Forty-six patients with chronic pain were treated with transcutaneous nerve stimulation (TNS). If there were any signs of sensory loss in the
pain
area, the electrodes were placed on the healthy side of the body. The effect of TNS was assessed quantitatively. After 9 months of repeated TNS, on average, the total
pain
score had improved by 39%. The subjective intensity of the
pain
, the frequency of
pain
and the need for analgesics had diminished by 46-47%. In those conditions in which total improvement was better than the mean (phantom limb pain, 65%; zoster neuralgia, 56%; thalamic
pain
, 45%), the healthy side of the body had been stimulated. In those in which the painful area had been stimulated (cancer pain, 32%;
low back pain
, 32%; brachialgia, 15%), the beneficial effect did not reach the mean for the whole series. This suggests that TNS of the healthy side of the body may give better long-term improvement than stimulation of the painful area. A theory of chronic pain and the mechanism of TNS is presented.

The protrusion of cervical intervertebral discs was divided into three pathological entities by Spurling; soft disc, hard disc and spondylosis. We applied these concept to the dorsal intervertebral disc disease and treated two cases of thoracic spondylosis. Case 1. A 41-year-old male entered the hospital because of the gradual progression of weakness of both legs of two months' duration. Since ten days before admission he had not had an errection and had not been to able to walk and micturate. He also complained of paresthesia radiating down the abdomen into both legs. There were no visceral complaints. Neurological examination revealed severe weakness of both legs with bilateral impairment of deep sensations and hypalgesia up to the level of T6. Reflexes in both legs were hyperactive with sustained clonus. Plantar responses were extensor bilaterally. Though plain X-rays showed no changes, tomography revealed a calcified intervertebral spur formation at the T5-6 interspace. A myelogram showed a complete block of the contrast medium at the level of the upper part of T6. The patient underwent a complete laminectomy from T3 through T6 and extradural anterior decompression with the removal of the calcified disc at the T5-6 interspace using an air drill. Postoperatively, he demonstrated an immediate improvement in sensation and a gradual recovery in motor power. At his follow-up examination 14 months after surgery he could walk without assistance. Case 2. A 47-year-old dwarfish woman (130 cm) with a
low back pain
and difficulty in walking for a few years duration was admitted. A few months before admission she felt
pain
at her left lateral abdomen. There was weakness of both legs, greater in the left. Reflexes in her left lower extremity were hyperactive with sustained clonus. Plantar responces were flexor bilaterally. Palin X-rays showed scoliosis of thoracic spine with the top at T7 level and calcified intervertebral masses at T10-11, T11-12 and T12-L1, extending into the canal that were confirmed more clearly by tomography. Myelography by a cisternal puncture disclosed a complete block at the level of T10. The patient underwent total laminectomy of T9 through L2 and extradural anterior decompression with the removal of calcified discs. At her follow-up examination 12 months after surgery she could walk for herself with some residual neurological signs, minimal weakness in the right leg and hypesthesia up to the level of T12 in the left. We have discussed the incidental, related diagnostic and operative problems of this disease.

On the basis of a 9-year experience with 231 patients with post-laminectomy and post-spinal fusion stenosis of the lumbar spinal canal, we emphasize the importance of recognizing and adequately decompressing such lesions. In the majority of such patients
pain
relief has been achieved, but some of the failures can be attributed to technical reasons. Most failures are due to associated lesions and to functional factors. A special surgical techniy be advisable. The indications for fusion however were few and require further investigation and evaluation. The significant failure rate points to the need for continuing research into the causes of
low back pain
and adequate management programs for the patient with the "multiple operated back" who still has disabling
pain
and drug dependence.

. Multiple emboli of fibrocartilaginous material with nucleus pulposus cells were found in the foci of myelomalacia. The embolism of nucleus pulposus tissue originating from Schmorl's nodules was probably carried via the blood vessels of the neighbouring vertebral bodies with retrograde flow through the perispinal venous plexus into spinal cord veins and hence into the arteries of the spinal cord via veno-arterial anastomoses. A transitory increase in intra-abdominal pressure was probably the cause of the inversion of venous flow.

