A twelve year old boy presented with acute cervical and interscapular pain, torticollis and low grade fever. The neurological examination was normal. Roentgenograms showed calcification of the T3-T4 disc and posterior herniation of the nucleus pulposus was demonstrated by computed tomography. A rapid and dramatic amelioration of the symptoms was produced by treatment with a non-steroidal anti-inflammatory drug. Intervertebral disc calcification in childhood is an uncommon and usually benign condition. Its clinical and radiological features are discussed in the light of the relevant literature.

Intervertebral disc calcification in children is a rare occurrence. The clinical symptoms and signs are distinctively confined to the cervical area with pain, limitation of motion, and torticollis. Long tract signs or radicular involvement are extremely unusual. CT scan and cervical spinal X-ray films shown the calcification to be in the nucleus pulposus with anterior or posterior mild protrusion into the spinal canal. Recovery without neurological sequelae is the rule in most of the pediatric cases with conservative treatment. Intervertebral disc calcification does not necessarily disappear with the onset of clinical symptoms. A case with persistent and even denser calcification at the same level of intervertebral disc space at the second episode of recurrence is illustrated.

An 8-year-old boy presented himself with a nontraumatic acute onset of thoracolumbar pain with an antalgic scoliosis. His past medical history did not reveal any infectious disorder. The back pain kept him from sleep and sports. Imaging revealed calcified intervertebral discs from thoracic intervertebral disc 10-11 to lumbar disc 2-3, with compression of the 11th thoracic root. Conservative treatment with analgesia, nonsteroidal anti-inflammatory drugs, and rest reduced the symptoms within a few days. The boy became asymptomatic within 2 weeks. Magnetic resonance imaging changes diminished in 2 months. Intervertebral disc calcification in children is a rare benign disorder which mostly affects the cervical spine. This is the first case, to our knowledge, of symptomatic intervertebral disc calcification involving the lumbar spine up to lumbar disc 2-3.

Intervertebral disc calcification (IVDC), though rare, remains an important differential of pediatric spinal pain. A 7-year-old boy presented with sudden-onset severe neck pain and restricted movements. There was no definite history of trauma or infection. Imaging of the cervical spine showed calcification of the intervertebral disc at C2-3 level, with significant posterior protrusion into the spinal canal causing compression of the cervical spinal cord. The child was kept on conservative management. The calcification and posterior protrusion showed near-complete resolution on 3-month follow-up. This case report emphasizes that childhood IVDC is a benign condition which commonly resolves spontaneously, without any surgical intervention and neurological sequelae.
