The
pain
, stiffness, swollen joints and progressive disability caused by rheumatoid arthritis (RA) makes it vital for healthcare providers to identify measures that enhance quality of life (QOL) for sufferers of this chronic disease. To date, very few studies have focused on QOL in middle-aged and older patients with RA in Taiwan. The purpose of this cross-sectional correlational study was to investigate the effects on QOL of disease severity, duration,
pain
, daily function, and sense of helplessness among middle-aged and older outpatients with RA. RA patients aged 50 and over were recruited (N = 158) from the outpatient clinic of a medical center in Taiwan. Data gathered and analyzed by hierarchical multiple regression included information on demographics, degree of
pain
, disease activity, daily function, and self- perception of helplessness. Length of time with RA diagnosis, severity of
pain
, disease activity, daily function, and sense of helplessness were found to correlate negatively with physical QOL (p < .01), and severity of
pain
, disease activity, daily function, and sense of helplessness were found to correlate negatively with the mental dimension of QOL (p < .01). Under hierarchical multiple regression, RA history, degree of
pain
, disease activity, daily function, and helplessness were found to explain 63.8% of total variance in the QOL physical component and 26.4% in the mental component. Degree of
pain
was the most significant factor to predict the QOL physical component (beta = -.627, p = .000). Findings of this study suggest that health care professionals should consider how best to assist patients suffering from RA to relieve
pain
and delay the onset of disabilities. Findings also suggest that great benefit can be realized by lessening the sense of
hopelessness
felt by patients.

The patient was a 44-yr-old man with end-stage renal disease who had developed chorea as a result of hypoglycemic injury to the basal ganglia and thalamus and who was subsequently diagnosed with depression and restless legs syndrome (RLS). For proper management, the presence of a complex medical condition including two contrasting diseases, chorea and RLS, had to be considered. Tramadol improved the
pain
and dysesthetic restlessness in his feet and legs, and this was gradually followed by improvements in his depressed mood, insomnia, lethargy, and feelings of
hopelessness
. This case suggests that the dopaminergic system participates intricately with the opioid, serotoninergic, and noradrenergic systems in the pathophysiology of RLS and
pain
and indirectly of depression and insomnia.

The aim of this study was to assess the relationship between sleep quality,
pain
, psychological distress, cognitive status and post-traumatic experience in advanced cancer patients. Participants were 82 advanced cancer patients referred to a palliative care unit for control of
pain
and other symptoms. A variety of assessment tools were used to examine the prevalence of sleep disturbance, the severity of
pain
and depression,
hopelessness
, cognitive function and quality of life. Using the Pittsburgh Sleep Quality Index (PSQI) 96% of patients were 'poor sleepers'. Statistically significant associations were found between PSQI and the SF-12 (Short Form-12) Quality of Life Instrument (MCS, P < 0.0005, PCS, P < 0.0005), depression (Greek Depression Inventory) (P < 0.0005) and
hopelessness
(Beck
Hopelessness
Scale) (P = 0.003). Strong associations were also found between PSQI and IES-R (Impact of Event Scale-Revised) (P = 0.004). The strongest predictors of poor sleep quality in this model were MCS (P < 0.0005), PCS (P < 0.0005) and IES-R (P = 0.010). Post-traumatic experience and quality of life seemed to be the strongest predictors of sleep quality in a sample of advanced cancer patients referred for palliative care.

The goals of this study were to assess the validity and usefulness of a new scale and to assess disability in a sample of patients with chronic daily headache. Participants were a convenience sample of 61 adult outpatients admitted to the Department of Medical Sciences of the Sant'Andrea Hospital in Rome, between September 2007 and May 2008. Inclusion criteria were, a diagnosis of chronic daily headache (illness duration >5 years). Patients were administered the Beck
Hopelessness
Scale (BHS), a specific section of the Mini-International Neuropsychiatric Interview (MINI) assessing suicidal intent, the Gotland Male Depression Scale (GMDS), and a scale devised to measure the degree of disability caused by the headache [Italian Perceived Disability Scale (IPDS)]. Analyses indicated that the IPDS had good internal consistency (Cronbach alpha = 0.93; average inter-item correlation = 0.40), and good convergent validity, with moderate to strong associations with measures assessing emotional distress (BHS, r = 0.47; P < 0.0001; GMDS, r = 0.72; P < 0.001). A single-item, logistic regression analysis indicated that the IPDS is able to predict suicide intent (Wald chi(2) = 5.04; P < 0.05) in chronic daily headache patients. The IPDS is a brief instrument that is useful for comparisons with other chronic illnesses, and it may be used both for basic research and clinical applications when screening for comorbidity with emotional distress and disorders.

To evaluate the relationship among quality of life, temperament, illness perception, and mental turmoil in patients affected by chronic daily headache with concomitant medication overuse headache. Participants were 116 consecutive adult outpatients admitted to the Department of General Medicine of the Sant'Andrea Hospital in Rome, between January 2007 and December 2007 with a diagnosis of chronic daily headache (illness duration >5 years). Patients were administered the Temperament Evaluation of Memphis, Pisa, Paris and San Diego-autoquestionnaire version (TEMPS-A), the Beck
Hopelessness
Scale (BHS), the Hamilton Rating Scale for Depression (HAM-D), the Mini-International Neuropsychiatric Interview (MINI), the Revised Illness Perception Questionnaire (IPQ), the Suicide Score Scale (SSS), and the Quality of Life Index (QL-Index). Twenty-eight percent of the patients evidenced moderate to severe depression, and 35% evidenced severe
hopelessness
. Analyses also indicated that quality of life, temperament, illness perception, and psychological turmoil are associated. However, a hierarchical multivariate regression analysis with quality of life as dependent variable indicated that only a model with mental turmoil variables may fit data; further, only the MINI suicidal intent resulted associated with quality of life (standardized regression coefficient = -0.55; t = -3.06; P < 0.01). Suicide risk may play a central role in affecting the quality of life of patients with chronic headache. The investigation of the interplay of factors that precipitate suicide risk should include assessment of chronic headache and its effects on wellbeing.

