In the department of Professor Dausset 41 cases of periodic disease referred by 7 clinics in Paris were examined. Two studies were carried out. A series of 31 not related patients was testes with 30 locus A and B antigen and the frequencies observed were compared with the frequencies in a French and Yemenite Jew population. No statistically valid increase of an HLA gene indicative of a relationship between periodic disease and HLA was found. In 5 cases of amyloidosis HLA A 28 was present and it is possible that this gene is related to the amyloid complication. A family study on 12 families, 7 of which included at least 2 affected children, confirmed the recessive hereditary character of the disease, but not in relation to the HLA system. A parallel clinical study was made, which corroborated the previous studies as regards the frequency of clinical signs. The therapeutic study proved the value of long-term colchicin treatment with or without antihistaminics. The frequency of
pain
and abdominal crises was reduced, but there was little effect on the articular manifestations.

The authors report their experience of 61 injections of osmic acid into the knee during the course of rheumatoid polyarthritis. The results stated to be very good and good, are classified by the disappearance of
pain
, swelling and stiffness, or a improvement in any one of these criteria. As observed in 67% of cases at the third month the result following changed with time. While these favourable results persisted into the fifth month, there was a remarkable maintenance of them to the 30th month in those patients who it was able to follow so long. The most distinct and lasting results were seen in those joints which showed the least radiological change. Tolerance of the injection of osmic acid was satisfactory apart from several painful reactions which improved in the 24 hours following injection. Two cases of thrombo-phlebitis were seen and one case of rupture of the synovium. If the results obtained with osmic acid appear less impressive than those obtained with isotopes, the simplicity of the method attracts attention as it needs no complicated equipment and is applicable to young invalids.

On the basis of a series of 108 cases of cervicobrachial neuralgia a study was made of the clinical characteristics, circumstances of appearance, associated neurological signs, sequelae, relapses, and aetiologies of these observations. This series permits consideration of the aetiopathogenesis of cervicobrachial neuralgias of rheumatic origin from a new angle. The disco-osteophytic nodule does not appear to play the role classically attributed to it. Three forms are distinguished: idiopathic cervicobrachial neuralgia of transient occurrence with a cyclic duration and little tendency to recurrence and not related to concomitant arthrotic lesions; long-term cervicobrachial neuralgia lasting more than 20 weeks and always requiring a search for narrow canal and root anomaly -sometimes the form of onset of cervicarthrotic myelopathies; cervico brachialgia caused by cervicarthrosis seen after the age of 60 and consisting of projected
pain
rather than true cervicobrachial neuralgia, often recurrent.

On the basis of 12 personal observations it is pointed out that the thoracobrachial syndrome may become apparent from
pain
or paraesthesia of the arm. These symptoms typically appear in certain positions or on effort. Essential arterial or venous complications require early diagnosis, which is only possible by vascular exploration (arteriography or phlebography with radiographs with abduction of the arm). The value of clinical changes appears to be very restricted. Simple surgery-resection of the first rib- eliminates all possible multiple compression causes and is indicated in serious or very incapaciting forms of the syndrome.

The authors restate that simple
pain
of the shoulder, which is a form of scapulo-humeral periarthritis characterized by the existence of
pain
without limitation of movement, is almost always associated with lesions of the tendons of the short muscle teres of the shoulder. They term chronic simple
pain
of the shoulder the rare cases where, despite therapy that is reputed to be efficient, there is a gradual development lasting more than one year, either continuously or with relaps periods of less than three weeks. They show that these chronic shoulders are practically always associated with a rupture of the coat of the muscle teres or a calcification of the tendons of the teres coat. Ruptures are as much frequent in men as in women and calcifications definitely more frequent in women (60%). Before 30 years of age, the most frequent lesion is almost always a calcification, and after 65 rupture is more common. Between 30 to 65, the probability of a calcification in a chronic simple painful shoulder, goes from 100% to 0, while the probability of a rupture increases from 0 to 100%.

The injection of the sacrolumbar roots allows, in the first place, the root to be made opaque in order to determine its morphology and, in the second place and above all, causes the disappearance of the radicular
pain
after injection of a local anesthetic, which helps the degree of sciatica to be identified. This method, which should be limited to exceptional cases, seems to be useful in patients who have undergone lumbar surgery and for whom the interpretation of contrast x-ray exams poses a particularly delicate problem. It can also be advisable when these exams are not conclusive or when the results do not coincide with the clinical data. Since March, 1974, 18 patients have undergone this exam. In 14 cases the disappearance of the radicular syndrome, after injection of xylocain close to the root, has allowed the root in question to be identified. In 3 cases radicular injection has practically made it possible to rule out an organic lesion. This exam had no side effect and was always tolerated even by two patients allergic to iodine.

The authors report 11 observations of middle-age women seeking medical treatment for polyarthralgia most often involving the hands and often several articulations of legs and arms, with
pain
sometimes increasing during the night and frequently accompanied by a feeling of stiffness in the morning. The clinical, biological, and radiological studies did not disclose any organic disease except for vasomotor disorders of the Raynaud's disease type, acrocyanosis, acrorigor and clubbed fingers. Therapy with vasodilator isoxsuprine caused the disappearance or the diminution of the
pain
in 10 out of 11 cases. The authors suggest that these polyarthralgia are the consequence of arterioloconstriction which is a disturbance common to all the vasomotor disorders observed in their 11 patients.

The authors present the results of histo-pathological observations made in 9 cases of sympatic algodystrophy reflex of the hip, subjected to a punch biopsy of the head and neck of the femur. The punch biopsy was decided upon, either because of difficult diagnosis, or because of the abnormally long duration of the
pain
syndrome. In all cases, there were bone lesions. The most common were in the marrow tissue, with stasis (9 cases), fibrosis (7 cases) and necrosis at small, isolated points (5 cases). There was osteoclastic resorption in the bones in 3 cases, and rapid osteogenesis in 6 cases. The latter anomaly revealed the rapid reconstruction characterizing the syndrome. Finally, the authors comment on the relationship between algodystrophy and necrosis of the hip, and the advantages of punch biopsy.

In 34 patients with coronary atherosclerosis a pacing test was performed with measurement of the lactate, glucose, potassium and inorganic phosphate coronary arterio-venous differences. Eighteen of these 34 patients felt no
pain
during the pacing test. In this group of asymptomatic patients, there was no significant change of the lactate, glucose, potassium and inorganic phosphate myocardial extraction. In the 16 patients who felt an anginal pain during the pacing test, there was a significant myocardial production of lactate, but the myocardial loss of potassium and inorganic phosphate was not continuously statistically significant. Myocardial extraction of glucose during the pacing-induced angina did not increase. Although, in some patients, both the potassium and the inorganic phosphate might be used to assess a condition of myocardial ischaemia, the lactate remains the best metabolic criterion for pacing-induced ischaemia.

Two randomized series of 60 cases of myocardial infarction or menace syndrome have been treated at the acute stage, one by Heparin alone, the other by the combination Urokinase-Heparin. The average dosage was 300 mg Heparin in the first series, of 2,700,000 CTA units of Urokinase combined with 240 mg of Heparin in the second series. After the first 24 hours, equal heparinization was performed in both series up to the third week. Significantly different results were obtained in the two series. They favour Urokinase and concern: -- the disappearance time of
pain
, -- the course of the arrhythmias and of cardiac failure, -- the regression or limitation of the necrosis q waves and the lesion areas on the electrocardiogram. Finally the 30th-day overall mortality was 13% in the Heparin series and 3% in the myocardial infarction on the way of constitution, or which have done so for less than 24 hours.
