Cortisol is a key stress hormone that is implicated in a variety of physiological responses. Attenuated Cortisol Awakening Response (CAR) is associated with many negative health outcomes, but little research has investigated CAR and pain. The current study examines the association of CAR with experimental acute-pain ratings in healthy men and women. Attenuated CAR was related to greater pain intensity and unpleasantness ratings. Future research should examine this association across various pain populations.

Remembering painful incidents has important adaptive value but may also contribute to clinical symptoms of posttraumatic stress disorder and chronic pain states. Because glucocorticoids are known to impair memory retrieval processes, we investigated whether cortisol affects recall of previously experienced pain in healthy young men. In a double-blind, placebo-controlled crossover study, 20 male participants were presented pictures, half of them combined with a heat-pain stimulus. The next day, the same pictures were shown in the absence of pain. Cortisol (20 mg) administered 1h before retention testing reduced recall of explicit contextual pain memory, whereas it did not affect pain threshold or pain tolerance.

Massage therapy has been demonstrated to be effective during pregnancy. Women who received massage therapy reported decreased depression, anxiety, and leg and back pain. Cortisol levels decreased and, in turn, excessive fetal activity decreased, and the rate of prematurity was lower in the massage group. In a study of labor pain, women who received massage therapy experienced significantly less pain, and their labors were on average 3 h shorter with less need for medication. An underlying mechanism we have been exploring is that these effects are mediated by increased vagal activity. This likely occurs by the stimulation of pressure receptors that are innervated by vagal afferent fibers, which ultimately project to the limbic system, including hypothalamic structures involved in autonomic nervous system regulation and cortisol secretion.

This study assessed the efficacy of ketoprofen for mitigating pain following dehorning with an electric cautery iron. Forty Holstein heifer calves, 4 to 8 wk of age, were randomized to receive a lidocaine cornual nerve block and either an injection of ketoprofen (3 mg/kg IM) or physiological saline, 10 min prior to dehorning. Cortisol was measured from serum obtained 10 min prior to dehorning and at 3 and 6 h post-dehorning. Calf behavior was video-recorded between 0 to 2, 3 to 5, and 6 to 8 h post-dehorning, and frequency of ear flicks, head shakes, head rubs, lying, standing, feeding, and grooming were recorded. Finally, 24-h intake of calf starter was recorded. There was no effect of treatment on cortisol (P > 0.1); however, ketoprofen-treated calves displayed less ear-flicks and total head behavior (P < 0.05), and tended to consume more starter (P = 0.09) than control calves. Ketoprofen is effective for mitigating behavioral effects of postsurgical pain following dehorning in 4- to 8-week-old calves.

The aim of the No Pain in Labour (NoPiL) study was to evaluate the stress and clinical outcome of infants vaginally born without maternal analgesia and after maternal epidural or systemic analgesia. We studied 120 healthy term infants, 41 in the no analgesia group, 38 in the epidural analgesia group, and 41 in the systemic analgesia group. Cortisol, beta-endorphin, oxidative stress markers (ie: total hydroperoxide (TH) and advanced oxidation protein products (AOPP)), interleukin-1beta (IL-1beta), and interleukin-8 (IL-8) cytokines were measured in arterial cord blood samples. Infants in the 3 groups had similar Apgar score, cord blood pH and occurrence of hypoglycaemia, hyperbilirubinemia, and respiratory depression. Cortisol and endorphin plasma levels did not differ in the groups, nor did TH and AOPP values. IL-1beta and IL-8 cytokine were higher in infants born after maternal epidural analgesia than in other groups. Short-term outcome and stress were similar in infants vaginally born without maternal analgesia and after epidural and systemic analgesia. The possible implications of the highest interleukin levels in the epidural analgesia group deserve further study.

This study evaluated level of salivary cortisol and perceived burden, stress and health of mothers and primary caregivers of children (4-11 years of age) with cerebral palsy (purpose group, n=37) and those for mothers of children of the same age without developmental problems (control group, n=38). Anthropometric and socioeconomic data were collected from the participants, who also completed the perceived stress questionnaire, the Burden Interview and the 36-Item Short Form Health Survey (SF-36). Cortisol level was assayed in saliva samples collected at various times in a single day and the area under the cortisol curve was then determined. Both groups presented low socioeconomic level and high, although equivalent, perceived stress index. However, the purpose group showed lower cortisol levels, as well as lower scores for many of the SF-36 domains related to physical well-being (physical functioning, role-physical, vitality, and general health) and social functioning. Nevertheless, bodily pain was also reported to be lower. For the control group, the area under the cortisol curve correlated negatively with mental health and social functioning. For the purpose group, where the burden is greater, no such correlation was found. It was concluded that mothers of healthy children leaving in unfavorable socioeconomic conditions face high levels of stress with the hypothalamus-pituitary-adrenal cortex axis function preserved. However, to the mothers of children with cerebral palsy, who live in even worse socioeconomic conditions and also have the burden of caring for a disabled child, the level of stress was overwhelming, to an extent that it impaired the hypothalamus-pituitary-adrenal cortex axis function, as well as reflecting negatively on certain aspects of their physical and psychological well-being. This must receive consideration during the treatment of the child, an approach which is in line with present day tendencies towards family-centered models of assistance to disabled children.

