Sera from 8 of 9 patients with osteogenic sarcoma equally lysed autologous tissue-cultured cells of both skin and osteosarcoma in the presence of complement. Of 155 normal human sera tested, 103 (66%) lysed allogeneic normal ksin in tissue culture. These antibodies appeared more prevalent in younger (96% in ages 11-20 yr) than older (33% in ages 41-50 yr) humans. The presence of these "natural" antibodies against normal and malignant cells growing in tissue culture was possibly directed against components adsorbed to the cells during tissue culture or to "new" cell-surface antigens expressed by these cells grown in tissue culture. These non-tumor-related neoantigens on normal and malignant cells in tissue culture represented a potential source of confusion in studies of the serologic response of humans to tumors.

A case of an osteoblastoma of the proximal femur with a unique local, massive reactive periostitis mimicking osteosarcoma or osteomyelitis and unique systemic manifestations is reported. The severe toxic manifestations included: massive weight loss, chronic fever, anemia, systemic periostosis, and other signs. Due to confusion as to diagnosis, lack of response to numerous antibiotic regimens, and severe cachexia with clinical signs of impending death, an amputation was performed. Pathologic study revealed an osteoblastoma. A thorough review of the case suggests that the signs and symptoms were possibly consequent to an immune response mounted against the tumor rather than to secondary infection, although the latter possibility cannot be completely excluded.

Sinonasal neoplasms and neoplasm-like proliferations composed of light microscopically poorly differentiated or undifferentiated, small- to medium-sized cells cause considerable diagnostic confusion. Lesions in this category include lymphoepithelioma (undifferentiated carcinoma), olfactory neuroblastoma, small-cell undifferentiated (oat cell) carcinoma, sinonasal undifferentiated carcinoma, malignant melanoma, pituitary adenoma, lymphoid hyperplasia, malignant lymphoma, plasmacytoma, lymphomatoid granulomatosis, rhabdomyosarcoma, mesenchymal chondrosarcoma, small cell osteosarcoma, Ewing's sarcoma, and synovial sarcoma. Many of these lesions can be definitively diagnosed based on light microscopic features alone, but, in some instances, additional techniques such as immunohistochemistry are of value. The authors review the pertinent clinicopathologic features of the above lesions, with emphasis on light microscopic, immunohistochemical, and ultrastructural features of particular utility in differential diagnosis.

Ifosfamide was given to 61 patients with malignant solid tumors diagnosed before the age of 21 years. In this phase II study, all patients received 1.6 g/m2/day X 5 iv over 15 minutes followed by mesna at a dose of 400 mg/m2 iv at 15 minutes and 4 and 6 hours after ifosfamide. Responses were observed in five of 15 patients with osteosarcoma, two of ten with neuroblastoma, two of six with Wilms' tumor, two of five with rhabdomyosarcoma, four of eight with other soft tissue sarcomas, one of one with retinoblastoma, one of two with germ cell tumors, one of one with B-cell lymphoma, and one of one with a primitive neuroectodermal tumor. Fifty-nine of 61 patients had received prior alkylating agent therapy which included cyclophosphamide, cisplatin, mechlorethamine, melphalan, or dacarbazine. Fourteen of 19 responses developed in patients whose tumors were resistant to treatment with cyclophosphamide. A patient with malignant Schwannoma who had received no prior chemotherapy developed a complete response which lasted 12 months. A patient with brain metastases of osteosarcoma has had complete response for greater than 2 years. Complete response was also observed in a patient with B-cell lymphoma. Toxicity consisted of mild to moderate nausea and vomiting, transient reversible myelosuppression, occasional elevation of serum BUN or creatinine, and transient neurotoxicity characterized by somnolence, confusion, weakness, tremor, hallucinations, or seizures. We conclude that ifosfamide is an important alkylating agent without apparent complete cross-resistance with cyclophosphamide, and as such should be further investigated for determination of its activity in patients with pediatric neoplasms and considered for incorporation into phase II-III trials for certain tumors.

Human fibroblast cultures, when stimulated with interleukin-1 (IL-1) produce a growth factor for B-cell hybridoma and plasmocytoma cell lines. The availability of both a fast-growing and high-producer cell line (MG-63 osteosarcoma cells) and of a highly sensitive and specific assay system for this hybridoma growth factor (HGF) allowed us to obtain analytically pure preparations. Crude HGF from MG-63 cells was processed through a five-step concentration and purification schedule. Sequential adsorption to controlled pore glass (CPG) beads, antibody affinity chromatography and gel filtration resulted in a 10,000-fold purification to a specific activity of 10(9) units/mg HGF. Electrophoretically pure HGF was obtained after additional purification by cation-exchange chromatography and reversed-phase HPLC. The purification procedure revealed two distinct biologically active HGF components. The amino-terminal sequence of one of the two components was determined and found to correspond to that already predicted from cDNA clones of a protein alternatively called 26-kDa protein, interferon-beta 2 (IFN-beta 2) or B-cell stimulating factor-2 (BSF-2). The first two designations (26-kDa protein and IFN-beta 2) refer to a postulated fibroblast secretory protein with so far no unambiguously defined function; the latter designation (BSF-2) refers to a T-cell product possessing differentiation stimulatory effect on B-cell lines. The reported results firmly establish that the protein is secreted by fibroblasts and reveal that it possesses B-cell growth stimulatory activity. The new designation interleukin-6 (IL-6) is proposed to resolve prescribing nomenclature confusion.

