Thirty-two patients with psoriatic arthritis unresponsive to conventional therapy were treated wtih the antipyrimidine azaribine. For the group, improvement was highly significant (P less than 0.01) when average painful joint count, ring size, grip strength,

, and morning gel were compared with baseline measurements. The spectrum of response of individual patients varied from total remission or arthritis in 5 patients to no improvement in 7.
Dermatitis
improved more than 50% in 26 patients. Reduction in hematocrit, gastrointestinal irritation, and occasional central nervous system toxicity were readily reversed by decreasing or discontinuing dosage. Azaribine appears to be a useful agent for trial in the treatment of refractory psoriatic arthritis.

An epidemic of Lyme borreliosis on Hokkaido island, Japan, was surveyed by questionnaire in 587 hunters. The 308 returns (52.4%) revealed episodes of tick bite (usually 2 or more) in 210 cases. Skin rashes appeared in 164 cases. Associated itching,
pain
, etc., but not skin rash, were more frequent after 2 or more tick bite. Previous determinations of anti-Borrelia burgdorferi antibody titers in these subjects and in controls suggested an association with outdoor activities other than hunting, and among 33 cases of dermatological symptoms and 23 of articular symptoms, 4 and 2, respectively, were antibody-positive. Most of these antibody-positive subjects had suffered tick bite at least twice, but had had no antibiotic treatment. Three subjects with joint disorders were antibody-positive. These findings suggested that the many hunters exposed to Borrelia burgdorferi frequently carried latent infections, and that some had latent form of arthritis,

and other disorders.

The Norplant System consists of 6 capsules each containing 36 mg of crystal line levonorgestrel (LNG), for a total dose of 216 mg LNG. The capsules are inserted subdermally in the mid-upper arm, and LNG diffuses continuously through the capsule walls for 5 years. In the United States the Norplant System was approved in December 1990. The initial rate of hormone delivery is about 85 mcg/day, then decreases over the next 9 months to 5 mcg/day, and over the ensuing 9 months to 35 mcg/day. Thereafter, the diffusion rate levels off for the next 3.5 years, averaging around 30-35 mcg/day. Removal results in a drop in the plasma concentration of levonorgestrel to below contraceptive levels within 24 hours and below the detectable limit of .1 pg/ml at 96 hours. Preliminarily data from 402 users over 5 years show improvements of Norplant produced better efficacy in all weight classes and a lowered cumulative rate of 1.1 pregnancies of 100 users. The pregnancy rate for the first year of use is 02., better than for oral contraceptives. Side effects include headache, nervousness, nausea, dizziness,

caused by contact with tentacles of jellyfish was studied on 25 volunteers. Two tentacles cut from a living jellyfish, Carybdea rastonii, were applied on each of the forearms and skin reactions were observed. All volunteers complained of severe
pain
, which lasted from 10 min to 8 hrs. Erythema and wheal appeared within 3 to 4 min and enlarged for 15 to 20 min. Erythema subsided within 24 hrs to 3 days in all but two individuals. Seven to 13 days after the application, linear erythema and papulo-vesicular lesions with pruritus were observed on the forearms of 15 out of 25 volunteers tested. These flare-up lesions lasted for one week leaving slight pigmentation. Histological findings from the flare-up lesions corresponded to those of allergic contact dermatitis. The lymphocyte response to the jellyfish venom in the subjects who had recurring lesions was greater than that in either the subjects with no recurring lesions or the control group, who was never exposed to jellyfish.

Inpatient and community-based care can be complementary in relation to the management of HIV disease. Medical records from 200 inpatients of Chikankata Hospital near Lusaka, Zambia and 200 home based patients were examined and compared for the common symptoms of presentation of HIV disease, associated opportunistic infections, and treatment protocols. Drug costs of both groups were also compared. The most common respiratory symptoms in the 2 groups are cough, chest pains, weight loss, and hemoptysis. Treatment employed for these symptoms were cortimoxazole, penicillin V, erthromycin, and tetracycline. Acetyl saliclic acid and paracetamol were used for
pain
relief in both groups. Gastointestinal system symptoms for both groups were diarrhea, weight loss, abdominal pain, and vomiting. Cotrimoxazole and metronidazole were used in treating diarrhea. Additional treatment protocol for the 2 patient samples included oral rehydration therapy for dehydration, antacid or bismuth subsalicylate for diarrhea and enteritis, and mycostatin for oral candidiasis. Central nervous system symptomatology included headache, dementia, neckace, and lethargy. Chloramphenicol was employed in treating bacterial meningitis. Diazepam and chlorpromazine were effective for restless patients. Genito-urinary system symptomatology for the 2 groups included dysuria, genital ulcers, hematuria, viral warts, and buboes. Furthermore, hospital treatment for HIV positive patients is more expensive than HIV negative patients; hospital costs for 50 HIV negative patients totaled US$415.94 compared to US$1204.98 HIV positive/PTB negative patients and US$1705.62 for HIV positive/PTB positive patients. Once noxious substances penetrate, they are less likely to be rapidly cleared and often will not provoke integumental reactions or

Thus older individuals are not only more vulnerable to environmental insults but also fail to recognize that they are being adversely affected. As a result, these substances may accumulate following repeated exposure to the point that the threshold for clinical display is exceeded, whereas the threshold would never be reached in younger individuals. In conclusion, I would like to stress that one of the most important observations to come out of our studies is that there exist cutaneous clues to an individual's physiologic age. It is generally appreciated that some elderly individuals seem to be much more youthful and other much older than their stated ages, suggesting that there may be a wide gap between chronologic and physiologic age. Indeed, one of the central concerns of gerontology is to develop methods by which physiologic aging can be monitored. We are certainly not the first to realize that the skin and its appendages may furnish useful markers of physiologic age. In fact, DuNuoy, who was the first to formalize the concept of physiologic age, did so on the basis of age-associated differences in cutaneous healing. By far the most comprehensive ongoing longitudinal study of human aging is that being conducted by the National Institute on Aging Gerontology Research Center at the Baltimore City Hospital. Although the integument was regretfully ignored, a saving grace of this study was the inclusion of a visual estimate of apparent age by the examining physician. This made it possible to determine whether individuals who looked older than their chronologic age were older physiologically. On 19 of the 24 tests, individuals in the subgroup who looked oldest for their age were physiologically older as well. Seventy seven patients affected by postphlebitic syndrome (PPS) during acute inflammatory and/or obstructive complications were controlled. Thirty nine patients were treated with a new low molecular weight heparin (Fluxum), 16,000 I.U. AXa/day subcutaneously for 10 days and, subsequently, 8,000 I.U. AXa/day subcutaneously for up to 50 days.
