Defensive and alimentary conditioned reflexes were studied on normal cats and cats with neural isolation of the neocortex. The cardiac component of the conditioned reflexes is elaborated in decorticated animals at a similar rate and is as steady as in normal cats. Motor conditioned reactions in decorticated cats are difficult to elaborate. Both in normal and decorticated cats unconditioned pain stimulation evokes tachycardia, while a conditioned signal, paired with this unconditioned stimulus, produces bradicardia. Ban assumption has been made that the primary conditioned reaction consists in the appearance of a certain emotional state which changes the cardiac rhythm in a typical way.

The aim of the study was to measure incidence of oral impacts on daily performances and their related features in a low dental disease population. 501 people aged 35-44 years in 16 rural villages in Ban Phang district, Khon Kaen, Thailand, were interviewed about oral impacts on nine physical, psychological and social aspects of performance during the past 6 months, and then had an oral examination. The clinical and behavioural data showed that the sample had low caries (DMFT = 2.7) and a low utilization of dental services. 73.6% of all subjects had at least one daily performance affected by an oral impact. The highest incidence of performances affected were Eating (49.7%), Emotional stability (46.5%) and Smiling (26.1%). Eating, Emotional stability and Cleaning teeth performances had a high frequency or long duration of impacts, but a low severity. The low frequency performances; Physical activities, Major role activity and Sleeping were rated as high severity. Pain and discomfort were mainly perceived as the causes of impacts (40.1%) for almost every performance except Smiling. Toothache was the major causal oral condition (32.7%) of almost all aspects of performance. It was concluded that this low caries people have as high an incidence of oral impacts as industrialized, high dental disease populations. Frequency and severity presented the paradoxical effect on different performances and should both be taken into account for overall estimation of impacts.

The sensitivity of the Median Nerve Stress Test (Stress Test) described by La Ban et al. and performed hyperextending for one minute the supinated wrist and the distal interphalangeal joint of the index finger and looking for pain in the proximal forearm was evaluated in 140 arms with Carpal Tunnel Syndrome confirmed electrophysiologically. The Stress Test was positive in 60 hands (42.8%), the Phalen's sign in 79 (56.4%) and the Tinel's sign in 59 (42.1%). Hypoaesthesia to pinprick in the distribution of the median nerve was found in 45 hands (32.1%) and weakness or hypotrophia of thenar eminence in 17 (12.1%). In spite of his low sensitivity, in some cases the Stress Test was the only clinical positive sign and, in addition to electrophysiological examination, may be helpful in clinical practice.

Peripheral T cell lymphomas-unspecified (PTCL-U) is an uncommon malignant tumor, accounting for 5%-7% of non-Hodgkin's lymphoma. Clinical feature of a case of PTCL-U was investigated and the optimal treatment protocol was proposed. The clinical presentation of this case was progressing limbs weakness with sudden paralysis. Multiple superficial lymph nodes swelling and decreased sensation in all modalities below T10 level were found in physical examination. Bilateral leg paralysis (Grade 0/5) with high muscle tension, overactive knee reflex, bilateral Babinski sign (+) were present. Magnetic resonance imaging(MRI) showed the mass, which was around spinal cord located in T6-T8 vertebra canal, with intense enhancement after the administration of gadolinium diethylenetriamine penta-acetic acid. Abnormal signal in multiple centrum and accessories could also be found in MRI scan. Initial diagnosis was lymphoma, multiple systems involved. Laminectomy and epidural tumor resection were performed immediately. The dural sac was narrowed obviously by surrounding tumor. The result of pathological examination was PTCL-U. After operation, the level of pain sensation was descent, and touch sensation recovered. The patient received chemotherapy and his muscle strength was partly recovered in 4 months. Early diagnosis of peripheral T cell lymphomas-unspecified was difficult and easily misdiagnosed with poor prognosis. A review of literature showed central nervous system lesions occurred in advanced stage. However, the report of representation of intraspinal mass as initial symptom like the case was rare. For acute paraplegia, laminectomy and tumor resection should be performed as soon as possible. Regular radiotherapy and chemotherapy should be considered after operation. However, the value of local chemotherapy need to be further investigated.

