Spontaneous pneumothorax occurring as a complication of the antitumor effect of cytotoxic chemotherapy has been reported in occasional cases of osteogenic sarcoma. Its occurrence in other tumors has not been described. This report describes two cases of this complication in patients with germinal tumors and discusses possible pathophysiologic mechanisms.

Spontaneous pneumothorax is an uncommon complication of lung metastatic disease. In most of the cases reported until today, the primary disease was a sarcoma (osteogenic sarcoma, soft tissue sarcoma, hemangioendotheliosarcoma, and Ewing's sarcoma). An exceptional case of spontaneous pneumothorax in a patient suffering from carcinoma of the breast with lung metastases, is herein presented. The pneumothorax developed immediately after regression of lung metastases during administration of combined chemotherapy. Some etiological factors, as well as the rarity of this complication and its treatment, are also discussed.

Spontaneous pneumothorax is a rare complication in both primary and metastatic pulmonary neoplasms. Occasionally, pneumothorax is associated with chemotherapy of pulmonary malignancies. Pneumothorax after chemotherapy has been reported only in cases with osteogenic sarcoma, synovial sarcoma, fibrosarcoma, germinal tumors, and lymphoma with lung metastasis. We report a case of a patient with malignant thymoma who suffered from lung metastasis after radiation and adjuvant chemotherapy from lung metastasis after radiation and adjuvant chemotherapy (cyclophosphamide, adriamycin, and cis-platinum). A chest X-ray taken 2 days after chemotherapy showed bilateral pneumothorax, which was resolved with conservative treatment. The pneumothorax in this patient is believed to have been caused by the rupturing of the tumor into the pleural cavity and the bronchi.

Spontaneous pneumothorax is a rare manifestation of primary lung cancer or metastasis. It is estimated that < 1% of all cases of spontaneous pneumothorax are tumor-associated and metastatic osteogenic or soft-tissue sarcomas are associated most commonly with pneumothorax especially in the setting of cytotoxic chemotherapy or radiotherapy. In this article, we report three pediatric cases with osteosarcoma that developed spontaneous pneumothorax during chemotherapy with a review of the literature. Two of them had lung metastasis at the time of the detection of pneumothorax and the remaining patient was found to have a bronchopleural fistula. SPx is an emergency situation and early diagnosis and management can improve prognosis and quality of life of the patient however the optimal management has yet to be determined.

Spontaneous pneumothorax is a rare complication of chemotherapy in the treatment of lung neoplasms. It is relatively common in osteosarcoma lung metastases patients following chemotherapy, but the cause is still unknown. Here, we report a case of bilateral spontaneous pneumothorax occurring in a 40-year-old female in whom lung metastases from osteosarcoma were treated with combination chemotherapy. The patient had osteosarcoma on the right side of the mandible, and developed bilateral spontaneous pneumothorax. Through contrasting chest computed tomography, pneumothorax was identified at the site of the pulmonary metastases and occurred following the formation of bullae. The pneumothorax and bullae were produced after the patient received chemotherapy. In October 2014, the patient succumbed to the disease due to hemorrhage of the primary osteosarcoma lesions.

Spontaneous pneumothorax is an emergency situation and early diagnosis and management is very important. Pneumothorax is rarely the first manifestation of lung metastasis. Spontaneous pneumothoraxes have been reported in some osteosarcoma patients. We hereby report a case of a bilateral spontaneous pneumothorax, revealing metastasis of an osteosarcoma.