In this cross-sectional prevalence study in 1503 14-year-old Finnish schoolchildren (n = 1503)

was found to be the third most common form of
pain
interfering with schoolwork or leisure time during the past 12 months. The lifetime cumulative incidence of

was 30% and that of sciatica 1.8%. Of the 417 pupils who had experienced
low back pain
at some time, 39% had suffered during the past month; 65% recovered in one month from the last
pain
episode, while 35.2% of those reporting disabling
low back pain
during the past year were aware of recurrent or continual

. Thus, 7.8% (n = 107) of the whole population could be classified as "low back pain chronics": 86% of the

chronics had trouble with at least one of the daily activities listed, most commonly with sitting at school. Excluding

in the extremities or sciatica, girls reported various pains more commonly than boys. Moreover, girls reporting recurrent

had more trouble with their daily activities due to

The aim of this study was to investigate physical impairment in patients with chronic
low back pain
, to develop a method of clinical evaluation suitable for routine use, and to consider the relationship between
pain
, disability, and physical impairment. Twenty-seven physical tests were investigated. Permanent anatomic and structural impairments of spinal deformities, spinal fractures, surgical scarring, and neurologic deficits were excluded as not relevant to the patient with
low back pain
in the absence of nerve root involvement or previous surgery. Three consecutive 20-patient reproducibility studies were used to develop reliable methods of examination for 23 of the tests. Only four tests were excluded as unreliable: sacral angle, pelvic tilt, and separate lumbar and pelvic extension, none of which are part of routine clinical examination or have any proven relationship to disability. The remaining 23 physical tests were evaluated in 70 asymptomatic subjects and 120 patients with chronic
low back pain
. Passive knee flexion, passive hip flexion, hip flexion strength, hip abduction strength,
pain
reproduction on each of these tests, and the prone extension strength test were excluded because they were too closely related to nonorganic and behavioral responses to examination. Eight tests successfully discriminated patients with
low back pain
from normal subjects and were significantly related to self-report disability in activities of daily living: pelvic flexion, total flexion, total extension, lateral flexion, straight leg raising, spinal tenderness, bilateral active straight leg raising, and sit-up. Factor analysis failed to demonstrate an underlying statistical dimension of physical impairment. However, an empirical combination of total flexion, total extension, average lateral flexion, average straight leg raising, spinal tenderness, bilateral active straight leg raising, and sit-up provided an equally satisfactory alternative. Simple cut-offs from normal subjects made the scale simple and quick to use. This final scale successfully discriminated 78% of patients and normal subjects and explained 25% of the variance of disability, with a specificity of 86% and sensitivity of 76%. This scale provides an objective clinical evaluation that meets the criteria for evaluating physical impairment, yet is simple, reliable, and suitable for routine clinical use. It should, however, be emphasized that all the tests included in the final scale are measures of current functional limitation rather than of permanent anatomic or structural impairment. Cytomegalovirus (CMV) has been associated with a spectrum of peripheral nerve syndromes in patients with AIDS. Lumbosacral polyradiculopathy, a rapidly progressive syndrome occurring in about 1% of AIDS patients, is characterized by
low back pain
, sphincter disturbance, progressive flaccid paraparesis, and neutrophil cerebrospinal fluid pleocytosis. CMV has been found consistently in the lumbosacral roots in association with inflammation, demyelination, and axonal loss. Anti-CMV therapy has been reported in open studies to prolong survival and may lead to improvement of the neurologic deficit. A few patients with mononeuritis multiplex have been found to have CMV inclusions associated with inflammation and axonal loss in the peripheral nerve. The diagnosis, dependent on finding CMV in a biopsy sample, has been made too infrequently to estimate the incidence and prognosis or response to treatment.
Painful
distal symmetrical peripheral neuropathy, the most frequent neuropathy in AIDS, is characterized by subacute onset of
pain
in the feet, with associated findings including mild sensory signs and fiber degeneration axonal atrophy on sural nerve biopsy. A prospective study found an association of this syndrome with CMV disease of other organs, and CMV has been found in the dorsal root ganglion in a single case. Anti-CMV therapy might be of benefit in these peripheral nerve syndromes; however, the effect of such treatment needs to be assessed in controlled studies.

