In 1973 LeVeen et al. described a new technique for extensive, retroperitoneal disobliteration of the aorto-bifemoral tract using the arterial disobliteration device (ADD). In Europe this technique was first described in 1981 by Widdershoven and Willekens. Later on more publications from Spain, Poland, Belgium and the Netherlands appeared. At the Groot Ziekengasthuis in 's-Hertogenbosch (the Netherlands) the new procedure was introduced in 1984. Until 1991, 158 patients were operated. The results were analysed. The average follow-up time was 44 months (range 3-84). In 44.5% of these patients intermittent claudication was the reason for operation. In 40.5% of the cases there was untreatable ischemic rest pain and in 15% there was also ischemic gangrene of one or both legs. Aorto-bi-iliac disobliteration was performed in 55 patients, aorto-bi-femoral disobliteration in 31 patients and unilateral disobliteration in another 48 patients. In 24 patients we performed an unilateral aorto-femoral with a contralateral aorto-iliac desobstruction. Both the operation technique and the results are described. Five patients died in the early postoperative period and in 16 patients early reintervention i.e. within 6 hours after the initial operation was necessary for hemorrhage or acute vascular occlusion. During the follow-up 15 patients died. Recurrent occlusion was seen six times while in two patients restenosis developed. In conclusion; in selected patients with aorto-femoral occlusive disease the ADD-procedure can replace the up to now usual procedures.

Otinum, a new drug made in Poland, was used to treat inflammatory diseases of the middle ear of 40 patients. The drug was applied topically as drops or tampons inserted into the external acoustic meatus. As controls, 16 patients with identical problems, who were treated with 3% boric acid alcohol, were used. Otinum had a distinct pain-killing and anti-inflammatory effect, as well as a mild keratolytic effect. Otinum is recommended to be used for acute otitis media and exacerbated chronic otitis media in the absence of pathogenic microorganisms.

A total of 67 patients with treatment resistant chronic venous ulcers were admitted to hospital for 6 weeks of bed rest and daily dressings. The patients came from a rural area in Poland with poor socioeconomic conditions. They were randomized to treatment with either standard dressings or with cadexomer iodine. After 6 weeks all but four patients had shown a clear reduction of ulcer area; the mean reduction was 54% within the former group and 71% with cadexomer iodine. The latter treatment was significantly more effective than the standard hospital dressings in debriding the ulcer, accelerating healing and reducing pain. Elevation of serum concentrations of protein-bound iodine occurred after treatment with cadexomer iodine in patients with large ulcers, but tests of thyroid function showed no changes associated with the use of cadexomer iodine. It is concluded that cadexomer iodine significantly accelerates the healing of chronic, infected, treatment-resistant, venous ulcers in hospitalized patients.

The discussion is presented on certain factors influencing the motivation of the patients to undergo stomatological treatment. They included: 1. Increasing positive motivation to treatment: striving to alleviate pain caused by decayed tooth, realization of aims not related to health, cultural aspects. 2. Motivating treatment avoidance: fear of pain connected with the treatment, consequence of low availability of dentists in Poland unconscious anxiety connected with: violation of bodily integrity of the patient, manipulation in particularly important oral area, stomatologist's domination over patient.

The standardized interviews of 26 chronic back pain patients from central Europe (Switzerland, Germany, Poland) were compared with those of 28 patients from Mediterranean countries (Italy, Spain, Yugoslavia, Turkey), all of whom had been referred for participation in an integrated treatment program for chronic back pain. The two samples differed significantly in most of the psychosocial aspects studied. Patients from Mediterranean countries had a significantly lower level of education and were mostly employed as unskilled workers, while patients from central Europe were mostly housewives or skilled workers with higher levels of education. In the Mediterranean sample the back pain had generally developed much faster into a disabling disease with the attendant consequences (sick leave, loss of job, litigation). More than one third of the Mediterranean patients lived with a partner who was also sick and unable to work. These patients also adopted a more passive stance towards their illness by rarely using self-help, and showed poor readiness to participate in the self-monitoring orientated treatment program proposed to them. They were less aware of the influence of their own behaviour on pain and of any relation between the illness and their present or previous life situation. They complained less of a broken home in their childhood or earlier medical problems, but more frequently reported suffering from poverty in their childhood. The uneven composition of the two samples reflects the special social situation of foreign workers from Mediterranean countries in central Europe. Unskilled workers are significantly overrepresented in the latter segment of the adult working population of the study area (city of Basel, Switzerland). This overrepresentation is similar to that in our patient sample. The special situation of foreign workers from Mediterranean countries seems to account for their high incidence of chronic intractable back pain.

