It is the attempt of this study to determine the efficacy of response to acupuncture at 24 hours, one week, and three week intervals at the end of a series of treatments in 328 patients. Treated for a variety of disorders, approximately 60% had satisfactory responses of up to 3 weeks duration. The 328 patients were classified into 13 categories of problem disorders: headaches, cervical
pain
, lumbar
pain
without radiation, lumbar
pain
with radiation, isolated sciatica, parathoracic
pain
, knee pain, elbow pain, shoulder pain, gereralized musculoskeletal pains (
rheumatism
), neurological disorders, and a general miscellaneous category. The problem disorders were then arranged into etiological categories. In general for each problem disorder, except for neurological, the average response ranged in the 60-65% range at the third week interval.

Five patients with systemic lupus erythematosus (SLE), four of whom died with colonic perforations, are reported. Perforation of the colon constituted the most frequent cause of death among 107 patients with SLE admitted to the
Rheumatic Disease
Unit during a three year period. All five patients with colonic perforation had clinical and laboratory manifestations of active SLE in addition to the abdominal syndrome. Most striking was evidence of active arteritis in all patients with either central nervous system involvement and/or peripheral arteritis, in addition to that found in the gastrointestinal tract. Hyperglobulinemia and rheumatoid factor as well as antinuclear antibodies were present at some time in all patients. The abdominal syndrome was characterized by the insidious onset of lower quadrant
pain
which was intermittent and colicky. Although direct abdominal tenderness was eventually present in all patients, rebound tenderness and hypoactive bowel sounds were variable and abdominal rigidity occurred only in one patient and late in the course. The differential diagnosis of abdominal pain in SLE is reviewed and possible mechanisms for the production of colonic perforations are discussed. It is suggested that the presence of rheumatoid factors in conjunction with circulating immune complexes may be the pathogenetic mechanism via the production of a mesenteric arteritis.

Improvement from gold therapy is not immediate but comes about very slowly. The patient and physician should be forewarned that as many as 8 to 14 weeks may pass before any benefit is noted, and that months of treatment may be required before improvement is sufficient to decrease disability. With effective therapy, signs of inflammation gradually decrease,
pain
abates, and function improves. Not all patients benefit, and whether beneficial results occur can be determined only by clinical trial. There is no advance method of determining whether treatment will be effective. Gold therapy suppresses rheumatoid inflammation and checks the progress of the disease. It should not be used alone, but shoud be part of a well-planned program of treatment carefully adjusted to the needs of the individual. This therapy will include steroidal and nonsteroidal anti-inflammatory agents (including aspirin), which should not be reduced or abandoned until definite improvement from gold can be determined. With the possible exception of palindromic and juvenile forms, RA is the only rheumatic disease benefited by intramuscular gold therapy. The treatment should never be used for other forms of
rheumatism
.

A study was made of 45 patients suffering from hydroxyapatite
rheumatism
(multiple tendon calcifications disease). There were 36 women and 9 men aged between 15 and 61 years with an average age of 14. The following joints were involved (the first figure refers to clinical affection, that between brackets to radiologically demonstrable calcifications): shoulder 34 (36); neck 14 (15); wrist 18 (11); fingers 19 (17); hip 11 (29); knee 14 (18); ankle 10 (14); foot 5 (6); spinal column 16 (24). In 30 patients the disease manifested itself in the form of acute recurrent migratory arthritis resembling gout. In 8 cases, it developed in the form of acute recurrent migratory arthritis resembling gout. In 8 cases, it developed in the form of acute polyarthritis and in 7 as rheumatoid arthritis without radiological lesions. Thirty-two patients could be followed up. Four of them were cured, 9 showed improvement but still suffered attacks of
pain
. Eighteen patients were not improved, their pains growing more chronic. Four patients had a family history of multiple tendon calcifications disease.

