The effects of 2 doses of nefopam, d-amphetamine, pentazocine, and placebo were studied in healthy male sleep-deprived volunteers to determine whether the drugs improved or impaired coordination and whether they induced subjective effects. A critical tracking task was used to study hand-eye coordination. D-amphetamine, 10 mg orally, significantly improved tracking performance and made subjects feel better able to perform tasks but more anxious. It also made them feel more alert, steady, sociable, and strong. Pentazocine, 45 mg intramuscularly, caused deterioration in tracking performance and was followed by reports of depression, gloominess, dreaminess, nausea, and injection site pain. There was no significant change in tracking performance or subjective effects after both doses of nefopam and placebo.

The effects of synthetic salmon CT, administered subcutaneously and intermittently (1 MRC U/kg/day for 15 days/month over 6 months) were investigated in 15 uremic patients on regular dialysis treatment (RDT), all presenting various degrees of osteodystrophy. Clinically, osteoarticular pain disappeared in 8 out of 10 cases; 1 patient with rib fractures had a rapid calcification of the bone fracture repair tissue. No significant changes were found in serum calcium and PTH levels. Phosphotemia showed a significant decrease within the first 20 days. The varying individual hypophosphatemic response proved to be related to the initial level of phosphatemia. The alkaline phosphatase, when increased, showed a decrease to the normal range. A significant decrease in osteoclastic hyperactivity (active resorption surface, osteoclast index) and a slight increase in osteoblastic pool (active osteoid surface) were documented. No change was noted when osteomalacia predominated. Side effects included: anorexia, nausea, vomiting, face flushing. Our data suggest that salmon CT may be usefully employed in chronic uremic patients on RDT, when secondary hyperparathyroidism predominates.

Nine patients with intracerebral metastasis from lung carcinoma were treated with intracarotid and intravertebral artery infusion of 1,3-bis(2-chloroethyl)-1-nitrosourea (BCNU). Four of these patients considered definite responders showed unequivocal clinical improvement and definite decreases in the size of tumors evaluated by neurologic examination, computerized tomographic (CT) scan and radionuclide brain scan (RBS). One patient's clinical condition stabilized with doubtful improvement of diagnostic tests (probable responder). The remaining four patients had further unfavorable progression of the clinical and scan findings and were clearly nonresponders. Complications were transient and included: local pain in the eye, orbit, and occipital-nuchal area during infusion in 7 patients, focal seizure in 3 patients, mild confusion with disorientation in 2 patients, and nausea in 2 patients. Our findings suggest that intra-arterial BCNU therapy may be effective and may be used as an adjuvant to surgery and/or radiotherapy for the treatment of metastatic brain tumor from lung carcinoma.

Despite the recent publicity given to studies pointing out the negative side effects associated with the use of oral contraceptives, the pill is still the most common form of contraceptive used in Great Britain and constitutes the form of contraception for many women. Some of the negative effects currently receiving wide publicity, no longer represent a serious threat for the pill user; many of these effects have been negated or ameliorated by modifying the composition of the pills. In combined pills the estrogen content has been reduced from 150ug to 20-35ug. In the 2 major studies linking oral contraceptives with the development of cardiovascular disease most of the women in the studied population had taken pills containing 50-100ug of estrogen. Given the wide choice of pills currently available, many of the negative effects, such as nausea and pain, cna be ameliorated by choosing a more appropriate oral contraceptive for the specific patient. Other side effects such as headaches and poor cycle control can be treated by regimen modification. Although there is a relationship between pill use and hypertension, pills containing levonorgestrel in combination with 30ug of ethinyloestradiol have only a slight effect on blood pressure. Lactation is not reduced for women who take progestogen only pills. Fertility is successfully restored in almost all women shortly after they stop using the pill. Drug interaction failures can be avoided in many cases by prescribing pills containing 50ug of estrogen for women taking other drugs. In order to obtain immediate effectiveness, the combined pill can be started on the 1st day of the period instead of waiting until the 5th day.

The characteristic clinical features and morphological findings of five cases of (clinically not diagnosed) fusiform aneurysm of the basilar or vertebral arteries were recurring attacks of positional occipital headache, pain and stiffness in the neck, cranial nerve disturbances, expecially oculomotor palsies and anisokoria, nystagmus, attacks of nausea, vomiting and sweating, tachycardia, pyramidal tract symptoms, and pareses. Severe hypertension had been present in four instances. The aneurysm, which is usually thrombosed, pressed against the pons and medulla oblongata as a space-occupying mass. In addition to hypertension and atheromatosis, congenital defect in the arterial wall are probably significant causative factor. To mistake an aneurysm for a cervical syndrome may be fatal to the patient.

