, is designed to penetrate the sclera and selectively destroy the ciliary body and processes without damaging the overlying tissue. Complications include conjunctival edema, corneal edema, iritis, gas in the anterior chamber,

, hyphema, hypopyon, vitreous hemorrhage, and cataract. We present a case in which a patient developed focal areas of scleral thinning 6 weeks after transscleral Nd:YAG cyclophotocoagulation. The possibility of scleral damage or thinning should be kept in mind when performing the procedure.

Systemic effects of two topical B adrenergic blocking agents, carteolol and metipranolol, recently introduced in the treatment of open angle chronic
glaucoma
, were investigated in two groups of 10 asthmatic patients, according to a controlled trial device. Saline eye drops as placebo, then, 30 mn later beta blocker eye-drops, were instillated at usual dose. Heart rate, systolic and diastolic blood pressure, vital capacity (VC) and expiratory outflow (FEV1), were checked every 15 mn during 90 mn. They did not vary under placebo. After carteolol and metipranolol, heart rate decreased more than 10% in 7 out of 10 patients in each group (extreme individual changes: -16.7 and -25.0%). Bradycardia was always sino atrial. FEV1 was lowered in 3 patients with carteolol and in 6 with metipranolol (extreme individual values: -32.3 and -31.8%). If time is not taken in consideration, the lowest values were -8.6 +/-4.6% with carteolol and -17.9 +/- 3.3% with metipranolol. CV was reduced only in 1 patient under carteolol and 2 patients under metipranolol. Systemic blood pressure remained unchanged. 7 patients complained of ocular
pain
when metipranolol was instillated. Comparison of both ocular topics shows that metipranolol lowers heart rate and FEV1 more than carteolol, which has a sympathomimetic intrinsic activity. These two drugs have the same clinical efficiency. Our results point out the risks outcoming from their systemic diffusion, and the absolute necessity to comply with the general rules of good use of beta blockers, even with eye drops, mainly the contra-indications and the strict adjustment of individual doses.

We performed a retrospective clinicopathological review of 124 patients with epithelial downgrowth seen over a 30-year-period at the Massachusetts Eye and Ear Infirmary. The purpose of this study was to determine predisposing factors in the development of epithelial downgrowth, diagnostic symptoms and signs, and the results of various types of treatments. The incidence of this condition after cataract surgery was 0.12%, decreasing to 0.08% over the latter decade. 82% of postsurgical patients with epithelial downgrowth presented to the ophthalmologist within one year following surgery, commonly complaining of decreasing visual acuity, red eye, and
pain
. The most common presenting signs of epithelial downgrowth were retrocorneal membrane, which was seen in 45% of patients,
glaucoma
in 43%, corneal oedema in 21%, and a positive Seidel test in 23%. We found that a variety of conditions predispose to epithelial downgrowth but that no one factor was present in all cases. Damage to the underlying endothelium, corneal stromal vascularisation, and stromal downgrowth appeared to be important events not emphasised in prior studies. The angle was partially or totally closed in 87% of enucleation specimens. Patients treated surgically underwent fewer enucleations than those treated medically or not treated.

, or a history of collagen-vascular disease, when present, help to alert the clinician to the correct diagnosis. Posterior scleritis affects women more often than men, but annular ciliochoroidal effusion and choroidal folds are more common in men. Exudative macular detachment and a circumscribed fundus mass are more common in women. This paper reviews the world literature on posterior scleritis and describes findings in a series of 43 patients seen at Wills Eye Hospital. It stresses the clinical features and ancillary diagnostic tests that help to establish the diagnosis.

Complicated retinal detachments (RDs) were successfully managed in 150 eyes of 170 consecutive patients by one surgeon (JLF) using silicone oil in conjunction with modern pars plana vitrectomy. Long-term postoperative complications were observed between 6 months and 5 years of follow-up. Cataracts developed in all phakic eyes and all corneas with oil-endothelial touch showed band keratopathy within 6 months. Recurrent detachments were noted in 22% of eyes during silicone oil tamponade and occurred in 13% of eyes after the oil had been removed. Other complications associated with the use of oil for vitreous surgery included pupillary block
glaucoma
(3%), closure of the inferior iridectomy (14%), fibrous epiretinal and subretinal proliferations (15%),

A telephone survey of 50 owners of blind dogs was made in order to assess how well visually deprived dogs can cope within their environment. Ten of the 50 dogs surveyed died or were euthanased as a direct result of their blindness or ocular disease. Three owners were unable to come to terms with their pet's visual loss. Two dogs died as a result of accidents directly associated with blindness and five dogs were euthanased due to
pain
caused by
glaucoma
. Twenty-seven owners reported permanent behaviour or temperament changes in their pets. Behavioural changes included a more cautious approach to the environment and closer contact with the owner. Owners were aware of an obvious compensatory reliance on the senses of hearing and smell. Permanent temperament changes were reported in only two dogs both of whom became more aggressive. Only six of the surveyed dogs were unable or unwilling to find their way around in familiar surroundings. The most consistent measures undertaken by owners to ensure the dogs' safety was to prevent access to roads and to ensure that there was minimal movement of furniture within the home. Six dogs lost interest in exercise after the onset of blindness but a further six were still allowed to roam at large. Of the 32 owners who accompanied their dogs eight reported that restraint on a lead was unnecessary. Twenty-eight owners had encountered people who had suggested it was unkind to keep a blind dog.

occurred. Case 1 complained of facial skin eruption, and was given intravenous acyclovir for 7 days. Hyphema and high intraocular pressure occurred in the left eye 10 days after the onset of the skin eruption. Case 2 had severe
pain
and blisters on her face, and was given intravenous acyclovir for 7 days. An intracameral hemorrhage and
glaucoma
developed in the right eye 15 days after the onset of the skin lesion. Intravenous acyclovir may be necessary for longer than 7-day periods if the iridocyclitis remains.
