Clinical features of vascular leiomyoma of the upper extremity which have not been described or have been described incorrectly are documented with a report of 11 consecutive cases seen within the past 11 years. All patients were followed without recurrence or surgical morbidity.
Vascular leiomyoma
has been described as a painful nodule found more frequently on the extensor surfaces, but in our experience spontaneous or paroxysmal
pain
is exceptional and the tumour tends to be located on the volar surface when it is in the hand. In the digit, they tend to be found at the base away from the midline, and not seen distal to the DIP joint.

A rare case of intrabony angiomyoma affecting the mandible is presented. The patient was 25 years old male who complained of swelling and
pain
in the posterior segment of his lower jaw. Histopathologically diagnosed angiomyoma is discussed with emphasis on the pathogenesis in the present case. Literature concerning the lesion is reviewed. In the present paper we report a case of angiomyoma affecting the mandible.
Angiomyomas
are also known as vascular leiomyomas which commonly occur in the wrist and ankle region. They are painful tumours as was also evident in our case. These tumours are characterized histopathologically by convuluted thick walled vessels associated with bundles of well differentiated smooth muscle elements. They rarely affect the oral cavity. Therefore our case is one of the few cases affecting the oral cavity as reported earlier. Tumours of the smooth muscle cells which form involuntary muscle are known as leiomyomas. These commonly affect the alimentary tract. They are rarely found to affect the oral cavity.

Angioleiomyomas
are rare smooth-muscle tumors that occur anywhere in the body. We present the case of a 46-year-old woman with a 3-year history of a painless angioleiomyoma in the right-hand fourth web space with the typical histologic features of tortuous vascular channels and thick muscular walls. In the 39 cases of angioleiomyoma of an extremity treated at the Mayo Clinic, we found a male-to-female ratio of 1:2.2, a mean age of 52 years, and
pain
as the presenting symptom in 67 percent of the patients. The lesions usually were less than 1 cm in diameter, and they decreased significantly in size with increasing patient age. Simple excision was curative.
Angioleiomyoma
should be considered in all patients with nodular lesions of an extremity.

Patients with chronic pain constitute a major problem for the surgeon as the cause of their
pain
is often difficult to diagnose and is only rarely a well circumscribed lesion which is amenable to surgical or other treatment. We report a case of chronic pain due to a vascular leiomyoma beneath the deep fascia of the leg.
Angioleiomyomas

Angioleiomyoma
is frequently painful and the cause of the
pain
is unknown. The purpose of this study was to compare the mast cell population and innervation of painful and painless angioleiomyomas. Twenty-four cases of angioleiomyoma were examined; 16 painful and 8 painless cases. Pinacyanol erythrosinate and antibodies to protein gene product (PGP) 9.5 were used to demonstrate mast cells and nerves respectively. PGP 9.5-immunoreactive nerve fibres were found in most of the painful (13/16 cases) and painless lesions (5/8). Mast cells were not seen in half of the painful lesions but were seen in most of the painless lesions (7/8). The median mast cell density was 1.1 cells/mm2 for the painful lesions and 21.9 cells/mm2 for the painless lesions (P = 0.048, Mann-Whitney test). The lower mast cell density in the painful lesions may reflect increased mast cell degranulation. It is proposed that neural and vascular events, similar to those occurring in the triple response to mild cutaneous injury, may produce
pain
in angioleiomyoma and other painful skin tumours.

Angioleiomyoma
is a solitary subcutaneous tumor characterized by
pain
in about half of patients with this tumor, and the pathogenesis of this
pain
has been a cause of much debate. To clarify the mechanism of
pain
and cytoskeletal property of tumor cells, 50 angioleiomyomas were studied clinicopathologically and immunohistochemically. The tumors occurred preferentially on the extremities, particularly the lower leg (46%), and the female to male ratio was 1.9:1. They were classified into three histological subtypes: (i) solid (30 cases); (ii) venous (15 cases); and (iii) cavernous (five cases). The
pain
and/or tenderness were present in 26 out of 49 patients (52%), in which small nerve fibers immunoreactive for S-100 protein and PGP9.5 were identified within the capsule of 20 tumors (77%) and the tumor stroma of 18 (69%), irrespective of the histological subtypes. In 24 patients where the
pain
was absent or unknown, nerves were observed within the capsule of 19 tumors (79%) and tumor parenchyma of 10 (42%). Many cells in all 50 tumors were positive for alpha-smooth muscle actin, and a relatively large number of cells in many tumors were positive for vimentin, desmin and collagen type IV. Also, cytokeratin (CAM5.2) reactivity was scattered in a few cells of four tumors. From these findings, the peculiar
pain
of angioleiomyomas could be mediated by the nerve fibers especially located within the tumor parenchyma. Although the expression of intermediate filaments in angioleiomyomas was heterogeneous, the overall cytoskeletal features were of smooth muscle cell differentiation.

Angioleiomyoma
of the nasal cavity is an extremely rare tumour (Sawada, 1990). We present an unusual case of angioleiomyoma arising from the lateral wall of the nasal cavity without any symptoms of
pain
or epistaxis. Another unusual feature was that the tumour clinically resembled an inverted papilloma.

The authors report a case of an angioleiomyoma at the anteromedial side of the knee mimicking a neurinoma of the infrapatellar branch of the saphenous nerve.

pain

We report a case of an angioleiomyoma occurring in a 40-year-old Kenyan female. The patient presented with recurrent
pain
and a soft tissue swelling at the posteromedial aspect of her right knee.
Angioleiomyomas
are benign soft tissue tumours, which occur most commonly in the skin of the lower extremities in middle-aged females. They very rarely occur in the knee and are treated curatively with resection. A brief literature review is included.

Angioleiomyoma
, a benign soft tissue tumor composed of smooth muscle cells and vascular endothelium, occurs most commonly in the extremities, the lower leg being a common site of occurrence. It rarely is found in the head and neck area, especially in the nasolabial groove. Surgical excision is the gold standard for diagnosis and treatment of angioleiomyoma; a preoperative diagnosis may be difficult. Here, a case of angioleiomyoma found in the nasolabial groove and associated with toothache is presented.
