has led to confusion regarding its true meaning. This term should not be used to diagnose patellofemoral

, but to describe lesions of articular cartilage. Recent advances using stereophotogrammetry to evaluate the patellofemoral joint and its articular cartilage may lead to new insight into the etiology of these lesions.

In patellofemoral disorders, some cases respond well to conservative management thus the authors' initial treatment is conservative. The Patellar Band (PB) was reported previously (Nakamura et al., 1987). Since then the indications for the band have been investigated. Sixty four patients treated by the PB without operative treatment were classified into eight groups. The Severity of Dysfunction (SOD) was assessed by three grades. The First Grade is dull

on climbing up and down stairs, the Third Grade is a feeling of insecurity. The grouping was as follows: Group Ia - plica syndrome with first Grade of SOD and Ib with Second Grade of SOD. Group IIa -

with First Grade of SOD and IIb with Second Grade of SOD. Group III - maltracking patella with patellar

on flexion. Group IVa - subluxation or dislocation of patella with no previous history of patellar symptom and IVb - recurrent dislocation. Group V - degenerative change of the patella. The PB has been proved to be most effective in Groups Ia, IIa and IVb although it is beneficial in half the cases in Groups IIb and III. The subluxation of the patella was partially reduced without recurrence of dislocation during sports activity and the feeling of insecurity was relieved by the PB. The overall results were not related to age or activity level of the patient. The indication of the band for painful knees was not clearly determined in this study. In all operated cases, it was effective for postoperative instability after lateral release of the retinaculum.

The patellofemoral joint is one of the most common sources of painful symptoms encountered by athletes and is one of the most frequently injured joints. Patellar incongruency is the primary pathologic condition that affects the patellofemoral joint and has been reported to be associated with patellar subluxation, patellar dislocation,

We studied all patients undergoing arthroscopic resection of symptomatic plica without other intraarticular abnormality at our institution from October 1981 to March 1987. To be considered abnormal, plicae had to be thickened and/or fibrotic when viewed arthroscopically. Seventy-six of nearly 2000 patients (4%) who underwent diagnostic arthroscopies met our inclusion criteria. Clinical response after arthroscopic resection was evaluated in 51 patients at an average of 47 months (range, 15 to 77). Excellent or good results were obtained in 57 (75%) of the patients. Eleven patients (14%) had an impingement lesion defined as a localized femoral condylar ridge or groove of the articular surface that impinged upon the plica with increasing flexion. All of these patients had an excellent or good result. Other factors associated with a favorable outcome included a specific preoperative diagnosis localizing symptoms to the medial compartment, onset of
pain
after a period of increased athletic activity or after a twisting injury, and younger age. Poor prognostic factors included associated
chondromalacia
and an unclear preoperative diagnosis.

. The exact cause of this condition has long been debated, although a causative relationship between meniscal tears and spontaneous osteonecrosis about the knee has been postulated. Seven patients with knee pain, meniscal tears, and

without initial evidence of osteonecrosis at magnetic resonance (MR) imaging underwent arthroscopic surgery with meniscal recontouring or repair and cartilage shaving. These patients returned within 2-14 months with recurrent

in the treated knee. MR imaging then demonstrated abnormalities consistent with osteonecrosis. Osteonecrosis of the femoral condyle or tibial plateau may be a late sequela of meniscal injury in association with
chondromalacia
and arthroscopic surgery. This diagnosis should be suspected in patients with recurrent knee pain after arthroscopic repair of meniscal tears. The precise relationship of this pattern of osteonecrosis to that previously described as spontaneous requires further investigation.

A new operative technique in arthroscopic treatment of
chondromalacia
using ultraviolet laser systems is introduced. The postoperative results are evaluated in a prospective and randomized clinical trial. One hundred and forty patients stage II or III
chondromalacia
according to Outerbridge were randomly assigned to arthroscopic operation using either laser or mechanical instruments. After a 6-month follow-up period the clinical results were compared, guided by a specially designed modification of the Lysholm scoring scale. In the short-term follow-up laser surgery gave superior results in regard to reducing
pain
(P less than 0.05) and leading to a lower incidence of reactive synovitis (P less than 0.01). No difference was found in respect of disability and functional impairment. Our results lead to the conclusion that arthroscopic laser application seems to be a successful procedure in the treatment of degenerative cartilage disorders, providing precise ablation of tissue without significant thermal damage to the remaining cartilage.
