A general model of the autonomic neuroeffector junction is proposed. In this model, emphasis is placed on the muscle effector bundle with electrotonic coupling between individual cells via gap junctions (or nexuses) and en passage release of transmitter from autonomic nerve
varicosities
. This release results in transmission to effector cells across junctional clefts ranging from about 20 nm in the vas deferens and iris to as much as 2000 nm in some large arteries. The ultrastructural identification of different autonomic nerve types is described. Current theories on the synthesis, storage, release, and inactivation of transmitter during cholinergic, adrenergic, and purinergic transmission are summarized. Some speculations are made about the possible involvement of purinergic nerves in the innervation of vessels and mast cells in the skin, and whether this involvement results in a functional link between ATP, histamine, bradykinin, and prostaglandin in cutaneous vasodilatation. Another possibility considered as the basis for this reflex is the release of substance P from sensory (
pain
) nerve collaterals in the skin.

Oral naproxen in doses of 500 mg and 750 mg daily was compared with oral indomethacin, 75 mg daily, in a double-blind, completely randomized study of patients with post-operative
pain
after out-patients
varicose vein
surgery. Altogether, 120 patients were studied. In the study 750 mg naproxen proved to be equal in respect of analgesic efficacy to 75 mg indomethacin, and it was clearly superior to 1500 mg acetylsalicylic acid. Naproxen, 500 to 750 mg daily, afforded adequate post-operative analgesia in 98% of patients. The side-effects were mild. On the basis of the study, naproxen can be recommended as a
pain
-relieving drug after minor surgery, particularly when an antiphlogistic effect is also desirable.

with a new regimen which involves intermittent compression dressings. In a fully randomized between-patient prospective trial it has been found that the use of such dressings results in a significant reduction in postoperative

had a higher psychoneurotic score in each case. Within the mastalgia group no difference in scores was observed between patients with cyclical mastalgia and those with mastalgia due to periductal mastitis. Both groups of surgical outpatients had significantly lower scores in major traits than the psychiatric group, except for a small group of patients with breast

who persistently failed to respond to treatment. Patients with mastalgia are therefore no more "neurotic" than those with

Five hundred and sixty-four patients undergoing abdominal surgery, who were carefully assessed at the time of operation for evidence of venous thrombosis, have been followed up to determine the incidence of leg symptoms,
varicose veins
and post-thrombotic syndrome. Patients are frequently troubled by
pain
, swelling and phlebitis, which can persist for up to a year after operation. New
varicose veins
developed by 1 year in 20 per cent of patients. These occurred with increased frequency in patients who also developed a deep vein thrombosis, but they were also seen in patients who showed no clinical or isotopic evidence of thrombosis. The post-thrombotic syndrome was present in 26 patients by 3 years after the operation; half of these patients had suffered the syndrome before the definitive operation. Assessment of the long term effects of venous thrombosis must be carried out against the background of similar effects seen in patients without thrombi, and the development of the post-thrombotic syndrome may best be considered as the summation of a number of incidents, overt or occult, occurring throughout a lifetime.

While current views of the incidence, histology, and cytogenesis are presented, the main purpose of this article is to call attention to the identifying clinical and radiological characteristics of these spinal tumors. A discrepancy between complaints of
varicose veins
of the legs such as stabbing in the legs,
pain
in the legs, tiredness of the legs, heaviness of the legs, feeling of swelling in the legs and nocturnal cramps in the calves, and anatomical signs of
varicose veins
in the legs was observed. Therefore these complaints were analyzed in 345 antenatal patients regarding their psychological aspects. No complaints of venous insufficiency were found in 47% of the pregnant women. Of the 53% antenatal patients with complaints, 24% had mild, 18% had moderate and 11% had severe complaints. Antenatal patients with well noticable neurotic personality traits in the MPI had complaints twice as often as women without neurotic traits (p smaller than 0.05). Introverted antenatal patients (according to the MPI) had more venous leg complaints than expected (p smaller than 0.05). Pregnant women with frequent psychosomatic complaints and altered general well being had the highest incidence of
varicose veins
complaints (p smaller than 0.001) or p smaller than 0.05). In addition to these psychometric data, there was also a correlation of the gynaecological history with complaints of venous insufficiency in the legs during pregnancy. Pregnant women with a very painful menarche had more
