A case of Krukenberg's tumour of the ovary in a 41-year-old patient is described. A year before admission she had mild gastric trouble, repeated short-lasting amenorrhea followed by prolonged bleeding. The menstruations preceding hospitalization were normal and painless. More serious trouble developed suddenly, four days before admission, in the form of intense pain in the lower abdomen and moderate bleeding. The woman had delivered twice and had 6 abortions. Her mother died of intestinal cancer. On examination, the patient was found to have two tumours: one on the right side, the size of a newborn's head, and one on the left side, the size of a female fist, both kidney-shaped and mobile. Also numerous small knots on the small intestine, peritoneum, and omentum, as well as a considerable amount of ascites were observed. An uterine myoma was found as well. Histerectomy and ablation of both adnexa were performed. The therapy was continued by radiation but the patient died 5 months following the operation.

Leiomyoma of the duodenum is a rare condition which is most often diagnosed only as an incidental finding at post-mortem. It may present clinically with pain or haemorrhage. A case is described of degeneration of a leiomyoma of the fourth part of the duodenum giving rise to pyogenic liver abscess. There is no previous report of this association in the literature.

The case report of a glass specimen bottle which accidentally dropped into the peritoneal cavity at time of laparotomy for a ruptured right tubal pregnancy is presented. It had come to rest on the fundus, open end upward, and during respiration loops of small intestine entered and exited the bottle. When the complaint of pain brought the patient to the authors 6 years later a diagnosis of fibroid uterus was made. It is suggested that all materials used in the abdomen be radio-opaque and that routine postoperative X-rays be taken before the patient leaves the surgical suite.

A total of 678 women were interviewed about symptoms before as well as advantages and disadvantages after abdominal hysterectomy. Medical complications, side effects, days of hospitalization and some possible differences between supravaginal hysterectomy (SVH) and total hysterectomy (TH) were also investigated. TH had been performed in 79% and SVH in 21% of the women. Leiomyoma was the most frequent diagnosis (79%). Heavy menstrual bleeding and dysmenorrhea were the dominant symptoms before surgery. After the operation, 71% of the women had no subjective gynecological complaints. Relief from heavy bleeding and pain was considered the major advantage and as many as 29% of the women experienced no disadvantage at all with the operation. Regarding sexual life, 39% experienced intercourse as much improved or better and 40% as unchanged. An intraabdominal abscess was more often found postoperatively in patients operated with TH than with SVH but with this exception no objective finding favoured one method more than the other. When new, alternative treatments such as endometrial ablation and GnRH analogues are introduced and promoted they should be subjected to careful evaluation and compared with the therapeutic efficacy of hysterectomy.

The study included 211 women using Cu-T 380A, of whom 155 presented with complaints related to IUD use. All women were examined by real-time ultrasound using abdominal sector and/or vaginal scanners. The parameters that were studied included the top-fundal distance (distance between the device top and the highest point in the uterine cavity), the intercornual and the fundus-isthmus diameters (of the uterine cavity). Any abnormal finding was also recorded. About 50% of complainers had top-fundal distance more than 4 mm, compared to 28% of non-complainers. An intercornual diameter, too small (less than 30 mm) or too wide (greater than or equal to 38 mm), was significantly more frequent in women complaining of bleeding and pain. Other abnormal findings were diagnosed in 25% of complainers compared to 7% in those without complaint. These findings included partial expulsion, appearance suggestive of pelvic inflammatory disease, ovarian swellings, embedding and fibroid uterus. Ultrasonography can give useful insights in managing IUD-related complaints in selected cases.

There is a 24% complication rate after first trimester induced abortions using traditional methods. This rate increases significantly in women who have a history of severe gynecological diseases (SGDs), e.g., uterine myoma, scars on the uterus, and developmental defects of the genitals. The goal of this investigation was to shed light on the effectiveness of miniabortion among pregnant women with SGDs. 61 patients aged 21-43 years with SGDs (33 with uterine myomas, 22 with scars, and 6 with uterine developmental defects) who underwent abortion by vacuum aspiration were studied. All women had had 1-3 births and 1-11 abortions. Only one patient with scars required dilatation of the cervix to Hegar 6. 85% of the patients tolerated the operation well, while 15% either had lower abdominal pain or vertigo, which abated soon after the procedure. 3-10 days after the operation slight menstruation reactions developed in 97% women that lasted 1-3 days. In 11% of cases the menstruation reaction was accompanied by pain. In two patients with uterine myoma the bleeding did no stop within 5 days after the abortion. One of these patients had profuse bleeding with clots, and 13 days after abortion she developed postabortal endometritis. One patient with scars became pregnant. The rate of complications amounted to 3.3%. The high risk of postabortal complications owing to the defects of the myometrium necessitated the administration of weak uterine drugs, such as ergotal. Trihopol was also administered for the prevention of inflammation. Starting the fifth day after the beginning of menstruation-like reactions, the patients were started on estrogen-gestagen preparations for contraception for three menstrual cycles. After the end of the menstrual reaction, they were enrolled in rehabilitative physiotherapy: electrophoresis or ultrasound applied to the area of the uterus. Regular menstruation usually resumed 20-30 days after the beginning of the menstrual reaction. The highly effective miniabortion curettage is recommended because of negligible risk of trauma.

