Scoliosis and kyphosis are spinal deformities that are commonly detected in the adolescent years. Scoliosis has many etiologies but is usually idiopathic. The natural history concerns pulmonary restriction, back pain, and self-image. Young children with large curves are at the greatest risk for progression. Treatment includes observation, bracing, or surgery. Kyphosis is a pathologic increase in sagittal curves of the spine. Scheuermann kyphosis is a distinct category of deformity that is diagnosed on the basis of the radiographic criteria of 3 adjacent vertebral levels with 5 degrees of wedging. The deformity is the main concern for teenage patients and their families, whereas
pain
is the main complaint in adults.

The treatment of foot problems in the adolescent should be nonsurgical whenever possible. Surgical treatment should be considered when medical treatment does not relieve
pain
. Surgical corrections to prevent problems in adulthood may cause complications and lead to an undesirable outcome. This article highlights treatment for overriding fifth toe, juvenile bunion, Morton toe, Freiberg infraction, stress fractures of metatarsals, accessory navicular, tarsal coalition, plantar fascitis, pes cavus, and pes planus. Significant deformities generally require treatment before ambulation, and innocent deformities usually resolve spontaneously.

Anterior knee-
pain
syndrome would best be defined as a painful condition that arises in or around the patellofemoral joint and is insidious in onset and bilateral, with no macroscopic gross pathology. Anterior knee pain as a descriptive term would define the need to search further for a specific cause of
pain
, because neither patellar cartilage damage nor malalignment would necessarily be correlated to
pain
. The examining physician should be aware that specifically in adolescents the higher chance would be that, eventually, no clear-cut pathology would be found; thus, great caution should be taken before diagnosing anterior knee-
pain
syndrome as existing unilaterally. It could be claimed, following the above, that as opposed to the term "anterior knee pain," the term "anterior knee-
pain
syndrome" is an exclusion diagnosis that can be applied after macroscopic pathology has been ruled out.

Reflex sympathetic dystrophy is defined as chronic musculoskeletal
pain
and autonomic dysfunction. It is a difficult diagnosis to make, and the adolescent often sees many specialists before arriving at the correct diagnosis. In this article I review reflex sympathetic dystrophy and reflect on the differential diagnosis, pertinent medical history, personal characteristics of patients with reflex sympathetic dystrophy, physical examination, and laboratory evaluation. Principles of management are considered, including physical therapy, pharmacology, psychological therapy, and alternative therapies. Accurate diagnosis and management are critical for not prolonging the adolescent's and the family's suffering. It is important to provide aggressive physical therapy, stress management, relaxation training, and close follow-up. It is also critical to avoid immobilization, surgery, or invasive procedures and unnecessary tests.

The German acupuncture trials
ART
and GERAC have shown that acupuncture and sham or minimal acupuncture were equally effective in the reduction of chronic pain symptoms. These results have prompted an ongoing discussion as to whether acupuncture exerts its effects through a placebo response. Increasing knowledge about the neurobiology of
pain
and its intrinsic control suggests a combination of acupuncture-specific neurophysiologic effects combined with effects that match those of expectation-induced placebo analgesia.

When one thinks of the failures of
ART
one naturally thinks of the suffering that they generate; but behind this
pain
, there is the suffering of infertility and, behind this latter, other distress we showed at the time of a recent research that they could play a part in infertility itself. The psychotherapy will help assume the failures, so that the traumatisms do not come to solidify in an impossible mourning, covering non elaborate former grieves. A sublimation of the desire of reproduction can make it possible for the unfertile man to peacefully consider other forms of paternity. For it to be possible, it is necessary that the subject should not be in the refusal of the psychic effraction, which the announcement of its infertility produces. When it is the case, to put on the couple on psychological assistance may help to restore a share of the narcissistic wounds; more particularly the one related to the suffering of the couple that often comes to be added to the wound of infertility.

