This reports a rare case of diverticulum of the sigmoid in a 73 years old patient, which presence was detected about 4 years before surgery. This giant diverticulum manifested itself mainly through
pain
followed by
anorexia
. On X-ray it had the appearance of a gas cyst. The pathogeny of such a diverticulum is to be found in inflammatory factors at the base of implantation; it results from pressure effects in the diverticulum with the probable action of anaerobic bacterial fermentation. The pathologic examination does not always allow differenciation between a giant diverticulum and a duplication, especially where the later communicates with the digestive tract. Twenty-four cases of Legionnaires' disease were diagnosed at the Wadsworth Veterans Administration Hospital during a 5-month period. All cases occurred in persons exposed to the hospital environment during the usual incubation period of Legionnaires' disease. The clinical illness was quite characteristic. All patients complained of weakness, malaise,

, and cough. Rigors, diarrhea, and pleuritic

were frequent symptoms. All patients had a maximum temperature of greater than or equal to 39.4 degrees C. Thirteen of 22 patients had relative bradycardia. Chest roentgenograms documented pneumonia in all patients. Leukocytosis, hyponatremia, hypophosphatemia, and abnormal liver-function test results were typical. Diagnosis was made by serologic criteria in 20 patients, postmortem examination of tissue in two, and both serology and tissue examination in two. Four patients in whom the disease was not suspected died of Legionnaires' disease. One patient died of unrelated causes. Fifteen of 19 survivors received erythromycin therapy. The presentation of Legionnaires' disease was characteristic enough to allow early, specific therapy.

The indication field of Nortase, a combination of microbial lipolytic and proteolytic enzymes, comprises the replacement therapy of maldigestion and insufficiency of pancreas. Its efficacy and tolerance were tested in 100 patients in an open study under the conditions of general practice. During the 15-day treatment the following symptoms were evaluated:

in the epigastrium, nausea after the meals, belching, pyrosis, the quality of feces and the body weight. 96% of the patients showed relief of the symptoms after treatment, 65% a therapeutic result ranging from very good to good. In 53% an improvement of the quality of feces was observed and 76% reached an increase in weight. 6 patients had some small side effects, in 1 case the treatment had to be interrupted. The altogether good results confirmed the results of former investigations on the acid stability and the high lipolytic activity of lipase from Rhizopus arrhizus.

The diagnosis of Crohn's disease in 13 patients (ten females and three males) at the Howard University Hospital during the ten-year period, 1965-1975, is examined. The most common presenting symptoms were right lower quadrant (RLQ)

, weight loss, and vomiting, while the most common physical finding was RLQ tenderness. Ileocolic involvement occurred most frequently. Eight patients had surgical resection. The most frequent operation was ileocolic resection with ileo-ascending colostomy. The chief indications for surgery were: (1) presumed appendicitis, (2) intestinal obstruction, and (3) internal fistulae.Crohn's disease seems to occur in blacks much less frequently than in whites. When compared to series of white patients reported by others, the series studied here has more females and more ileocolic involvement. In most other series, the greatest involvement is in the "ileum only" group.

Focal nodular hyperplasia (FNH) of the liver is a benign tumour with a cirrhosis-like appearance. The tumour is rare, but it is a relevant differential diagnosis in young patients with a hepatic mass. Radiologically and grossly FNH cannot be distinguished from malignant tumours. Biopsy with frozen section usually provides the diagnosis. There is no evidence that FNH is premalignant. Only a few patients have symptoms requiring treatment (

, nausea, and vomiting) and complications (weight loss, portal compression) are extremely rare. The preferred treatment for symptomatic tumours is removal. In patients without symptoms FNH can be left untreated without any undue risk.