The patient is a 48-year-old woman. She came to our hospital because of consistent colporrhagia for one month. Her first menstrual came when she was 17 years old, her last menstrual period (LMP) was on March 23, 2005. When she came to our hospital, she looked weak and anemic. Temperature (T): 36.6 degrees C; pulse(P): 80 counts per minute; respire(R): 19 counts per minute; blood pressure (BP): 130/90 mmHg(1 mmHg=0.133 kPa). Physical examination showed that her vagina was smooth, there was some lifeblood in it; the cervical was hypertrophy; the womb was anteposition; there was no pressure pain; the two side adjuncts were normal. The laboratory examination showed that the white blood cell (WBC) was 3.40 x 10(9)/L the red blood cell (RBC) 3.14 x 10(12)/L the hemoglobin (HGB) 88.0 g/L and the platelet (PLT) 206 x10(9) L. B ultrasound showed that the womb was uniformity aggrandizement. The pathology showed that the endometrium was glandular hyperplasia. The final diagnosis was dysfunction womb hemeorrhage. After the uterine curettage and hormonal regulation, there remained some hemorrhage. The ovarine arterial embolization (OAE) was operated on June 30, 2005. The angiography showed the two side internal lliac arteries had no uterine artery. The two side ovarian arteries were stout. The balance stage showed that the womb's blood supply came from ovarian artery. The consistent colporrhagia stopped after the OAE with gelatin sponge through 3.0F SP microcatheter. After 18 months' follow up survey, the woman's menstrual halted, and she was satisfied with it.

A 35-year-old woman patient, complained of intermittent pain in the left flank for a week. Abdominal ultrasound and computed tomography scan revealed a left renal mass. Nephrectomy was performed, and a final diagnosis of adult Wilms' tumor was made based on histopathology and immunohistology. Chemotherapy was conducted for 4 times with dactinomycin D, vincristine and doxorubicin after surgery. At the time of the last recheck, the patient was alive with no evidence of disease. Adult Wilms' tumor is the exceptional malignant renal tumor. Only 1%-2% of Wilms' tumor is diagnosed in adult patients. Because of unspecific tumor symptoms in adults, the diagnosis is frequently made by histology. The diagnosis of adult Wilms' tumor is difficult to make preoperatively because diagnostic imaging only confirms the presence of a renal mass. Stage and histology are the clinical factors guiding the selection of postsurgical treatment and prognosis. Having retrospected 6 cases of adult Wilms' tumor between 1950 and 2007 and reviewed related reports, we conclude that the proper strategies of adjuvant treatment as applied to childhood Wilms' tumor patients after surgery can conspicuously improve the outcome in adult patients.

The temperature-sensitive transient receptor potential (TRP) channels, is also called thermo TRP, including TRPV1, TRPV2, TRPV3, TRPV4, TRPM8 and TRPA1, which are expressed in sensory neurons and non-neuronal cells (e.g.keratinocyte, mast cell) of the skin. Thermo TRP channels are activated/sensitized by physical and chemical mediators, which participate in thermosensation and thermoregulation, so that they are key players in pruritus or pain pathogenesis. Thermo TRP channels are also involved in cutaneous neurogenic inflammation, thus they are regarded as molecular targets for future therapy in skin inflammation, pruritus and pain. In addition, following a basic syntax and molecular substrate of nociception and pruriception established by TRP channels-centered concept, the sensory categories can be distinguished and re-defined. Thermo TRP channels should be taken into account when analyzing the pathogenesis and management of itch or pruritic dermatosis.

Voltage-gated sodium channels are critical for the generation and conduction of nerve impulses. Recent studies show that in primary sensory neurons, the expression and dynamic regulation of several sodium channel subtypes play important roles in neuropathic pain. A number of SCN9A (encoding Nav1.7) gene point mutations are related with human genetic pain disorders. Transgenic and specific knockout techniques have revealed that Nav1.3, Nav1.8, Nav1.9 are important for the development and maintenance of neuropathic pain condition. Specific blockers of these sodium channels have been demonstrated to be effective in alleviating allodynia and hyperalgesia. Here we reviewed the roles of sodium channels in neuropathic pain, which may be applicable for the development of new drugs with enhanced efficacy for neuropathic pain treatment.

It may play a role in the development and maintenance of visceral pain and hypersensitivity states, which has generated a major interest in the development of specific vanilloid antagonists. In this review, a summary of the contribution of TRPV1 in the development of visceral hypersensitivity in functional dyspepsia and irritable bowel syndrome is presented with the structure, location, and related molecular mechanisms.

To discuss the diagnosis and therapy of congenital vesicovaginal fistula (VVF). We reported 1 case of pregnancy and congenital VVF and summarized the pathogenesis and characters. Congenital VVF is extremely rare and characterised by continuous urinary leakage,cyclical hematuria with severe suprapubic pain. It is always associated with other urinary tract abnormalities, urinary tract infection and stone. The pathogenesis is related with genetic, environment, drugs, radiation and incomplete fusion of mullerian. The diagnosis relies on clinical manifestation and radiological examinations.Surgical resection is the key to treatment. Pregnancy and congenital VVF is extremely rare and always misdiagnosised associated with other urinary tract abnormalities. Early diagnosis and making sure the location, size and relationship with other tissues are important. Reasonable preoperative preparation, surgical and postoperative surgical care are the key for treatment.