For the physiotherapy of rheumatic diseases principles of order are recommended. The aims of physiotherapy (allevition of
pain
, improvement of mobility, inhibition of inflammation), dosages of physiotherapy (intensity of treatment, time of treatment, region of treatment), quantification of
pain
and reaction to treatment are in the foreground in these cases. With regard to an appropriate choice of methods these principles of order are explained at the instance of the rheumatoid arthritis and the ankylosing spondylitis. Concerning the
rheumatism
of the soft tissues a classification of the therapy according to localisation and course of the disease (acute, recurrent, chronic) is explained.

The occurrence of "objective" symptoms of TMJ -disorder, as measured by clinical examination, and self-reported "subjective" symptoms, were investigated in a survey of 241 persons ages 65-79. This group constituted the participants of a sample which was drawn to cover all persons in this age category living in the county of Troms in Norway. The participation rate was 86. Participants were examined and interviewed by dentists in graduate training. Eight per cent of the group reported to have had TMJ
pain
recently. Fourteen per cent said they had clicking or creptiation. These symptoms were twice as commonly reported by women as by men, and they were also most frequent among people complaining of
rheumatism
or general joint pain. By the clinical examination, clicking and/or crepitation was found in 27% of the individuals. Limitation of jaw movement was registered in only 3 persons. These "objective" symptoms were evenly common with both sexes. Dental/prosthetic status had seemingly no distinct influence of the findings.

Rheumatic symptoms that are a somatic expression of emotional disturbance are common, and often are mistakenly attributed by physicians to organic disease. Psychogenic rheumatism is encountered most frequently among middle-aged women. The typical complaint is of widespread
pain
and stiffness, often with report of swelling and paresthesia, but symptoms characteristically are vague. Overt psychiatric disturbance may be apparent. Distinction from organic disease should not be difficult if physicians recognize the typical presentation, and find that symptoms fail to correspond to patterns of organic disorders. When organic disease coexists with psychogenic
rheumatism
, as often is the case, the objective abnormalities are insufficient to explain the symptoms.

Plasma-free tryptophan is inversely related to the severity of subjective
pain
in 8 patients who fulfilled criteria for a variety of non-articular
rheumatism
, the "fibrositis syndrome". The observation is consistent with animal and human studies suggesting a relationship between reduced brain serotonin metabolism and
pain
reactivity.

In a double-blind study, diclofenac sodium (Voltaren)--administered for 14 days in a dosage of 25 mg t.i.d.--was compared with naproxen (250 mg b.i.d. for 14 days) in the treatment of 120 patients with soft-tissue
rheumatism
. Assessment of the efficacy of the two treatments was based upon changes, recorded once a week, in the following parameters:
pain
at rest and on movement, swelling, local tenderness, functional impairment, limitation of movement, and sleep disturbances. The incidence of unwanted effects was also recorded, and the effects themselves were classified according to the probability of their being causally related to the trial preparations. Both drugs appeared to be effective in relieving symptoms associated with soft-tissue
rheumatism
. In most indications the therapeutic efficacy of the two preparations was similar. In patients suffering from diseases affecting the shoulder region, however, diclofenac sodium was significantly more effective. Unwanted effects rarely occurred with either drug.

A survey of the public's attitude to arthritis was commissioned by the Arthritis and
Rheumatism
Council (ARC) in 1974. The survey was carried out by National Opinion Polls (NOP) Market Research Ltd. The ARC were interested to find out whether people underestimated arthritis and what they thought about the consequences. Respondents were asked to rank a number of illnesses in terms of their seriousness and the loss of work to which they give rise. They were also shown a series of statements about arthritis and
rheumatism
in order to elicit the extent of their agreement with the views these represented. The public appear to have fairly realistic ideas about the magnitude of the problem and of its nature and associations. However, although the
pain
of arthritis and
rheumatism
was appreciated, their possible impact in terms of disturbance of the normal pattern of daily life tended to be underestimated. The findings reflect the climate of opinion from which present and potential arthritis sufferers come and may therefore reveal part of their attitudes to their disease.
