onset. They consist in a highly inflammatory polyarthritis, fairly similar to that seen in rheumatoid polyarthritis. They often herald a reactive leprous exacerbation and are dependent upon immunologic disturbances in brittle leprosy (mainly lepromatous). Joint pain should be differentiated from neurologic pain resulting from peripheral neuropathy which is often concomitant. Leprosy should be considered among the causes of polyarthritis, especially in immigrants, but also in residents who have travelled to areas where leprosy is endemic.

Two new cases of spondylitis associated with endocarditis are reported. Approximately 30 cases have already been published in the medical literature. Although this association is infrequent, the possibility of its occurrence calls for repeated auscultation in patients with spondylitis and focal roentgenograms in patient with bacterial endocarditis who develop vertebral pain.

The alcoholic patient is usually anxious. Anxiety is increased by withdrawal. The anxiety-relieving effect of tiapride was studied in 20 alcoholics, with a mean age of 44 years. 18 patients were male. Alcoholism was chronic in 18 cases and paroxystic in two. At the time of withdrawal each patient was given 3 intramuscular injections daily for 7 days, then 3 tablets per day. Results, which were evaluated according to the Hamilton score, were excellent in 9 cases, good in 10 and poor in 1: no failure was recorded. The symptoms which responded best were fear, somatic and psychic manifestations of anxiety, depressive feelings and sleep disturbances. Concomitantly, digestive disorders, anorexia, tremor and pain were alleviated. Tolerance was excellent: no neurologic, digestive, cardiovascular or biologic manifestations were recorded. In caring for alcoholic patients, the critical time of withdrawal is undeniably facilitated by the use of tiapride.

Five cases of chondrosarcoma are reported. Some specific characteristics of this tumor are recalled : prevalence is approximately 10% of primary malignant bone lesions, onset usually occurs between 40 and 70 years of age, prevalence is higher in males, elective sites are the proximal long bones and pelvis, pain is often the initial symptomatology, the patient's general condition remains satisfactory over a long period of time, sedimentation rates have no diagnostic or prognostic value, medullary arteriography and computed tomography are helpful for evaluation the topography and the extension of a vertebral localization, and the course is protracted. Prognosis is dependent on both the surgical possibilities and the characteristics of the tumor (site, size, histologic grade). Conversely to metastases which occur electively in grade III tumors, recurrences are not necessarily correlated with the histologic grade; they often occur in tumors whose localization precluded radical surgery and may be the occasion of development into a higher grade of malignancy. Inasmuch as it is feasible, carcinologic surgery is the only effective therapy; it carries hope for 10 year survival times in approximatley 70% of patients.

We report on the case of a 16-year-old male patient who presented with a peripheral neuropathy remarkable by the severity of pain, the proximal involvement and the association with an underlying myelopathy. All these symptoms coincided with an acute exacerbation of Crohn disease (regional enteritis) involving mainly the duodenojejunal segment, and were ascribed to a major folic acid deficiency, with total recovery following supplementation. In this connection we recall the various neurological symptoms induced by folic acid deficiency. We point out how difficult it is to prove the responsibility of such deficiencies in the production of neurologic diseases, mainly because of the possible intrication with other pathogenic factors: combined deficiencies or direct action of the factor responsible for the deficiency on the nervous system. Guidelines for solving these difficulties are suggested.