The purpose of this study was to investigate the
hopelessness
level and the relationship of depression, anxiety and disease-related factors to the presence of
hopelessness
among Turkish patients with cancer. Ninety-five patients hospitalized for cancer treatments were recruited for current study. Data were collected by using a demographic questionnaire, the
Pain
Numeric Rating Scale, the Beck
Hopelessness
Scale, and the Hospital Anxiety Depression Scale. The mean
hopelessness
score was 5.20+/-4.39. There were significant differences in terms of
hopelessness
between the patients who had metastasis and
pain
as compared with those without metastasis and
pain
(p<0.05). There were also found that significant correlation between
hopelessness
and depression and between
hopelessness
and anxiety (r=0.721; r=0.645, respectively, p<0.001). Foreword stepwise multiple regression analysis revealed that the independent predictors of
hopelessness
were depression score and thr presence of metastasis (F=55.133; p<0.001). The findings suggest that levels of
hopelessness
among cancer patients with
pain
and metastasis are higher than among those without
pain
and metastasis, and that the severity of
pain
, anxiety, and depression is positively correlated with
hopelessness
level. The assessment of
hopelessness
,
pain
, anxiety and depression levels of the patients with cancer should be an essential part of health care practice. Therefore, when arranging care assessment, to evaluate
hopelessness
could help professionals to appropriately refer patients to further psychological care resources.

This study aimed at determining the functional neuroanatomy of mental
pain
, a hitherto neglected symptom in the study of depression, which according to DSM-IV is stronglylinked with suicide. Mental
pain
(measured with the Orbach & Mikulincer Mental
Pain
Scale), suicidal ideation (measured using the Hamilton Rating Scale for Depression),
hopelessness
(measured using Beck's
Hopelessness
Scale), and regional cerebral blood flow as measured with single photon emission computed tomography were assessed in 39 depressed individuals. Levels of mental
pain
were significantly and positively associated with suicidal ideation and levels of
hopelessness
. When compared with patients with low levels of mental
pain
, those with high levels of mental
pain
showed relatively increased perfusion in the right dorsolateral prefrontal cortex, occipital cortex and inferior frontal gyrus and in the left inferior temporal gyrus, and relatively decreased perfusion at the medulla. The findings indicate that mental
pain
in depressed patients is associated with an increased risk of suicide and that high levels of mental
pain
are associated with changes in perfusion in brain areas that are involved in the regulation of emotions. Further study is warranted to understand whether this association reflects increased emotional processing or decreased cognitive control over mental
pain
in depressed individuals.

One suicide occurs every 16 minutes. Intentionally killing oneself is distinctly human; other species don't commit suicide. Suicide is an exceedingly complex phenomenon stemming from intolerable stress and the inability to cope. Elders tend to plan their suicides well, choose means of killing themselves that are more violent than younger people do, and are less likely to survive the attempt than others. Numerous factors increase risk for elder suicide: recent bereavement,
pain
, chronic illness,
hopelessness
, and despair. Identifying and treating depression early is essential to avoid tragedy. Regardless, many elders are treated with anxiolytics and analgesics in lieu of antidepressants, and some medications may increase suicide risk. Consultant pharmacists need to be aware that suicide is a serious concern for elders. When depression seems to be worsening in an elder, aggressive action is needed.

Nurses who care for dying patients are under pressure emotionally because of their beliefs and values about death as well as the emotions and reactions of the patients and their families. This study examines the emotional difficulties of nurses caring for oncology patients in Turkey. The study used a descriptive survey design. The participants were 157 nurses from three medical oncology units in Ankara. Results showed that nurses had difficulty in talking to oncology patients about end-of-life issues and found that caring for dying patients affected their personal lives. This study also showed that the length of nurses' work experience had no effect on their feelings and perceptions toward terminally ill patients. However, the nurses who had more work experience were more likely to report difficulty in talking to patients. Most of the nurses expressed feelings of inadequacy and
hopelessness
about
pain
management and treatments.

We investigated the potential prognostic factors for clinically relevant improvements in
pain
intensity and
pain
-related disability after myofeedback-based teletreatment. Sixty-five female computer users, 56 female patients with whiplash-associated disorders and 18 female patients with non-specific neck and shoulder pain participated in the study. They received myofeedback-based teletreatment or usual treatment. Questionnaires concerning prognostic factors,
pain
and disability were completed before the start of the intervention (baseline) and at follow-ups at the end of the intervention, and after 3 and 6 months. Logistic regression analyses were performed in order to investigate prognostic factors for clinically relevant improvement. In the intervention group, improvement in
pain
intensity was predicted by baseline
pain
intensity. Baseline
pain
intensity and disability, and fear-avoidance and endurance related
pain
coping responses were prognostic factors for outcome in
pain
-related disability in this group. There were few differences between the intervention groups; fear-avoidance coping responses influenced the outcome after teletreatment only. Myofeedback-based teletreatment appears to be an useful telemedicine intervention, especially for participants with moderate to high levels of
pain
and disability, high perceived help/
hopelessness
, and those who tend to deal with their
pain
by avoiding social and physical activities.