A study of oral contraceptive (o.c.) and IUD use among single nulliparae, 14-18 years of age, is presented. 100 women were prescribed a combination preparation, and Copper T IUDs were inserted in 100 women. 33%-40% had not previously used any form of contraception. 9 of the o.c. group and 14 of the IUD group had undergone abortion before. There were no pregnancies in either group throughout the observation period of 955 months for the o.c. group and 1134 months for the IUD group. The most common side effects in the o.c. group were weight gain, depression, nausea, and decreased libido; 44 reported no side effects. Pain increased menstrual bleeding, and fluor were the most commonly reported side effects in the IUD group; 21 reported no side effects. The IUDs were expelled in 17 cases, in 12 of which they were immediately reinserted. 4 cases of salpingitis were reported in the IUD group. 68 in the IUD group said they were satisfied with the contraceptive method, compared to 59 in the o.c. group. The continuation rate was 75% for the IUD group after an average 13.2 months of observation and 55% in the o.c. group after an average 12.7 months of observation.

After a brief survey of the expected advantages of the early interruption of pregnancy by the Karman method, the author describes his own observations on immediate and early complications in 850 cases. In the course of intervention, 7.45% of the patients reacted with a vegetative manifestation of cervical shock--pallor, nausea, vomiting, colic-like pain in the lower part of the abdomen (mainly in nullipara). The aspirated amount of material did not surpass 50 ml in women with amenorrhea of 40-45 days duration. The mean duration of the aspiration was 1 minute, 57 seconds. There was menstruation-like bleeding from day 3 to days 10-12 in 86.3% of the women with interruption of pregnancy. Its occurrence in 2.49% of the patients was preceded by colic-like pain and shortlived elevation of axillary temperature up to 38oC. Inflammatory complications were registered up to the 2nd month in 2.49% of the 79.3% followed. The aspiration system with the hand vacuum extractor (Malstrom type) was used successfully for creating negative pressure and thus the special syringe (Karman type) was replaced.

Since the introduction of phenothiazines into clinical practice in 1952, over 250 million people have received these drugs for the treatment of psychotic states. In addition to the phenothiazines, five other classes of neuroleptic medications are now in use: butyrophenones, thioxanthenes, dihydroindolones, diphenylbutylpiperidines, and dibenzoxazepines. Besides their use in the treatment of psychosis, these drugs have been used in the treatment of anxiety, depression, nausea, alcoholic withdrawal, and pain, and are often administered in combination with other medications. Through the use of these drugs, many psychotic patients have been able to move back into the community, and the family physician is coming into contact with more patients on maintenance dosages of neuroleptics. He/she may wish to prescribe these drugs or may, in the treatment of a medical problem, need to prescribe other medication to an individual already receiving neuroleptics. It is important, therefore, for the family physician to be aware of the side effects of these drugs and of complications which can arise when neuroleptics are given in combination with other families of drugs.

A comparison was made of the relief of pain after operation, obtained following the i.v. administration of buprenophrine and pethidine in 60 patients with lower abdominal incisions. No difference could be detected between the maximum analgesia produced by eigher drug, but analgesia following buprenorphine appeared to last about four times as long as that following pethidine. When the drugs were compared on a "dose per body weight" basis the results supported a ratio of potency in the order of 1 : 200 in favour of buprenorphine. Vomiting, drowsiness and dizziness were less frequent following buprenorphine, but a similar frequency of nausea was observed with both agents. Marked miosis occurred 5--10 min after the i.v. injection of buprenorphine, but no serious side-effects were observed with either drug. The i.v. injection of buprenorphine, but no serious side-effects were observed with either drug. The i.v. administration of buprenorphine appeared to be effective in the management of pain after operation in patients with lower abdominal incisions.

Interposition of the colon between the liver and diaphragm, ie. Chilaiditi's syndrome, visible during roentgenographic examination, has been described as an asymptomatic finding in adults of no clinical importance: it is occasionally symptom-producing in children. In mentally retarded adults, however, a unique syndrome commonly occurs, characterized by nausea, pain, vomiting, anorexia, distension, audible bowel sounds, and constipation: all are associated with three roentgenographic features of interposition. The symptoms respond to simple treatment and can be prevented by routine prophylactic measures.