Children and adolescents are not spared any of the skeletal neoplasms. The frequency is variable: The majority of patients with osteosarcoma are less than 20 years of age, but the majority of patients with chondrosarcoma are more than 20. The differential diagnosis of tumors of bone is often difficult because of confusion of these tumors with disorderly responses to injury. The radiographic features are important in the diagnosis of these lesions and in the differentiation of these lesions from other tumors of bone. Little information about differences, if any, in biologic behavior is available. Nevertheless, pathologists, pediatricians, radiologists, and orthopedists must keep in mind the wide range of skeletal neoplasms that may occur in children and adolescents. Advances in the treatment of malignant tumors of bone, particularly osteosarcoma, have made it imperative on pathologists to provide accurate diagnoses and interpretations of the effects of treatment on these neoplasms.

Two patients with extraosseous osteosarcoma of the gluteal region are presented. In one early case liquifaction of the tumour delayed the diagnosis due to confusion with a cold abscess. In the second patient the tumour developed in a region that had been subjected to post-operative radiotherapy for a uterine carcinoma seven years earlier. In both patients the tumour developed rapidly and with a fatal outcome only a few months after apparently satisfactory local excision.

The immunoconjugate XMMCO-791/RTA consists of ricin A chain bound to a murine monoclonal antibody MoAb 791T. This monoclonal antibody (MoAb) binds to a glycoprotein of 72 kD, which is expressed on human colorectal carcinoma, ovarian carcinoma, and osteogenic sarcoma. XMMCO-791/RTA was tested in a Phase I trial with proposed dose escalation steps of 0.02, 0.04, 0.15, and 0.2 mg/kg per day. Twelve patients with metastatic colorectal carcinoma were treated at 0.02, 0.03, and 0.04 mg/kg per day dose levels administered over 1 hour on days 1-5. Study-related toxicities were hypotension (6 patients); greater than 10% weight gain (6 patients); peripheral edema (9 patients); fever (4 patients); confusion (3 patients); diarrhea (3 patients); proteinuria, as identified by dipstick (3 patients), greater than 0.6 mg/dl decrease in serum albumin (11 patients); greater than 25% decrease in oncotic pressure (10 patients), and a decrease in ionized calcium (8 patients). Six patients received a second course of treatment. HAMA levels developed in 9 patients and titers increased with number of courses administered. Decreased overall toxicity, in comparison to the first course, was noted, but one patient had an allergic-type response (hypotension, crushing chest pain, diaphoresis) after the test dose of the second course (HAMA level > 10,000 IgG). Life-threatening toxicity in the form of fluid shift, resulting in noncardiac pulmonary edema and third-spacing occurred after course 1 in 1 of 3 patients at the 0.04 mg/kg per day level. No further dose escalation was attempted and no antitumor activity was seen.

The aim of this phase II study was to determine the efficacy of high-dose ifosfamide with moderate dose etoposide in childhood osteosarcoma. From January 1992 to January 1995, 27 children (15 male, 12 female) with relapsed or refractory evaluable osteosarcoma were included in a phase II study of two courses of ifosfamide 3g/m2/day and etoposide 75 mg/m2/day for 4 days. Median age was 14 years (7-19 years). All but one had received high-dose methotrexate and doxorubicin as first-line treatment. 22 patients had previously received ifosfamide. This regimen was given as first-line in 1 patient, second-line in 23 and third-line in 3. Evaluable disease was lung metastases in 21 patients, local relapse in 5 and adenopathy in 1. There were six complete responses, seven partial responses, three minor responses, six stable disease and five progressive disease (including one mixed response). Response rate was 48% (95% confidence interval, 29-67%). Duration of response was not available (10 responding patients had other treatments). Response rate was equivalent in the subgroup of 22 patients who had previously received ifosfamide (4 CR, 6 PR). Among 3 patients who received the phase II regimen as third-line chemotherapy, there was 1 PR. All but 4 patients had a well tolerated grade 4 neutropenia. Transient mild confusion or seizures were each observed once. 5 patients are alive 15-31 months after the beginning of chemotherapy. This combination of drugs at this dosage has tolerable toxicity, is efficient and deserves evaluation in phase III studies.

Osteoclast-like giant cells (GC) may dominate the histologic pattern not only in conventional giant-cell tumor (GCT)--originating as a radiologically pure lytic, possibly trabeculated lesion especially within the epiphyses of long tubular bones (LB) and pelvic bones of adults--but also in many tumor-like lesions as well as in various benign and malignant bone tumors which may simulate each other. Although the mononuclear cells as well as the amount of collagene fibres don't differ significantly, these lesions can be distinguished by substantial differences concerning their site, radiomorphology and the patients age. The unique lesion which can be recognized by histology only is chondroblastoma--centered in epiphyseal regions as GCT, mostly in the 2nd decade--by its typical mononuclear cells independently of the typical chondroid matrix and calcifications. The brown tumor of hyperparathyreoidism is associated with elevated serum Ca and parathormon, which is not altered in the histologically identical giant cell granuloma. In contrast to GCT aneurysmal bone cyst prefers the metaphyseal area of LB and the posterior parts of vertebras in the 2nd decade. Metaphyseal fibrous defects occurring during growth period leave the epiphyses unaffected and display typical x-rays. Villondoular synovitis sometimes can produce osteolytic defects. GC-rich malignant tumors which present with clear cut atypia except some cases of GC-variants of osteosarcoma, are: Giant-cell rich osteosarcoma, the rare malignant GCT, giant-cell ("osteoclastic") sarcoma, MFH and GC-rich metastases of carcinomas. All of them occur in middle aged and older patients except osteosarcoma and don't affect the epiphyses primarily except malignant GCT. To avoid confusion in GC-lesions it is conditio sine qua non to take into account for diagnosis not only histology but especially radiomorphology as well as site of the lesion and patients age.
