Diffuse persistent glomerular immaturity and focal proximal tubular ectasia were seen in bilateral open renal biopsy specimens for an infant with fluid and salt depletion and slowly progressive renal failure. Subsequently, diffuse tubulopapillary renal adenoma subtotally replaced each kidney, thereby, necessitating renal transplantation. Origin of diffuse metanephric adenoma from persistent primitive epithelium of the proximal nephron is postulated and partly substantiated. We propose that this case of persistent proximal nephronic epithelial immaturity and diffuse metanephric adenoma is a variant of nephroblastomatosis and that in this case, a first trimester suicide attempt with aspirin may have initiated the maturation defect that preceded neoplastic transformation.

We present the clinicopathologic characteristics of 110 colorectal mixed hyperplastic adenomatous polyps (MHAP) that exhibited the architectural but not the cytologic features of a hyperplastic polyp. They are compared with 60 traditional adenomas, 40 hyperplastic polyps, and five colonic polyps that contained admixed but well-defined hyperplastic and adenomatous glands (HP/AD). The patients with MHAP ranged in age from 15 to 88 years (mean, 63 years). Five patients had two or more (up to seven) lesions. MHAP measured 0.2-7.5 cm in diameter. They were distributed throughout the colorectum, but a slight preponderance of large lesions (more than 1.0 cm) occurred in the cecum and appendix. All MHAP were characterized by a serrated glandular pattern simulating that seen in hyperplasia (27% of MHAP were initially diagnosed as hyperplastic polyps). However, MHAP were distinguished by the presence of goblet cell immaturity, upper zone mitoses, prominence of nucleoli, and the absence of a thickened collagen table. Although surface mitotic activity, nuclear pseudostratification, and nuclear cytoplasmic ratio were greater in MHAP than in hyperplastic polyps, they were slightly less than in traditional adenomas. Thirty-seven percent of MHAP contained foci of significant dysplasia; 11% contained areas of intramucosal carcinoma. We conclude that these lesions reflect a morphologically unique variant of adenoma and suggest that they be termed "serrated adenoma" in order to emphasize their neoplastic nature. We further offer the hypothesis that MHAP may arise from the neoplastic transformation of a more differentiated cell in the crypt than the traditional adenoma.

We applied the protein A-gold (PAG) technique to two normal human pituitary glands and three samples of hGH-producing pituitary adenoma in order to demonstrate immunoreactive hGH at the ultrastructural level. Ultrathin mirror sections immunostained for hGH were observed by electron microscopy and the numbers of immunoreactive gold particles were calculated. The numbers of gold particles of the same secretory granule shared by the mirror sections showed counts which were approximately the same. Thus, assuming that the influence on the hGH antigenicity of the tissue treatment and conditions of immunostaining should be the same at all points on the ultrathin sections, the numbers of gold particles were considered to be a quantitative indicator of the amount of hGH antigen on the secretory granules of sections processed under the same conditions. The numbers of gold particles per micron2 of cut surface of secretory granules in hGH-producing adenoma cells were statistically lower than those of hGH-positive cells from normal pituitary glands, suggesting endocrinological derangement of the tumor cells together with morphological immaturity in their secretory granules and other organelles.

The authors compared the degrees of cell maturity, antigenicity and secretory function of parathyroid (PTG) tissues from cadavers (C-PTG), 20-28 wk fetus (F-PTG) and PTG adenoma (A-PTG). In A-PTG tissues, clustered or scattered immature cells were observed, but there were none in F-PTG and C-PTG. C-PTG and A-PTG tissues revealed better secretory function than F-PTG. No clear difference was found in HLA class I (ABC) antigen quantity among the three donor PTG tissues, but the amount of HLA class II (DR) antigen in A-PTG and F-PTG were significantly lower than that in C-PTG. The results demonstrate that C-PTG, with better function, and F-PTG, with lower antigenicity, can be used in clinical transplantation. A-PTG is not suitable for clinical transplantation due to their immaturity and possibility of carcinomatous change.

The alveolar adenoma of the lung is a rare benign tumor characterized by a proliferation of both the alveolar epithelial cells and the mesenchymal septal cells. Immunohistochemically, the epithelial cells stain for cytokeratin (CK) AE1AE3, CK7, thyroid transcription factor 1 (TTF1), and surfactant apoprotein confirming the derivation by the type 2 pneumocytes. The stromal cells are negative for these markers but they show focally smooth muscle and muscle-specific actin positivity. We describe two cases that showed immunohistochemically a CD34 positivity of the mesenchymal septal cells. This aspect has been previously described in a two cases report, but not emphasized by the authors as a distinctive feature of the lesion. We consider this CD34 positivity as a marker of immaturity or stemness of the lesional septal spindle cells, that could be responsible of the different phenotypic and morphological profile of the interstitial cells, that could be, therefore, considered neoplastic and not reactive.