As with any athlete, the dancer has a high risk for injury. Most studies carried out relate to classical and modern dance; however, there is a lack of reports on injuries involving other dance techniques. This study is an attempt to determine the differences in the incidence, the exposure-related rates, and the kind of injuries in three different dance techniques. A prospective study about dance injuries was carried out between 2004 and 2007 on students of modern, Mexican folkloric, and Spanish dance at the Escuela Nacional de Danza. A total of 1,168 injuries were registered in 444 students; the injury rate was 4 injuries/student for modern dance and 2 injuries/student for Mexican folkloric and Spanish dance. The rate per training hours was 4 for modern, 1.8 for Mexican folkloric, and 1.5 injuries/1,000 hr of training for Spanish dance. The lower extremity is the most frequent structure injured (70.47%), and overuse injuries comprised 29% of the total. The most frequent injuries were strain, sprain, back pain, and patellofemoral
pain
. This study has a consistent medical diagnosis of the injuries and is the first attempt in Mexico to compare the incidence of injuries in different dance techniques. To decrease the frequency of student injury, it is important to incorporate prevention programs into dance program curricula. More studies are necessary to define causes and mechanisms of injury, as well as an analysis of training methodology, to decrease the incidence of the muscle imbalances resulting in injury.

Endometriosis causes pelvic pain and infertility. Infertility results from effects of endometriosis exerted in the pelvic cavity, in the ovaries and/or on the uterus. Medical treatment effective on
pain
and at preventing disease recurrence following surgery is of no use for improving the chances of conceiving naturally. Surgery however improves the chances of conceiving in the 12-18 months afterward. Endometriosis through extension of the disease to the ovaries may harm ovarian response to COS needed in
ART
. Surgery for endometrioma(s) may further reduce ovarian responses to COS in case of endometriosis. Remarkably however, reduced ovarian responses due to endometriosis are not necessarily associated with reduced oocyte quality and
ART
outcome. Pre-
ART
treatment with oral contraceptives (OC) improves
ART
outcome in case of ovarian endometriosis particularly, if endometriomas are present at the time of oocyte retrieval. This measure requires however that a proper OC-FSH/hMG interval is respected and that "LH" effects are provided during the ovarian stimulation, using either hMG or small doses of hCG. These latter precautions prevent the adverse outcome reported in case of pre-
ART
use of OC when ovarian stimulation is conducted using r-FSH exclusively.

The Life and Livelihood Study was designed to describe and understand the experience of women musicians treated for breast cancer. This report focuses on Phase I of the study, a web-based survey that examined subjects' physical symptoms and side effects following breast cancer treatment. Subjects were recruited nationally, using advertisements in musicians' publications and presentations at national meetings. Subjects were asked about specific side effects or symptoms they had experienced, their severity and duration, and the effects of symptoms on their capacity to make music. Subjects were also asked what aspect of their breast cancer treatment they associated with each symptom and were invited to provide comments. A total of 321 individuals logged on: 100 met all inclusion criteria. Of these, 90 completed the entire survey. Commonly reported symptoms included fatigue (70%), problems with cognition (53%), limitations in upper body movement (51%), and
pain
(45%). Many reported that their symptoms were of moderate or greater intensity, and that they persisted for >12 months or were ongoing. The survey documented that many subjects experienced diminished capacity to function as musicians, especially due to
pain
, limitations in upper body and extremity movement, numbness in the chest and/or arms, contracture/fibrosis, and shortness of breath. These findings are consistent with emerging studies that describe long-term effects of breast cancer treatments. In planning for breast cancer treatment, rehabilitation and survivorship, consideration should be given to how treatment is likely to affect fitness for ongoing professional work.

Musculoskeletal
pain
is known to be prevalent among musicians. Unfortunately, there are a lack of standard measures to quantify perceived
pain
in this population. The principal objective of the present study was to develop a self-reported questionnaire targeting musculoskeletal
pain
that is specific to musical activity. The Musculoskeletal
Pain
Questionnaire for Musicians (MPQM) is composed of 10 items investigating diverse areas related to musculoskeletal
pain
, divided into three components: a set of items related to disability associated with
pain
(4 items, component 1), a second one related to
pain
intensity (4 items, component 2), and a third one related to the frequency and duration of
pain
episodes (2 items, component 3). Thirty-one professional musicians, from the province of Quebec (Canada), entered the study and answered to the MPQM. Data collected from the MPQM was submitted to a principal component analysis. It found that results from the 10 items of the questionnaire were structured around three factors:
pain
-related disability (32.71% of variance),
pain
intensity (25.42% of variance), and frequency and duration of
pain
(18.2% of variance). Convergent validity was also tested, and an adequate correlation was obtained between the MPQM and the Chronic Pain Grade Questionnaire (r = 0.65, p = <0.01). Internal consistency for the whole instrument was measured and supported by a Cronbach's alpha of 0.768. Because the MPQM shows adequate psychometric characteristics, it is believed that it could be helpful in research on the correlates of musculoskeletal
pain
in musicians.
