A new test for the evaluation of fetal lung maturity, called
tap
test, been recently developed. This rapid, inexpensive, and reliable test is easy to perform and can be used whenever testing for phospholipids are not available. We performed this test in 260 clear samples of amniotic fluid obtained by transabdominal amniocentesis and the results were compared with those of the "shake test". When the "shake test" was positive (n = 156), the
tap
test was consistent with fetal lung maturity in all cases. When the "shake test" was intermediate or negative (n = 61 and n = 43, respectively), the
tap
test was consistent with
immaturity
in only 19.2% of the cases. Fifty-four fetuses were born within 72 hours after the amniocentesis. The "shake test" was positive in 50, intermediate in 3, and negative in 1. In all but one of there cases the
tap
test was consistent with maturity. The only case of RDS in this series occurred in an infant with immature result. These findings indicate that the
tap
test is a reliable indicator of fetal pulmonary maturity and probably with a higher specificity than the "shake test".

The clinical performance of three rapidly performed tests--
tap
test, TDx Fetal Lung Maturity and amniotic fluid absorbance at 650 nm--in predicting fetal lung maturity were evaluated in 300 samples of clear amniotic fluid. In all cases, delivery occurred within three days of sample collection. Of the 300 newborns, 44 developed respiratory distress syndrome. For the
tap
test, the sensitivity (95.4%), specificity (82.8%), and predictive value for maturity (99.7%) and
immaturity
(48.9%) were comparable to those of the TDx fetal lung maturity test: sensitivity of 100%, specificity of 88.3%, and predictive value for maturity of 100% and
immaturity
of 53.1%. The specificity (68.4%) and predictive value for
immaturity
(34.7%) of amniotic fluid absorbance at 650 nm were significantly lower than for the TDx and
tap
tests, whereas no significant difference was found for sensitivity and prediction of maturity between amniotic fluid absorbance at 650 nm and the TDx and
tap
tests. Both the
tap
and the TDx tests appear to be useful in antenatal assessment of fetal lung maturity in terms of reliability, rapidity and uniformity.

Three subject groups who have shown different biting forces were examined in order to investigate whether the physiological phenomenon that the duration of the silent period (SPD) of the masseter muscle would be influenced by background activities of the muscles. They were classified into child (CN, n = 10) and adult groups; the adult group was then further subdivided according to the strength of maximum biting force into a normal group (AN, n = 10) and a low biting force group with some symptoms (AS, n = 5). SPD was observed in the masseter muscles by applying chin
tap
stimulations during both clenching teeth (10%, 50% and maximum) and biting on a transducer (50 N, 100 N and maximum). With increase of muscle activities, SPD was significantly decreased in the AN and CN groups, whereas no significant changes were found in the AS group. When comparing SPDs among the three groups at the same voluntary effort, the AN group exhibited the shortest SPD followed by the CN and AS groups. The results suggest that SPD was positively influenced by the strength of the background activities of the muscles and that
immaturity
of the masseters in terms of muscle activity could affect the SPD.
