as an enthesiopathia resulting from an innervation disorder. We believe that also

can be caused by nerve root lesions. As a consequence, a change in conservative and operative management is necessary. The aim is a balanced gliding and rotational stability of the patella. This means a muscular compensation of the weakened muscle part and stabilisation of the lumbar and hip region. Surgical techniques should only be applied to normalize the gliding of the patella. There is no indication for ventralisation of tuberositas tibiae.

and osteoarthrosis the maximal isokinetic knee extension strength was measured. The

evoked during the recordings was rated on a nine-grade scale. The peak extension torques were markedly lower than in the non-diseased knee and in a reference group of healthy volunteers. Twenty months after advancement of the tibial tuberosity, the muscle strength was significantly increased and
pain
significantly alleviated, well parallelling the clinical improvement. Intra-articular anaesthesia almost instantaneously increased the knee extension torque considerably, although knee pain was unchanged.

The results indicate that arthroscopic evaluation of patellofemoral motion is an accurate way to define patellar malalignment. In a retrospective review of 184 knees treated for surgically diagnosed
chondromalacia
of the patella, the average follow-up period was 3.9 years. The treatment rendered included trephine and drilling in 71 knees, facetectomy in 57 knees, shaving in 35 knees, and a combination procedure in 21 knees. Knees were graded as 2, 3, or 4, depending on the depth and severity of the lesion. The best results were in Grades 2 and 3 disease treated by trephine and drilling or facetectomy. Six patients required further patellar surgery during the follow-up period. The purpose of surgical treatment of the patellar articular surface is to relieve

and preserve the mechanical advantage of the patella for normal knee function.

Five baseball pitchers, three college and two professional, with an average age of 24 years, exhibited
pain
between the acceleration phase and follow-through phase of the pitching motion. This caused the players to be unable to continue at the level of competition necessary to play. A significant osteophyte on the posteromedial aspect of the olecranon process was identified in all pitchers. This caused impingement with the articular wall of the olecranon fossa and often created an area of
chondromalacia
. The more commonly identified posterior osteophyte was present in all cases. However, if just this posterior osteophyte is removed, the described lesion will be missed, with resultant persistent disability. Surgical excision of the posteromedial osteophyte through a relatively atraumatic posterolateral approach allowed early return of function without morbidity. With an average follow up of 1 year, all of the pitchers returned for one full season at maximum effectiveness.

The tendon of the extensor of the fifth toe was used in five patients and the tendon of the semi-tendinosus muscle used in 22 patients. Followup ranged from 2 to 9 years, averaging 5.4 years. The stability of the reconstructed ligament did not change during the followup if integrity of other ligaments was preserved or reconstructed.
