Pulmonary
carcinosarcoma
is an aggressive rare malignant tumor comprising a mixture of carcinoma and sarcoma components containing differentiated mesenchymal elements, such as malignant cartilage, bone, and skeletal muscle. We report a case which presented with unusual clinical features and proved cryptic until death. At autopsy, it was a stage IV lung malignancy and histopathology revealed a
carcinosarcoma
comprising an adenocarcinoma and an
osteosarcoma
with metastasis to the heart, lymph nodes, and both adrenals. To our knowledge, this is the first case of this subtype with metastasis to the heart. The present case had an unusual clinical presentation and its elusive nature towards diagnosis despite dissemination is noteworthy and unique in the literature.

Carcinosarcoma
of the lung is a malignant tumor composed of a mixture of carcinoma and sarcoma elements. The carcinomatous component is most commonly squamous followed by adenocarcinoma. The sarcomatous component commonly comprises the bulk of the tumor and shows poorly differentiated spindle cell features. Foci of differentiated sarcomatous elements such as chondrosarcoma and
osteosarcoma
may be seen. Aspergillus pneumonia is the most common form of invasive aspergillosis and occurs mainly in patients with malignancy, immunocompromising or debilitating diseases. Patients with Aspergillus pneumonia present with fever, cough, chest pain and occasionally hemoptysis. Tissue examination is the most reliable method for diagnosis, and mortality rate is high.We describe a case of primary
carcinosarcoma
of the lung concurrently occurring with invasive pulmonary aspergillosis in a 66-year old patient.

Carcinosarcoma
of the pancreas is extremely rare and its histogenesis is still unclear. This is a report on a 64-year-old female patient with an intraductal
carcinosarcoma
arising from intraductal papillary-mucinous carcinoma (IPMC) in the pancreas tail. The
carcinosarcoma
grew as a polypoid mass within the main pancreatic duct. Histologically, the tumour consisted of adenocarcinoma covering the luminal surface of the lesion with minimal stromal invasion, and
osteosarcoma
occupying the stroma. Immunohistochemical and gene mutation analyses revealed that both the carcinomatous and sarcomatous tumour cells of the
carcinosarcoma
, as well as the IPMC cells, expressed TP53 and had identical mutations in KRAS and TP53 genes, indicating that these two neoplastic components of the
carcinosarcoma
shared a common tumorigenesis and arose from the IPMC. This is the first report of a
carcinosarcoma
originating in IPMC. These findings imply that
carcinosarcoma
with a heterologous mesenchymal component is of ductal origin.

A rare case of multiple malignant tumors (poorly differentiated squamous cell carcinoma and high grade
osteosarcoma
) arising in an ovarian dermoid cyst of a 55 year old female is reported. To the best of our knowledge, this is the first well documented example of such an unusual combination of tumors arising in a dermoid cyst. The
osteosarcoma
and squamous cell carcinoma appear to arise in the background of benign teratomatous environment of a dermoid cyst rather than from "pure" mixed mesodermal tumors of the ovary. The tumors did not appear to have well demarcated boundaries with a junction or close intermingling of both cell types, features less favorable for collision tumor or
carcinosarcoma
. Despite extensive surgery with negative surgical margins and combination chemotherapy, the patient had recurrence of the tumor within four months and she died secondary to septicemia to chemotherapy and bilateral pulmonary emboli shortly after.

Osteosarcoma
is the most common primary malignant osseous neoplasm, constituting approximately 35% of skeletal malignancies. The different subtypes of
osteosarcoma
are differentiated based on clinical, histologic, and radiographic data, as well as the variable amount of osteoid produced by malignant cells. The epithelioid
osteosarcoma
subtype accounts for only 5.7% of all osteosarcomas and portends an extremely poor prognosis. The 5-year survival rate for patients with epithelioid
osteosarcoma
treated with surgery (with or without chemotherapy) is 13.5%. This is in direct contrast to the >70% ten-year survival rate of conventional
osteosarcoma
treated with surgery and chemotherapy. This article presents a fatal case of epithelioid
osteosarcoma
in an 11-year-old girl with right knee pain of 6 months' duration. Biopsy demonstrated morphologic findings consistent with high-grade
osteosarcoma
with epithelioid features. The epithelioid component was positive for vimentin and CD99; however, fluorescent in situ hybridization for the (11;22) translocation was negative. In this case, the epithelioid cells failed to respond to conventional or subsequent experimental chemotherapy for
osteosarcoma
and eventual metastasized to the lymph nodes and lungs despite multiple ablative surgeries. This case report supports the concept of
carcinosarcoma
with malignant cells lines arising from 2 different cellular lineages or a common cellular precursor. The epithelial component was more aggressive than the cells of mesenchymal origin, highlighting the need for continued research and a more favorable outcome for this rare subset of
osteosarcoma
.

