in the toe without side effects. After this reversible block, a chemical lumbar sympathectomy was performed producing

The aim of this double-blind placebo-controlled study was to evaluate the therapeutic efficacy of Doxium in chronic venous insufficiency (CVI). 225 patients were treated randomly for 4 weeks with 1.5 g (3 capsules/day) of Doxium or placebo. The evolution of the
leg oedema
was determined by measuring calf and ankle circumferences.

and discomfort were assessed by visual analogue scale. The results show that at the end of the trial, all the examined parameters (
leg oedema

, day and night cramps, discomfort, heavy legs, paresthesia and restless legs) were significantly more improved in the Doxium group than in the placebo group: the leg volume was diminished by 3.8% in the Doxium group compared to 1.2% in the placebo (p less than 0.005). The overall assessment by the physicians showed an improvement in 82% of the Doxium-treated patients compared to 42% of the placebo group (p less than 0.0001). The tolerance of the treatment was comparable in both groups.

In order to find the most comfortable and effective posture to reduce lower

, 31 young women working in Chang Gung Memorial Hospital, ages ranging from 20 to 40, were studied by the volumetric displacement method with leg placed in a specially designed container. The effects of five different angles of leg elevation on reduction of
leg edema
were separately evaluated in 5 days. For each evaluation, two measurements of volumetric displacement of lower
leg edema
were performed after prolong sitting or standing of more than 4 hours, and at the end of 15 minutes of supine lying (angle of leg elevation = 0 degrees), or after the leg was elevated in an angle of 30 degrees, 45 degrees, 60 degrees, or 90 degrees respectively. A good correlation was found between the displaced volume and increasing angle of leg elevation (regression line Y = 99.109-0.016X, r = -0.96). There was a significant difference between leg elevation of 90 degrees and supine lying (t = 3.01, p less than 0.01). The degree of comfort in leg elevation was in the order of 30 degrees, 45 degrees, 60 degrees, 0 degrees, 90 degrees. Many subjects complained of numbness and throbbing

Seven patients with arteriovenous fistulae of the internal iliac artery are reported. Ages varied from 6 to 50 years (mean 32 years). Cutaneous angiomata ,

was present in 4 cases each, bleeding in 2 cases whilst only one was asymptomatic. Only 1 patient had cardiac insufficiency. In six patients the lesion was a congenital malformation but in one it was posttraumatic. Arteriography both global and selective, intravenous pyelography and cardiac output are routine. Recently, pelvic computerised tomography has been most helpful. Embolization is recommended in all cases with surgery within 24 hours unless the lesion is very extensive and considered to be inoperable. Repeat embolization is used for recurrence or very extensive lesions. Results were good in five patients; in two patients the results were clinically good but control angiograms showed a recurrence.

Thirty-four patients who received radiation therapy for carcinoma of the cervix developed hydronephrosis. Twelve of 34 (35%) had no evidence of pelvic malignancy, and the obstruction was caused by periureteral fibrosis. The incidence of obstructive uropathy due to periureteral fibrosis not associated with recurrent tumor increased when the obstructing lesion was unilateral, the clinical staging of the cervical carcinoma prior to therapy was Stage IB or Stage II rather than Stage III or Stage IV, and the obstructing lesion appeared 2 or more years after the completion of radiation therapy. these patients strongly suggest a recurrent tumor. The diagnosis of periureteral fibrosis should be considered in all patients who develop obstruction of the urinary tract after radiation therapy for carcinoma of the cervix. We describe a patient with advanced cancer whose severe symptoms of congestive heart failure were successfully treated with dobutamine. The intermittent intravenous administration of dobutamine 5 micrograms/kg/min for 3 hr per day at home enabled control of dyspnea,

, and increased urine output after 1 day. An improvement in renal function was observed in the following days. The mechanism and the utility of a palliative approach with dobutamine are discussed.
