The treatment of 40 patients with bone tumors have been presented. The primary tumors were located in the following sites: femur (14), tibia (8), fibula (4), humerus (4), scapula (1), clavicle (2), pelvis (5), hand (1). Investigated group were:
osteosarcoma
(18), Ewing's sarcoma (14), chondrosarcoma (2), fibrosarcoma (1), synovial sarcoma (1), chondroblastoma (4). In the most frequent malignant bone tumors,
osteosarcoma
and Ewing's sarcoma, unified management was adapted. The treatment was initiated with multidrug chemotherapy and followed by surgery or radiotherapy (Ewing's sarcoma) of the primary site. Surgery was performed in 30 cases: 19 mutilating operations because of the broad local invasion, 11 conservative surgical procedures (limb -- salvage operations). Satisfactory oncological and functional effect can be achieved after limb-salvage surgical procedures in the cases of localized, especially semimalignant bone tumors.

osteogenic sarcoma
localized in the femoral neck have been presented. As it has resulted from literature, such a localization is very rare and it matches great diagnostic difficulties, mainly caused by absence of typical periarticular reactions and ambiguous X-ray pictures. Angiography and computerized tomography are helpful in diagnosis of the type of tumor. Histopathological examination is decisive.

A case report of a cardiac metastasis from
osteosarcoma
of the pelvis is presented. The tumor obliterated nearly completely the right atrium and ventricle, inferior vena cava and pulmonary artery. It was successfully treated surgically and during a 9 month follow-up echocardiographical assessment was normal.

Monoclonal antibody 791T/36 directed against surface antigen on the human
osteosarcoma
cell line and cross-reacting with surface antigen p72 presenting on human PHA-stimulated T-lymphoblasts was used in analysis of p72 antigen expression on human mononuclear peripheral blood cells, between 2-4 days of culture. Using FACS-IV system and fluorescence microscope, it was shown that expression of p72-antigen is dependent on PHA-stimulation and the ability of lymphocytes proliferation. The expression of p72 preceding the entry of the cells into the cell cycle. This is not significantly dependent of cell size, stage of the cycle, and RNA-transcription activity. In PHA-stimulated cultures, between 2-4 days, the number of lymphoblasts expressing enough receptors for 791T/36 monoclonal antibody is sufficient to exhibit a distinct effect of Ricin A-chain conjugated with 791T/36 over 50% inhibition of 3H-Thymidine incorporation into the cells. These observations emphasises the importance of cross-reactions in cases using immunotoxins.

531 cases of malignant neoplasms and potentially malignant hyperplasia in children and adolescents diagnosed in the Department of Pathomorphology, Institute of Mother and Child between 1978/79 and 1983 are presented. Only patients treated in this hospital were included into the study. The most common diagnoses were neuroblastoma (68 cases), rhabdomyosarcoma (62 cases), nephroblastoma (56 cases),
osteogenic sarcoma
(47 cases), non-Hodgkin lymphoma (41 cases). Besides neoplasms most typical for childhood and adolescence the rare less typical cases were also seen.

A case of patient with
osteosarcoma
of the tibia resected by the Gruca method who survived 37 years is presented. In spite of the neoplastic recurrence (resected anatomically), long-lasting osteomyelitis, pathological fracture and increasing stiffness of the knee joint the patient has managed to carry out his life plans. This proves the Gruca method to be valuable although being presently replaced by polyvalent chemotherapy.

In this study we examined selected elements of the antioxidant defence system in the plasma of children with the most common solid tumors (nephroblastoma, neuroblastoma, rhabdomyosarcoma,
osteosarcoma
). We observed a significant increase of plasma antioxidant activity (AOA) in the majority of the examined children. This factor changed during clinical treatment, i.e. induction of chemotherapy, surgery and/or radiotherapy and during maintenance chemotherapy. We conclude that the elevated plasma antioxidant activity in children with malignancy may be due to the higher concentration of ceruloplasmin in affected children than in their healthy counterparts.

To improve the final treatment results in children with
osteosarcoma
, we applied after French DD-11 protocol HD MTX increasing with the younger age of patients, modified next on the basis of maximal serum drug concentrations (Cmax) as feed-back dosing. Toxic side effects were analysed according to WHO grading correlated with MTX elimination. We administered 39 HD MTX courses in 13 patients with
osteosarcoma
: aged 7-20 yrs (median 12 yrs). We performed 301 measurements of MTX concentration using the method of fluorescence polarisation. Therapeutic Cmax of 1000 microM/L and higher were obtained in 20 courses, the mean of lower values was 770 microM/L. We modified the next MTX doses in 23.7% of courses. Drug elimination was good in the majority of cases: in 34 of 39 courses at 24 hrs, in 36 of 39 at 48 hrs. Nevertheless, III and IV degree toxic side-effects accompanied about half of the courses and could not be predicted by MTX serum level measurements. HD MTX therapy with monitoring MTX serum levels proved feasible with acceptable toxicity. Therapeutic MTX levels were obtained in about 60% of cycles in patients with a favourable course of the disease in comparison with 25% in patients with an unfavorable course but the beneficial effect of age-tailored MTX and feed-back dosing on the treatment results will be possible to assess in the next 3 years.

from the very beginning in '70-ties, when only one drug treatment was applied through the following 20 years full of assays and clinical trials, which finally proved the necessity of multidrug chemotherapy in all cases of

. The introduction of crucial neoadjuvant chemotherapy with exemplary Rosen programme T-10 which enabled the progress of conservative limb-salvage surgery and experience with this programme of many national groups are discussed.