The factors involved in determining return to work following damage to the lower limbs were analyzed, on the basis of 519 patients (up to age 60) treated at the rehabilitation center of Konstancin, Poland. Correlations were determined between the results obtained in functional testing of pain complaints, movement range, muscle strength, locomotion and general fitness, and the patients' adjustment relative to gainful activity. It was found that the functional rehabilitation therapy carried out had in the majority of patients been appropriate in achieving the goals of treatment, i.e. patients' fitness, their range of movement and locomotion had been affected positively, and the occurrence of pain been reduced. The majority of patients had thus been enabled to return to work. Unsatisfactory outcomes, in the authors' opinion, have to do with severity/extent of lesion or articular fracture involving torsion, as well as with neglect of early-enough functional rehabilitation therapy, associated with a still frequently encountered, mistaken belief that rehabilitation constitutes the last stage in the post-traumatic treatment process.

An inquiry based on statistical data concerning rheumatic diseases in seven European countries was performed. The data reported were based on the ICD (8th revision, 1965). The time of reference was the period 1968-78. The countries concerned (Czechoslovakia, Finland, France, the GDR, Poland, Sweden and the United Kingdom) reported data on the following 'measures of frequency': rates of hospital discharge; rates of spells of sickness and incapacity for work; incidences of disability pension due to rheumatic disorders. The main results are threefold: The social importance of rheumatic diseases has been increasing in several European countries in the late 1960s and 1970s, though the situation differs from one country to another. There is sufficient evidence to assume a true increase in the occurrence of clinical symptoms among persons afflicted with 'degenerative arthropathy', i.e. osteoarthrosis and vertebrogenic pain. The increasing social importance of 'degenerative arthropathy' ought to stimulate a process of rethinking of the traditionally established priorities in rheumatological research and practice.

Midwives do not play an apparent role in family planning and in avoiding unwanted pregnancies. Nevertheless, they should pay more attention to this problem, primarily in the case of women who are less knowledgeable about family planning. Induced abortion should not be a means of family planning since it has many harmful consequences. Different means of contraception have been known for centuries, but the social movement for ""birth control"" dates back only to the 19th century. The International Society of Family Planning was organized in the 1950s, while the idea of birth control in Poland arose only in 1956. Before the amendment on abortion was added to the Polish constitution, the annual number of illegal abortions was about 300,000. In the era of legal abortions it is the task of physicians and midwives to propagate other means of family planning. Induced abortion results in complications in 2%-5% when carried out in the first 6-7 weeks of pregnancy, in 5%-10% in weeks 8-12 and in 20% after the 13th week. One of the most frequent complications is psychogenic. Among the women involved in a study conducted by Kokoszka, 104 out of a total of 500 women reported for a follow-up examination. Disturbance in orgasm occurred in 45 cases, decrease of sexual desire in 48 cases, reduction of sexual drive in 73 cases, and nervousness in 14 cases. The risks of induced abortion are ranked as 1) sexual nervousness 2) clinically diagnosable pain 3) general and organic nervousness. W. Poltawska adds the following. Psychological complications: depression and feelings of guilt, aggression and auto aggression, and prolonged changes in personality.

The authors describe the method of non-operative treatment of widely occurring low back pain syndromes diagnosed as due to isolated degeneration of the nucleus pulposus. In this phase of degenerative spinal disease pain is not due to nucleus pulposus prolapse. The patients are dissatisfied with the results of physiotherapy and rehabilitation, and surgical treatment brings no satisfactory results. The method of dissolving of nucleus pulposus, widely used in the USA, Australia and West European countries, has been used in Poland only in the clinical centre of the authors. The clinical experience gathered during these years made possible delineation of the syndrome of isolated nucleus pulposus degeneration and strict qualification for chemonucleolysis which gives success in about 90% of cases.

Vratizolin is a new non-steroidal anti-inflammatory drug registered for use on humans in Poland. Published and unpublished data on Vratizolin showed that it has anti-inflammatory, antiviral, antibacterial, antimycotic, analgesic and immunomodulating activities. The purpose of these randomized, parallel-group studies was to compare Vratizolin with other standard drugs, used for the treatment of mouth and ear infections. The study involved 193 patients with recurrent Herpes simplex, Herpes zoster oticus, Stomatitis herpetica and infections of the external ear canal. Vratizolin was used topically, as 3% hydrophilic cream or ointment, four times daily. Standard treatment included zinc ointment, Aphtin (boric acid plus glycerin), Oxycort and Dicortinef. In almost all of the treated patients the efficacy of Vratizolin treatment was superior to the drugs mentioned above. It was assessed by measuring disappearance of both objective (edema, erythema, crusting) and subjective symptoms (pain, burning and itching).
