Chlamydia trachomatis infection in the lower part of the genital tract of young girls and women may ascend to produce endometritis, salpingitis, perihepatitis, and other localized or generalized abdominal diseases. The resultant
pain
syndromes mimic a number of other common conditions that must be differentiated. A careful history and physical examination, with attention to historical and physical evidence of sexually transmitted disease, will alert the clinician to the possibility of chlamydial infection. Laboratory tests for C trachomatis may be helpful. However, tests of specimens from the lower genitourinary tract may yield negative results in patients with disease of the upper part of the genital tract and abdomen. Prompt recognition and treatment not only alleviate
pain
but also may help prevent inflammatory sequelae such as chronic painful adhesions, small-bowel obstruction, and tubal

The late luteal phase endometrial biopsy is an important part of the evaluation of patients with recurrent spontaneous abortion or

. The procedure can be painful and, if an adequate amount of tissue is not obtained, may have to be repeated. In this report, the results of endometrial biopsies performed in the late luteal phase with either the Novak curette or the PIPELLE Endometrial Suction Curette were analyzed retrospectively. The two techniques did not differ in the number of biopsy specimens found to have inadequate tissue for diagnosis; however, patients who underwent biopsy with the PIPELLE recalled significantly less
pain
associated with the procedure. When used in premenopausal patients undergoing endometrial biopsy during the late luteal phase, the PIPELE provided adequate tissue with preservation of glandular, stromal, and capsular architecture.

One hundred and thirty-one patients undergoing elective laparoscopic surgery for sterilization or
infertility
investigations were examined in a prospective study. Firstly, the contributions of the reason for undergoing the operation to the patients' psychological adjustment and recovery, is examined. Measures were taken of personality, mood, physical symptoms, vital signs and the patients' speed of returning to normal activities. The reason for laparoscopy does not appear to influence the amount of fear immediately before and after the operation, post-operative physical symptoms, nor the majority of mood states at follow up. There were significant differences between
infertility
and sterilization patients on state anxiety on the morning of surgery, vital signs, post-operative intramuscular analgesia, and the speed of returning to normal activities. In particular, sterilization patients did not report greater
pain
but received more analgesia, and returned to their normal activities more rapidly than
infertility
patients. Secondary
infertility
patients showed the slowest rate of return to normal activities, and reported more adverse mood states at 1 week follow up. The investigation of patients' attitudes to the operation revealed that patients undergoing sterilization and
infertility
investigation are concerned about the costs and benefits of the operation which include not only influencing fertility, but influencing menstruation and sexual activity. Patients misunderstood information about surgery, and the majority of patients welcomed additional psychological preparation. Suggestions concerning the management of patients undergoing laparoscopy for different fertility reasons are discussed.

was undertaken. Twenty-six of the 100 (26%) chronic pelvic pain patients and 34 of the 88 (39%)

patients exhibited pelvic adhesions as the only abnormal finding. Patients in each group with findings of pelvic adhesions were compared with respect to symptomatology, density of adhesions, and locations of adhesions. Only four of the 34
infertility
patients in whom pelvic adhesions were found complained of
pain
. Comparison of the chronic pelvic pain patients and the asymptomatic
infertility
patients did not reveal a significant difference in the density or the location of adhesions. These findings question the role of pelvic adhesions as a cause of chronic pelvic pain.

The major advantage of the argon laser in laparoscopic reproductive pelvic surgery is the ability to transmit the laser beam along a flexible fiber that can easily be directed at a lesion. Its use has been specifically described for the treatment of endometriosis because the absorption of the argon laser beam (488 + 514 nm) coincides with the pigmented endometriotic implants. It has been suggested that the argon laser would not be as useful in other
infertility
procedures because the lack of such pigmentation would reduce the lesion's absorption of the laser beam and therefore decrease the laser's efficacy of destruction. The argon laser was used in other kinds of reproductive pelvic surgery in 15 patients over seven months. Procedures performed laparoscopically with the laser were neosalpingostomy, fimbrioplasty, salpingo-oophorolysis, adhesiolysis and ablation of the uterosacral ligaments. Of six patients undergoing laparoscopic surgery with a preoperative diagnosis of pelvic pain, five reported complete resolution of the
pain
following therapy. There were no intraoperative or postoperative complications. In the short follow-up period, none of the patients conceived. In addition to its use in the treatment of endometriosis, the argon laser, transmitted laparoscopically via a fiber, can be safely used to perform a wide variety of other reproductive pelvic surgical procedures. Its relative efficiency with regard to these procedures, the subsequent pregnancy rates and the laser's ability to ameliorate pelvic pain remain to be established.

, and safety of argon laser photocoagulation of endometriosis were evaluated in 92 patients. All patients were treated at the time of diagnosis with the argon laser delivered through a flexible quartz fiber passed through the operating channel of a standard and unmodified laparoscope. The uncorrected pregnancy rate was 34% (19 of 56), with 64% of the pregnancies occurring within 6 months of therapy. The conception rate was slightly greater in women with
infertility
of 24 months or less and in women without other fertility factors. Ninety-two percent of 50 women with pelvic pain reported a reduction of their
pain
after treatment. The argon laser is an effective, simple, and safe alternative for the treatment of mild or moderate endometriosis.

The presence of a varicocele was confirmed in 121 of 123 patients complaining of scrotal
pain

, in whom radionuclide scanning was performed. In the first 29 patients (retrospective study), a high ligation of the internal spermatic vein was performed. In 94 patients, a percutaneous venography was done via the jugular vein followed by embolization by Gianturco coils (occluding spring embolus, Cook Group Company, USA) in 84 of them. In eight patients, due to technical difficulties during the embolization trial, a high ligation was performed at a later date. Eleven patients underwent embolization after failed high ligation. A posttreatment follow-up radionuclide study revealed recurrences in two of the 95 patients who underwent embolization. Embolization can be performed on an outpatient basis with minimal risks. Reembolization is feasible, and it in no way prevents a further high ligation should this be necessary.