Herein we present a 73-year-old man with primary
carcinosarcoma
of the liver, a rare malignant tumor of the liver. The case was followed up due to HBV-related liver cirrhosis. Regular check-up by ultrasound demonstrated a hyperechoic tumor in the left lobe of the liver, and he was referred and admitted to our hospital. Dynamic CT studies revealed a mostly hypoenhancing hepatic mass with a peripheral ring enhancement. Surgical resection was performed, and the resected tumor was macroscopically a simple nodular type, 3 cm in diameter, with a dense fibrous capsule. Microscopically, undifferentiated cells were dominant in the tumor, while moderately differentiated hepatocellular carcinoma (HCC) were also observed. A transitional zone was noted between the undifferentiated tumor and HCC. Tumor tissue with adenocarcinoma,
osteosarcoma
and chondrosarcoma were also detected. Immunohistochemical studies demonstrated that tumor cells were HepPar 1 positive in hepatocellular carcinoma, and CK19 and partly CK7 positive in adenocarcinoma. Moreover, CD56, chromogranin A and c-kit were occasionally positive in undifferentiated tumor cells. The diagnosis of
carcinosarcoma
was made based on the concomitant presence of HCC and sarcomatous components, yet it is noteworthy that various types of tumor cells were observed.

Carcinosarcoma
is exceedingly rare in the vulva. We describe a case of
carcinosarcoma
in a 67-year-old female patient who presented with recent enlargement and pain of a vulval nodule noted for 15 years. The excised tumor showed intermixed carcinomatous (adenocarcinoma and anaplastic carcinoma) and sarcomatous elements (
osteosarcoma
, chondrosarcoma, and leiomyosarcoma), which focally merged with several lobules of typical eccrine spiradenoma. The inguinal lymph nodes showed metastasis of the carcinomatous component only. This case represents the first reported case of vulval
carcinosarcoma
of the skin adnexal origin, and has to be distinguished from sarcomatoid carcinoma of epidermal origin because of a probable more aggressive behavior.

A 66-year-old woman visited our hospital complaining of painful, irritative urinary symptoms and macroscopic hematuria. Cystoscopy revealed a non-papillary tumor covered with necrotic tissue on the right side of the posterior wall of the bladder. Transurethral resection was performed ; histologically, the tumor was found to be composed of carcinomatous and sarcomatous elements. The carcinomatous element consisted of urothelial and squamous cell carcinomas. The sarcomatous element was composed of
osteosarcoma
, chondrosarcoma and spindle cell sarcoma. Immunohistochemical examination showed that the carcinomatous component was positive for cytokeratin and the sarcomatous component was positive for S-100 protein. The patient underwent total cystectomy with ileal conduit under the diagnosis of
carcinosarcoma
. Pathological examination showed no residual tumor. She was followed up with no signs of recurrence or metastasis. Computed tomography (CT) at nine months following surgery showed no evidence of recurrence. However, thirteen months after the operation, she complained of lower abdominal pain, and CT demonstrated a bulky intrapelvic tumor and right hydronephrosis. Her condition worsened rapidly and she died one month later.

Pulmonary
carcinosarcoma
is a rare disease entity defined as a neoplasm, which has biphasic features consisting of both epithelial and sarcomatous components. It has been reported that the most frequent epithelial component is squamous cell carcinoma, while the most frequent sarcomatous component is rhabdomyosarcoma. Pulmonary carcinosarcomas with
osteosarcoma
components are even rarer. We report a case of a potentially curative resection for
carcinosarcoma
with an
osteosarcoma
component. Thoracic surgeons should be aware of this rare tumor when lung tumors with ossification are encountered.

Carcinosarcoma
of the stomach is a rare biphasic tumor that consists of both carcinomatous and sarcomatous components. The common carcinoma component is tubular or papillary adenocarcinoma and the mesenchymal sarcomatous components are variable but may include leiomyosarcoma, rhabdomyosarcoma and
osteosarcoma
. The aim of this study was to describe the characteristics of gastric
carcinosarcoma
and to present a review of the available literature. We report a case of
carcinosarcoma
in a 62-year-old female including the clinical and histopathological features of this tumor. Following ultrasound and computed tomography scans, laparotomy was performed, revealing a large mass, followed by radical surgery. Macroscopically, a polypoid tumor was observed. Microscopically, the tumor was composed of moderately differentiated adenocarcinoma and poorly differentiated sarcoma with a high mitotic index and necrotic areas. At present, the achievement of a definitive diagnosis is dependent on immunohistochemical staining and radical surgery. Thus, more effective diagnostic methods are required to improve patient survival.
