A standardised form was developed to review the daily problems suffered by patients with
arthritis of the hip
and provide clinicians with information for planning treatment and in judging subsequent progress. The reports made by various patients in a preliminary study provided 81 statements on
pain
, restricted movements, and restricted activities that were then tested to identify the most reliable. The responses of 32 patients waiting for hip replacements and 66 patients who had undergone operation were compared with independent assessments of
pain
and physical limitations. The 33 statements eventually selected were chosen mainly on the basis of their sensitivity to differences between preoperative and postoperative patients and their correlation with the independent assessments. The final questionnaire provides a valid and concise summary of a patient's disabilities and is simple enough for the patient to complete while waiting to see the doctor.

Intrapartum haematogenous spread of vaginal group B Streptococcus is rare, but it can lead to severe complications like abscesses, endocarditis and meningitis postpartum. We report a postpartum periarticular hip abscess caused by group B Streptococcus. Clinically it caused

in the hip and a compression of the femoral nerve with motor and sensory component. Diagnosis was made by aspiration under computed tomography control. The only sign of infection was an increased sedimentation rate. After antibiotic treatment the symptoms disappeared and the abscess vanished, but it was reactivated 4 months after delivery, leading to
arthritis of the hip
joint. Another antibiotic treatment was administered for 8 weeks. 17 months postpartum the patient is well, but a development of a secondary coxarthrosis can not be excluded.

A study was conducted in general practice to assess the efficacy and safety of isoxicam 200 mg once daily compared with indomethacin 25 mg 3 times a day in the treatment of acute exacerbations of osteo-arthritis. The trial was conducted as a multicentre, double-blind, randomized parallel-group study with 'dummy loading' of the medications. Thirty-one general practitioners entered 309 patients in the study. Of these, 139 patients on isoxicam and 137 on indomethacin completed the treatment. The most common sites of osteo-arthritis were the knee (100 patients) and the hip (79 patients). On examination at 7 days and 14 days there was a significant improvement in both treatment groups. After 7 days the reduction of

and clinical symptoms of osteo-
arthritis of the hip
and knee was significant for both drugs at the P less than 0,001 level. There thus appeared to be no difference in clinical efficacy between the drugs. Eight patients in the isoxicam group experienced definite drug-related adverse reactions as against 19 in the indomethacin group. A total of 38 patients (18 on isoxicam and 20 on indomethacin) suffered probably drug-related effects. Isoxicam therefore appeared to be better tolerated than indomethacin.

An open, multicentre, outpatient study involving 77 Italian orthopaedic and rheumatology centres was conducted to evaluate the efficacy and tolerability of proglumetacin in the treatment of
arthritis of the hip
and of the knee. A total of 1522 patients each received 450 mg/day proglumetacin (150 mg tablets X 3) for a period of 4 weeks, with checks at the end of weeks 1, 2, 3 and 4. The evaluation of efficacy was carried out using the following parameters:

Pain
was reduced by 62% from the beginning to the end of the study. Complete tolerability was demonstrated in 82.9% (1262 patients) of the study group; side-effects, mainly involving the gastrointestinal tract, were seen in only 17.1% of the study group (260 patients). Side-effects were reported in 2.2% of the patients; 5.3% of these were shown to result from proglumetacin treatment. It was concluded that proglumetacin, at the optimum dose of 450 mg/day, answers the need for an effective and well tolerated drug of first choice in the treatment of arthritis.

A 20-year-old man with Crohn's disease was seen as an outpatient for
pain
in the left hip and gait disturbance. Initially,
arthritis of the hip
was suspected, because of the absence of fever or abdominal abnormalities. Two weeks later, fever, malaise, and pneumaturia developed and urine culture grew Escherichia coli. Laparotomy revealed a large occult retroperitoneal abscess on the left side, which had been irritating the psoas muscle. Surgical drainage effected complete recovery of hip function. The abscess in this patient had a very unusual presentation and was diagnosed chiefly through a high index of suspicion after a careful physical examination.

Thirty patients with chronic pyogenic or tuberculous
arthritis of the hip
treated by Girdlestone's excision arthroplasty were reviewed two to seven years after operation. There was marked or complete relief of
pain
in 29, control of infection in 27, squatting and sitting cross-legged was possible in 27, and 16 were able to stand on the operated limb. Overall results were good in 16, fair in nine, and poor in five. Tuberculous disease was not reactivated and the use of traction for 12 weeks and a weight-relieving caliper for 12 months after operation helped to reduce the shortening to an average of 3.8 centimetres. Excision arthroplasty is considered a sound operation to restore the ability to squat and sit cross-legged.

In 1986-1988 at the Orthopaedic clinic of the Institute for Postgraduate Medical Training 56 stems on non-cemented total endoprostheses of the hip joint type Schenker were implanted. The non-cemented stem was combined in all patients with a cemented socket Poldi. The endoprosthesis was indicated in particular in young patients in 59% in hip joints affected with secondary postdysplastic
arthritis of the hip
joint. The group was checked by Harris' method, the mean follow-up period in 1992 was 58 months. Of 36 followed up patients 21 operated patients reported
pain
in the area of the hip joint, 10 patients used analgetics. In nine patients reimplantation of the non-cemented Schenker stem was performed on account of aseptic loosening, in 11 patients the X-ray check revealed obvious signs of aspectic loosening associated with subjective complaints. The dependence of aseptic loosening on the quality of primary implantation is marked. In patients with perfect cortical contact of the stem aseptic loosening developed in 27% of the patients, in patients with inadequate cortical contact aseptic loosening was recorded in 80% of the operated patients. The general evaluation according to Harris comprised 42% excellent results, 30% good results, 14% satisfactory results and 14% poor results. This evaluation does not include patients who on account of
pain
and loosening of the femoral Schenker component were already re-operated. Because of the considerable number of complications implantation of this type of prosthesis was discontinued at the authors' department.

Between 1980 and 1991, ionizing radiation was applied for analgesic purposes to 181 patients (97 men, 84 women, mean age 54 [29-81] years) with degenerative-inflammatory skeletal disease. The long-term effects were evaluated by questionnaire. Radiation of 2.5 to 6.0 Gy achieved lasting
pain
relief in 21 of 30 patients (70%) with arthritis of the shoulder or humeroscapular periarthritis, 15 of 21 (71%) with
arthritis of the hip
, in 12 of 15 (80%) with heel spurs or Achilles tendon bursitis and 10 of 11 (91%) with epicondylitis.
Pain
relief lasted for longer than two years in 41 of the 77 patients (53%). There were no side effects at the stated dosage. According to dose measurements the theoretical risk of malignant tumour induction is 20-40/million radiated patients and thus four orders of magnitude below the spontaneous malignant tumour incidence rate. The genetic risk is even lower. Ionizing radiation of degenerative-inflammatory diseases is thus an effective form of treatment with few side effects.