To determine the risk factors of the subjective symptoms experienced by chain saw workers, we surveyed 206 forestry workers using this device in their work. The strength of the relationship between their symptoms and such potential risk factors as occupational, behavioral, and physical factors was evaluated by a multiple logistic regression model. It was found that many years of chain saw operation, and numerous hours of chain saw operation per day, increased the risk of white finger, numbness of hands and arms, chillness of hands and arms and difficulty in joint movement, indicating that these symptoms were closely related to vibration exposure. Many years of forestry work before chain saw work also increased the risk of joint pain, suggesting that bone and joint disorders are related to heavy manual work not using a chain saw. A second job involving manual labor increased the risk of weakness in the arms, stiffness of the shoulders, and
lumbago
. Infrequent bathing was associated with increased risk of
pain
in hands and arms and
lumbago
. Older workers had a higher prevalence of stomach discomfort. No risk factor was found to significantly increase the risk of easy fatigability, forgetfulness and sleep disorder. It was found that not only exposure to vibration but also other factors contributed to the prevalence of
pain
in hands and arms, joint pain, weakness in arms, stiffness of shoulder, and
lumbago
. Second job and infrequent bathing appeared to be related to the occurrence of certain subjective symptoms. These contributory factors should be taken into account when evaluating subjective symptoms encountered in field studies of forestry workers.

In Huang Di Nei Jing Su Wen, among the materials which heretofore have no English translation, there are three Chapters on
pain
. One of them was devoted entirely to the
low back pain
. This is certainly an indication of its importance even more than 2,300 years ago. Since it still plagues us nowadays, we have translated that Chapter of this medical classic to see what we can learn from the ancients. We attempted to second guess the ancients in the diagnosis of the various sets of symptoms, in the light of western medicine. We discussed the difficulties in interpreting the archaic text. We pointed out that there were associations of the Mais (i.e., the Meridians) with various sets of symptoms but the loci of puncture were rather vaguely described and had no names. We inserted our selections of currently used acupoints to match the described loci. We would like to solicit our readers' comments.

A parallel investigation of the success of treating patients with chronic
low back pain
has been carried out at the Moravci Spa, at the Department for Physical Therapy and Rehabilitation and at the Rheumatology Department of Maribor Teaching Hospital. One hundred patients suffering from low-back pain were given a 14-day treatment in the termomineral water (T--36 degrees C) of the Moravci Spa. A comparative group of another 100 patients also suffering from
pain
in the lumbar region of the spine underwent equal balneo-physical treatment in plain water (T--32 degrees C) at the Dept. for Physical Therapy and Rehabilitation and at the Dept. of Rheumatology at Maribor Teaching Hospital, Slovenia. The educational background of the two groups features a statistically significant difference (p < 0.001): the percentage of patients with lower education was higher at the Moravci Spa (67%) compared with only 46% among those treated at Maribor Teaching Hospital. Correspondingly, the difference in occupation of the two groups were similar (p < 0.005): prevalent among the patients at Moravci Spa were bluecollar workers (40%) compared with white-collar workers (27%). The percentage of white-collar workers at Maribor Teaching Hospital was 45%. The average age of the patients treated at the Moravci Spa was 46.9 +/- 9.5 years (28-77 years), at Maribor Teaching Hospital it was 45.2 +/- 8.2 years (26-71 years). There was no statistically significant difference in age (p < 0.10) between the two groups. Generalized spondylochondrosis was present in both groups, i.e. 87%; approximately 10% of the patients from both groups underwent surgical treatment of hernia disci.(ABSTRACT TRUNCATED AT 250 WORDS)