Basing on the concept of the uterine myoma presenting as a psychosomatic process that manifests in ischemic disease of the uterus, the author has tried psychoelectrostimulation in 76 patients with interstitial and subserous interstitial myomas as a method of psychotherapy and electrotranquilization. Indications for such treatment are the presence of psychoemotional stress syndrome (high anxiety level, pain and hemorrhagic syndromes, dysuria) and a rapid growth of the tumor. The reference group consisted of 73 patients with the same myoma forms, treated by hormonal therapy. Psychoelectroregulation has exerted a regulating effect on the central nervous, cardiovascular, endocrine and reproductive systems, it eliminated or alleviated the neurotic symptoms, and slowed down or arrested the tumor growth.

Leiomyoma of the esophagus was surgically treated in 15 patients in 1962-1987. Six patients were asymptomatic and nine presented with dysphagia alone or combined with retrosternal pain, vague thoracic discomfort, dyspnea and cough, or palpitations. Barium swallow and esophagoscopy provided the correct diagnosis preoperatively in most cases. Transthoracic enucleation of the tumor was performed via right thoracotomy in nine patients and left thoracotomy in six. The location of tumor was the upper third of the thoracic esophagus in three cases, middle third in six and lower third in six cases. There was no surgical mortality or morbidity. Surgical removal of esophageal leiomyoma thus gave relief of symptoms, with minimal risk and excellent functional outcome.

Seven women with endometriosis and three women with uterine myomas were treated for six months with tryptoreline (Decapeptyl, Decapeptyl Depot, Ferring). In six patients with endometriosis improvement was achieved according to the AFS classification and the finding on palpation and ultrasonic examination. In three patients with uterine myoma a reduction by 25-100% was recorded. Dysmenorrhoea and pain in the hypogastrium disappeared completely in all patients except one. The LH and FSH levels declined and E2 levels were similar as in the menopause. The menstrual cycle disappeared in the course of treatment and all women experienced flushes. Daily s. c. and monthly depot doses gave similar results. Four months after treatment the hormonal levels and the menstrual cycle were normal. Endometriosis disappeared twice, four times minor endometriotic nodes persisted, once a uterine myoma of half the size reappeared. Treatment with LH-RH agonists in endometriosis matches danazole treatment in uterine myomas it is suitable for special cases.

Leiomyoma of the esophagus is not uncommon in the adult population but is rarely seen in children; only 20 cases have been reported in the pediatric population. In this paper we describe two cases of esophageal leiomyoma in female patients aged 6 and 13 years and review previous reports. Several differences were noted between the pediatric and adult population. The mean age in children is 14 years (range, 4 to 20 years). Leiomyoma appears 1.71 times more often in females than in males. Localized lesions are found in only 9%, whereas the diffuse form predominates in 91%. The entire esophagus may be involved 35% of the time, and encroachment on the cardia or upper stomach occurs in 70%. Leiomyomas associated with familial syndromes (familial leiomyoma and Alport's syndrome) occur in 22% of the cases. Major symptoms include dysphagia (86%), dyspnea (36%), vomiting (27%), retrosternal pain (27%), and coughing (22%). The initial diagnosis following contrast studies is most often achalasia. The diagnosis of leiomyoma is made only with subsequent endoscopy. Enucleation was performed in only 11% of the cases; surgical resection (including part of the stomach) was necessary in 78% with a 21% postoperative mortality. Esophageal leiomyoma should be considered in the differential diagnosis of mediastinal masses and esophageal obstruction. Accurate preoperative diagnosis is desirable in order to plan proper surgical treatment.