This study investigated the use of sprayed intraperitoneal bupivacaine to relieve postoperative pain behavior and biochemical stress response after laparoscopic ovariohysterectomy (LOVH) in dogs. Sixteen sexually intact female dogs were randomly assigned to two groups. The sprayed intraperitoneal bupivacaine (SIB) group received 4.4 mg/kg of sprayed intraperitoneal bupivacaine diluted to 0.25% with an equivalent volume of saline after pneumoperitoneum. The control group received 1.76 mL/kg of saline in a similar fashion. Both groups received preoperative periportal 5% bupivacaine (1 mL) before incision. Postoperative pain was measured using the short form of the Glasgow composite measures pain scale (CMPS-SF, 0-24). Serum cortisol and glucose concentrations were measured preoperatively and 0.5, 1, 2, 4, 6, 12, and 24h postoperatively. The SIB group had significantly lower CMPS-SF compared to the control group 1, 2, 4, 6, and 12h after the operation. Cortisol concentrations were significantly increased from preoperative concentrations in the control group at 0.5, 1, 2, and 4h post operation and at 0.5 and 1h post operation in the SIB group. No significant differences were seen in serum glucose within each group. This report suggests that the use of sprayed intraperitoneal bupivacaine can be used as part of a multimodal approach for pain management after LOVH in dogs.

Shoulder and neck pain (SNP) and fibromyalgia syndrome (FMS), two musculoskeletal conditions of unknown pathogenesis, share some common features in terms of altered neuroendocrine responses, pain and stress perception. However, the pain distribution in SNP is localized, whereas in FMS is more widespread. Because regional musculoskeletal pain may represent an intermediate stage along a continuum towards widespread musculoskeletal pain we compared the cortisol awakening response (CAR) in women with SNP with the CAR in FMS patients and healthy controls (HC) in a controlled hospital-hotel setting. The aim of the study was to investigate whether SNP is related to a deviant regulation of the hypothalamic-pituitary-adrenal (HPA) axis. Eighteen women with SNP, 29 female FMS patients, and 27 female HC participated in the study. Cortisol samples were collected upon awakening, 30 and 60 min later. Questionnaires measuring pain levels, sleeping problems, perceived stress, and psychological characteristics were administered to the participants. Compared with HC, women with SNP had a tendency towards higher cortisol levels, whereas FMS had lower cortisol levels. Adjustment for potential confounders did not influence the results. Women with SNP and FMS patients reported more health complaints, pain, and perceived stress than the HC, but women with SNP were less affected than the FMS patients. Women with SNP showed a tendency towards an elevated HPA axis activity compared with HC. The current findings may indicate that the hypercortisolism in regional musculoskeletal pain represent an intermediate stage towards the development of a hypocortisolism in widespread musculoskeletal pain.

Physiological resonance between individuals is considered fundamental to the biological capacity for empathy. Observers of pain and distress commonly exhibit increases in reported distress, autonomic arousal, facial mimicry, and overlapping neural activity. An important, unstudied question is whether physiological stress can also resonate. Physiological stress is operationalized as activation of the hypothalamic pituitary adrenocortical (HPA) and sympatho-adrenomedullary (SAM) axes. People often report an aversive state resulting from the stress of another, but this could be conveyed through resonating arousal or distress, without activating the physiological stress response. Physiological stress is particularly important to examine since it commonly occurs chronically, with known negative effects on health. Salivary cortisol and salivary alpha-amylase (sAA) were measured in both speakers and observers during a modified Trier Social Stress Test (TSST) to assess activation of the HPA and SAM axes (respectively). Cortisol (but not sAA) responses resonated between speakers and observers. The cortisol response of observers increased with trait empathy and was not related to the speaker's subjective fear or distress. This study provides a novel method for examining physiological resonance, and indicates that we can indeed catch another's physiological stress, suggesting a specific health risk for those in the social network of stressed individuals.

The authors carried out a prospective study of the invasive nature ofaortofemoral reconstruction performed via the standard routine laparotomy (StLT, Group I, n 10) and minilaparotomy (MLT, Group II, n = 10). It was determined that the objective criteria for the invasive degree of the intervention were less pronounced in the group of the patients operated on through the MLT, with the main differences manifesting themselves to a greater extent in the early postoperative period. Despite statistically significant differences of the creatinphosphokinase (CPK) level as a marker of a parietal injury (562 U/I in Group 1, 243 U/I in Group II, P = 0.005), the concentration of Cortisol increased dramatically during the operation (by 288 and 162%, respectively) with a statistically insignificant deference between the groups (P = 0.08). Alterations in the vegetative status (determined by cardiointervalography) in the intraoperative period were ofa variously directed variable character, not differing between the groups. However, normalization ofthese parameters in the postoperative period occurred more rapidly in MLT. Group IH patients were noted to restore earlier the function of the gastrointestinal trapt with a less pronounced pain syndrome tested by the visual analogue scale, especially on postoperative days 2 and 4 (p<0.01 and. Hence, reconstruction of the infrarenal aorta through the MLT is characterized by a lesser invasive nature, with the above mentioned differences being more pronounced in the early postoperative period, since the main intraoperative factors of the invasion depend to a lesser extent on the approach size.
