This article describes 41 examples of an unusual fibrohistiocytic sarcoma which occurred primarily in the extremities of young individuals between the ages of 5 and 25 years (median 13 years). It manifested as a nodular subcutaneous growth that seldom caused tenderness or pain, and clinically was often mistaken for a hematoma or a hemangioma. Grossly, the tumor presented as a circumscribed, multinodular or multicystic, hemorrhagic mass that ranged in size from 0.7 to 10 cm (median 2.5 cm). On microscopic examination, it consisted principally of 1) solid arrays or nests of fibroblast- and histiocyte-like cells, not infrequently containing varying amounts of intracellular hemosiderin or lipid, 2) focal areas of hemorrhage or hemorrhagic cyst-like spaces, sometimes occupying the major portion of the tumor, and 3) aggregates of chronic inflammatory cells, chiefly lymphocytes and plasmacytes, a feature that caused confusion with a lymph node metastasis in several cases. Follow-up information, available in 24 patients, revealed a variable clinical course. Twenty-one patients were alive, 11 with recurrence (including one with 9 recurrences in a 21-year period) one with recurrence and metastasis and one with metastasis. Three patients had died of metastasis 1, 3, and 13 years respectively, after the initial surgical therapy. The exact histogenesis is still obscure. Most likely it is a tumor of fibroblast- and histiocyte-like cells, akin to malignant fibrous histiocytoma, but different in its age incidence, microscopic appearance and behavior.

A case of spinal extradural venous hemangioma is presented. Its initial manifestation simulated a herniated intervertebral cervical disc. The probable precipitating mechanism of its presentation in producing radicular pain and its successful surgical extirpation are discussed.

Experiences with 70 CO2-laser procedures are reported. 39 operations were performed in the larynx, 7 in the pharynx, 4 in the oral cavity; 19 were tonsillectomies and 1 the excision of an exophytic skin tumor. The advantages of the CO2-laser-surgery are: minimal hemorrhage during surgery, excellent visibility in the operative field, slight postoperative edema and reduced postoperative pain. Surgery with the CO-2-laser beam is best performed for the following indications: endolaryngeal microsurgery, procedures in highly vascularized organs (tongue) or on well vascularized tumors (hemangioma), operations in hemophiliacs.

Pain associated with a large hemangioma of the thigh was relieved by embolization of a portion of the lesion with Gelfoam. The branch to be occluded was selected when the distribution of the heat associated with the injection of contrast material corresponded to the distribution of the patient's pain.

Twenty-four patients with angiomas of the thoraco-lumbo-sacral region of the spinal cord, which characteristically occur in elderly men and are situated on the dorsum of the cord, were subjected to surgery after diagnosis by selective arteriography. It is suggested that in this variety of angioma the operation produced its effect by eliminating the raised pressure in the venous and capillary vessels within the spinal cord. There was no evidence of an arterial "steal." In all cases, followed up for a mean of five years, progression of neurological symptoms was arrested but, apart from motor disability and pain, any improvement among this already seriously disabled group was small. Diagnosis in the early stages is difficult but the history of mechanical factors exacerbating the symptoms and which occur in the majority of patients should suggest the presence of such a lesion and indicate the need for myelography. Any improvement in results can only come from greater awareness of the condition leading to earlier diagnosis and operation.

Diffuse hemangioma of the spleen occurred in a 59-year-old man. The presenting features were a dull ache and heaviness in the left upper quadrant for 3 weeks, severe left-sided pain and fever immediately before hospitalization, and a tender mass in the left upper quadrant. The condition was diagnosed at laparotomy; splenectomy was performed. In this case intrahemangiomatous hemorrhage had occurred, but not rupture of the spleen, which is a potentially lethal complication. Other complications of diffuse splenic hemangioma are thrombosis, infarction, infection with abscess formation, and partial calcification of the vascular spaces. The condition is rare; only 56 cases have been reported.

Synovial haemangioma of joints are rare, but usually affect the knee joints of children and adolescents. Recurrent swelling, usually due to haemarthrosis, and intermittent pain may be present for a long time before the diagnosis is made. In this case report the common features of the condition are described, with emphasis on the occurrence of atraumatic haemarthrosis in children. The value of arthroscopy in diagnosis is discussed.

Hemangiomas of bone are extremely common vascular tumors that are most commonly discovered as incidental findings in the vertebral column. Infrequently, these benign lesions may cause local or radicular pain and neurologic deficits, from myeloradiculopathy to paralysis. This report describes the occurrence of a symptomatic vertebral hemangioma during pregnancy, in order to illustrate current methods for diagnosis and treatment of these lesions when associated with spinal cord compression. The possible mechanisms by which they may become symptomatic during pregnancy are reviewed.

Over the last six years 8 patients with cavernous haemangioma of the liver were operated in our institute, of whom 7 women and 1 man, aged from 32 to 77 years average, 19.6 years. In two patients double haemangiomas of the liver were removed. So the number of resected haemangiomas was 10. The size of these haemangiomas was 2, 2, 3, 3.5, 6, 6, 6, 6, 6.5 and 16 cm respectively, average 5.7 cm. Diagnosis was established by ultrasonography, computed tomography, blood pool 99m-technetium-scintigraphy, laparoscopy and selective angiography. Indications for the operation were: palpable tumour and pain in 2 patients; suspected liver metastases secondary to colon cancer 7 years after right colectomy; suspicion to primary haematoma of the liver and suspicion to secondary liver tumour in a patient with a big tumour of the left ovary. Incidental ultrasonic finding of the liver tumour occurred in a patient with obstructive jaundice. In a female with Bartter's syndrome ultrasonography and explorative surgery were carried out in order to reveal endocrine tumour (reninoma), as well as an occasional finding during cholecystectomy. Various operative techniques were performed in these 5 patients: 3 left lobectomies, 1 segmentectomy of the V and the VI liver segments, and 3 atypical resections. The only postoperative complication in our series was a mild superficial variceal thrombophlebitis of the leg in one female patient which disappeared on conservative treatment. These 5 patients have been followed-up from 1 to 48 months (average 18 months), and were symptom-free, except the patient with Bartter's syndrome who stayed on treatment.

We report the case of a young male patient with a massive cavernous hemangioma of the glans penis that had developed since childhood. The patient complained of pain when standing for long periods and during erections; furthermore its appearance disturbed him for cosmetic reasons. After associated hemangiomas of the genitourinary tract had been excluded by ultrasonography, intravenous pyelogram and cystoscopy, laser coagulation of the hemangioma was performed with a neodymium:YAG laser with the patient under local anaesthesia. The results of this conservative form of therapy show that it represents an alternative to surgical procedures; we recommend laser coagulation as the treatment of choice in patients with penile hemangiomas. The pertinent literature is reviewed.
