Three healthy men with psoriatic plaques unresponsive to tar, anthralin, and UVB were treated with 2450 MHz microwave heating for 30 minutes at 42 degrees C. Two patients had plaques over bony prominences.
Pain
developed in both patients, and one had a hypotensive episode during the first treatment. The third patient, whose plaque was greater than 2 cm and was not located over a bony prominence, completed the 5 weeks of biweekly treatments with complete resolution of the plaque and no complications.

Many patients presenting for treatment of superficial varicosities and telangiectasias on the legs are seeking relief of
pain
or discomfort in addition to cosmetic improvement. Of 215 sclerotherapy patients, 114 (53%) complained of various painful symptoms involving areas of telangiectasias (vessels 0.1-1 mm in diameter) and varicosities (vessels greater than 1 mm in diameter). In order to determine whether sclerotherapy is effective in reducing leg pain and discomfort, hundreds of patients receiving two or more treatment sessions of compression sclerotherapy with hypertonic saline were surveyed for the pretreatment and post-treatment presence of numerous symptoms associated with telangiectasias and varicosities of the leg. For all symptoms combined, the overall reported relief of
pain
was 85% (p less than 0.001). Side effects from the treatment were minimal. These results indicate that sclerotherapy for telangiectasias and varicosities is not only a cosmetic procedure but a safe and valuable medical therapeutic modality, highly effective in relieving leg pain and discomfort.

Substance P (SP), S-100 protein, methionine-enkephalin, serotonin and myelin basic protein were studied in two solitary glomus tumours of the skin by peroxidase-antiperoxidase immunohistochemistry. Multiple SP-containing nerve fibres were distributed in the parenchyma of the tumour among proliferating glomus cells, and in the oedematous stroma of the tumour. Positive staining for myelin basic protein was detected in nerve fascicles in the capsule of the tumour, but not within the glomus tumour. S-100 protein immunoreactivity was found in nerve fascicles in the capsule of the tumour, and in addition, a few cells positive for S-100 protein were scattered throughout the stroma of the tumour. No positive staining for methionine-enkephalin and serotonin was found. The present finding may explain the clinical experience that the tumour is tender and can cause severe paroxysmal
pain
, because SP is known to be a primary sensory afferent neurotransmitter for mediating nociception. A possible role of SP for vasodilation in the glomus tumour is also discussed.

A 21 year old man complained of
pain
and burning sensation on the helices of both ears, precipitated by cold weather. Examination showed tender erythematous nodules. Biopsies of both ears showed the characteristic vascular and inflammatory changes of pernio. In addition, in the left ear, there were foci of necrotic collagen surrounded by palisaded histiocytes. The presence of necrobiosis in pernio has not been reported previously. In this case, necrobiosis would appear to have a vascular basis.

from a recent marine sting is reported. The absence of

over the entire lesion and the failure of the patient to develop immune specific antibodies to common local coelenterates aided in establishing the diagnosis.

Twenty-two adults with persistent, symptomatic viral warts on the hands or feet which had failed to respond to conventional therapy were treated by carbon dioxide (CO2) laser ablation under local anaesthesia. Four patients did not return for follow-up treatment. Of the remaining 18 patients, 12 had plantar warts, the other six had warts on the hands that were mainly periungual. The median duration of the warts prior to laser treatment was 5 years. The areas treated were solitary in eight and multiple in 10 cases. After an average follow-up period of 10 months (minimum 6 months), all of the treated areas were clear of warts in 10 patients (56%), some had cleared in three, and all the treated warts had relapsed in the other five patients. Side-effects were commonly observed, especially post-operative
pain
, temporary loss of function of the treated part and scarring. The use of the CO2 laser may clear otherwise resistant viral warts but a significant morbidity may occur with this method of treatment.
