Meningococcus endophthalmitis is exceptional. We report a case of ocular damage following type C meningococcus septicemia with no meningitis. A 20-year-old man reported to the emergency unit for polyarthritis pain in various joints, associated with chills, nausea, and diarrhea without fever. Ophthalmological examination revealed uveitis. A few days later, endogenous endophthalmitis was suggested because of a worsening general condition and fever spells to 39 degrees C. A hemoculture sampled on the patient's admission 4 days earlier revealed Neisseria meningitidis positivity. Meningococcus septicemia with no meningitis was diagnosed. Before the introduction of antibiotics, meningococcus meningitis was unfortunately frequent and ocular septic embolism was not a rare occurrence. The diagnosis of meningococcemia was delayed in our patient because of the atypical symptomatology and ocular manifestations in the forefront. As with any endogenous endophthalmitis, prognosis is bleak and it should be raised whenever suspected uveitis does not react to standard treatment.

In this study, we evaluate the distribution of nitric oxide (NO) in the serum of juvenile idiopathic arthritis (JIA) patients, correlating it with parameters of the severity of the disease. Ninety-seven patients with mean age 11.7 years and disease duration 4.8 years, showing active disease or not, grouped as oligoarticular (n = 34), polyarticular (n = 29) and systemic (n = 34) group, presenting uveitis and positive RF with erosive arthritis or active disease and erosions had significantly high levels of NO than the inactive ones. NO correlated with TNF-alpha in the oligoarticular subtype (P < 0.03), with pain in the polyarticular subtype with active disease (P < 0.04) and with ESR in the systemic subtype with active disease (P < 0.03). TNF-alpha concentration was high in all patients with active disease, accompanying NO production. The data confirm the production of NO in JIA patients, indicating a possible positive correlation between the production of NO and severity of the disease.

Uveitis is an ocular disease frequently encountered in dogs and cats. Its importance relates to the fact that it can induce severe ocular pain and or result in permanent loss of vision. In addition, uveitis can be the first or only manifestation of a systemic, life-threatening disease. Since uveitis can present in a manner similar to a number of other ocular conditions, it behooves the clinician to first establish a prompt, accurate diagnosis in order to give the appropriate prognosis and treatment.The first part of this review includes applied anatomy and physiology of the uveal tract, followed by a description of the various etiological factors found in uveitis. The second part deals with clinical signs and findings in anterior and posterior uveitis and with the sequelae of this ocular inflammatory process. Lastly, the third part contains therapeutic guidelines and a list of comprehensive reviews on this major topic in veterinary ophthalmology.

Extra-intestinal manifestations occur in at least 25% of Crohn's disease patients. Some extra-intestinal manifestations, such as erythema nodusum and peripheral arthropathy, will wax and wane in keeping with bowel inflammation. The more severe cutaneous ulcerations, uveitis, and axial arthropathy may precede bowel disease or persist after it subsides. Screening may be appropriate for eye disease and for osteoporosis to prevent complications. Medical management for extra-intestinal manifestations is similar to treatment for the bowel symptoms of Crohn's disease, with corticosteroids the mainstay. Pain and depression are associated with inflammatory bowel disease, and their control benefits patients. Recent small studies with anti-tumor necrosis factor (TNF) agents are promising for most extra-intestinal manifestations of Crohn's disease, and may permit more steroid-sparing disease control in the future.

The immune system has evolved to protect the host from microbial infection; nevertheless, a breakdown in the immune system often results in infection, cancer, and autoimmune diseases. Multiple sclerosis, rheumatoid arthritis, type 1 diabetes, inflammatory bowel disease, myocarditis, thyroiditis, uveitis, systemic lupus erythromatosis, and myasthenia gravis are organ-specific autoimmune diseases that afflict more than 5% of the population worldwide. Although the etiology is not known and a cure is still wanting, the use of herbal and dietary supplements is on the rise in patients with autoimmune diseases, mainly because they are effective, inexpensive, and relatively safe. Curcumin is a polyphenolic compound isolated from the rhizome of the plant Curcuma longa that has traditionally been used for pain and wound-healing. Recent studies have shown that curcumin ameliorates multiple sclerosis, rheumatoid arthritis, psoriasis, and inflammatory bowel disease in human or animal models. Curcumin inhibits these autoimmune diseases by regulating inflammatory cytokines such as IL-1beta, IL-6, IL-12, TNF-alpha and IFN-gamma and associated JAK-STAT, AP-1, and NF-kappaB signaling pathways in immune cells. Although the beneficial effects of nutraceuticals are traditionally achieved through dietary consumption at low levels for long periods of time, the use of purified active compounds such as curcumin at higher doses for therapeutic purposes needs extreme caution. A precise understanding of effective dose, safe regiment, and mechanism of action is required for the use of curcumin in the treatment of human autoimmune diseases.

A 10-year-old female child attended outpatients department of Regional Institute of ophthalmology, Kolkata with the complaints of gradual dimness of vision of both eyes, recurrent pain, redness and photophobia. On examination, she was found to have bilateral anterior uveitis, poliosis and vitiligo. Laboratory tests were performed to rule out other causes of granulomatous uveitis. She was treated with topical and systemic steroids and topical cycloplegic. Then pars plana vitrectomy with lensectomy was carried out. The vision still remained poor.

Low back pain is a frequent symptom to consult the general practioner or the osteo-articular specialist. It is important to identify patients with inflammatory back pain (5 to 10 %) because the management and the treatment are different. Spondylarthropathies, especially ankylosing spondylitis (AS) are the most frequently encountered. The inflammatory characteristics of pain give clues for the diagnosis and allow to make appropriate management. Biological tests and imaging from X rays to MRI are used. As therapy is primarily based on NSAID use, in case of unsatifactory response in peripheral arthritis, disease modyfing drugs such as salazopyrine and methotexate (MTX) are used. Anti TNF are used in AS patients unresponsive to previous therapy (infliximab, etanercept, adalimumab) with a response in 50% of the patients. Etanercept is however not indicated in patients with uveitis.

We report a case of 45-year-old male who presented with polyarthritis and posterior uveitis. He was given azathioprine (AZA) 150 mg day(-1) and methyl-prednisolone 48 mg day(-1) for his uveitis and polyarthritis. Eye and joint complaints improved with these medications within 1 month. Three months later, while he was on AZA, the patient presented with pain on his anterior chest wall associated with diffuse hyperemic and warm mass lesion. A wedge biopsy of the lesion was reported as diffuse large B-cell NHL. Here, we briefly discuss the relationship between arthritis, uveitis and lymphoproliferative disease.

We present the case of a 16-year-old male teen-ager admitted in the Ophthalmology Clinic Craiova, diagnosed with Anterior neglected acute uveitis in the left eye. Before and simultaneous with the ocular manifestations, the patient complained of pain on the hips joints. The pediatric examination, based on the clinical features and the laboratory findings, diagnosed him with Juvenile chronic arthritis, oligoarthritis type. The evolution was good under the corticosteroid therapy.

The clinical signs of uveitis occur as a result of inflammation within the vascular coat of the eye, which causes breakdown of the blood-aqueous barrier and blood-retinal barrier. Many infectious and noninfectious causes can incite episodes of uveitis. Although a complete diagnostic evaluation is highly recommended to identify any underlying etiologic agent, many cases remain idiopathic in nature. The goals of therapy are preserving vision, minimizing pain, and halting inflammation.
