The degree of pain produced by Amipaque 260 and Isopaque 260 at intraosseous phlebography in patients with varicose ulcerations in the ankle region was compared. Amipaque produced less pain than Isopaque. No complications or side-effects occurred during the observation period of about one year.

The authors report their initial experience with Amipaque (metrizamide) in hysterosalpingography. Amipaque was compared to Isopaque Cerebral in 37 patients, using a double-blind design. Excellent radiographs were obtained with both agents. No statistically significant difference in discomfort and pain was found up to 24 hours after the procedure. However, the aftereffects indicate that Amipaque might be less irritating to the pelvic peritoneal cavity.

In two studies 267 consecutive patients with suspected venous thrombosis were examined 125I-fibrinogen uptake test at phebography. In 161 patients meglumine metrizoate (Isopaque Cerebral) and in 106 patients metrizamide (Amipaque) was used as contrast medium. The quality of the examinations was equal in the two groups. During the examinations most of the patients examined with meglumine metrizoate had symptoms from the calf described as a cramp-like pain. Only few of the patients examined with metrizamide had such symptoms. In the two groups 47 patients examined with meglumine metrizoate and 41 examined with metrizamide had normal phlebography as well as initially normal 125I-fibrinogen uptake test. After phlebography, 29 of the patients in the first group had a significant rise in fibrinogen uptake while such a rise was not found in patients from the second group. Rephlebography showed fresh deep vein thrombosis in 7 out of 9 patients examined with meglumine metrizoate indicating that thrombosis could be provoked in as many as about half the patients with this high osmolar contrast medium. We have now used metrizamide in about 400 patients without any side-effects.

Amipaque 280 mg I/ml was compared to Isopaque Cerebral 280 mg I/ml for common carotid injection in twenty patients in a double blind trial. Simple comparison was also performed in examinations with selective external carotid injections. The reaction of the patients to the common carotid injection was mild on the whole but Amipaque caused significantly less discomfort than Isopaque. Selective external carotid injection of Amipaque caused very little discomfort while most patients complained of severe pain when Isopaque was used. No serious side-effect was observed.

In 20 patients the non-ionic Amipaque (metrizamide) and the ionic Isopaque Cerebral (meglumine/calcium-metrizoate) were injected into the femoral, brachial internal iliac or external carotid arteries. Strain gauge plethysmography of the lower leg demonstrated less increase in blood flow following injections of Amipaque (25%) than following injections of Isopaque Cerebral (95%). None of the 20 patients complained of pain following injection af Amipaque; 15 experienced pain following Isopaque Cerebral.

The arthrographic image quality and relative morbidity resulting from use of Omnipaque 300 (iohexol), Hexabrix 320 (ioxaglate sodium meglumine), and Isopaque Coronar 370 (metrizoate) were compared in a prospective double-blind study performed with 120 patients. Radiographs obtained 2, 5, 10, 15, 20, and 25 minutes after injection were judged for diagnostic quality. Relative morbidity was evaluated by the physician during the examination and later by the patient via a questionnaire. Hexabrix demonstrated the best and most persistent diagnostic quality over serial radiographs (P less than .05). Omnipaque caused significantly less postprocedural pain (P less than .05). The other types of discomfort measured did not indicate statistically significant differences in morbidity resulting from the three contrast agents.

Two water-soluble contrast media for hysterosalpingography--one with high viscosity (Perjodal H viscous) and one with low viscosity (Isopaque Amine) were compared with special reference to the development of pain in 61 patients. Patients examined with Isopaque Amine experienced significantly less pain and this contrast medium therefore is suggested as a suitable agent for hysterosalpingography in the investigation of sterility.

Next to tumour and pain, discharge from the nipples is the most common reason for remitting women to a breast clinic, and comprises 3-6% of the total cause of remittance. We describe a technique for localizing a duct system, where a defect is found by galactography by injection of methylenblue/Isopaque Amin 1:1 in the duct system prior to operation. Ultrasonography can be used to localize the ectatic ducts in cases where there is no secretion on the day of the scheduled operation. The blue colour enables the surgeon to excise the defect duct very precisely. Such a microductectomy preserves breast tissue and gives a better cosmetic result than with complete central ductectomy. We discuss examination and treatment of women with discharge from the nipple.
