On the basis of two cases of progressive external ophthalmoplegia the authors discuss the clinical manifestations of this disease and call attention to diagnostic difficulties.

In the reported case unilateral external ophthalmoplegia developed in a young woman, with concomitant pain and anaesthesia in the area innervated by the 1st branch of the trigeminal nerve. After ruling out an expanding lesion of the central nervous system, aneurysm, diabetes, myasthenia, multiple sclerosis and other diseases dexamethasone was administered in a total dose of 30 mg. Complete clinical remission was achieved. In the light of observations and a survey of the peritinent literature it has been accumed that the disease was caused by a non-specific process in the small vessels in the vicinity of the cavernous sinus.

The author presents 2 cases of internuclear ophthalmoplegia with unilateral upper lid retraction and widening of the palpebral fissure of the adducted eye with a simultaneous narrowing of the palpebral fissure of the other eye. In the 3rd case of internuclear ophthalmoplegia with paralysis of the lateral rectus muscle, the sign of upper lid retraction was present in the adduced eye.

The authors describe a case of a 36-year old male patient with the Miller Fisher syndrome of ophthalmoplegia, ataxia and areflexia. An apparent etiopathogenetic factor in this case appears to be ECHO 7 virus. The pathological process was localized in the brainstem.

A case of headaches with recurrent paralysis of cranial nerves III, IV, V and VI is reported. A good therapeutic effect was obtained with corticoids. After ruling out other diseases and causes of ophthalmoplegia the Tolosa-Hunt syndrome was diagnosed.

A case of mitochondrial enzymopathy, called also ophthalmoplegia plus, was observed in a 31-year-old man. Histoenzymatic investigations demonstrated in the myocytes decreased and irregularity of reactions for succinic dehydrogenase, tetrazole reductase and mitochondrial ATPase. In electron microscopy paracrystalline structures, lamellar bodies and concentrically condensed cristae were seen in the mitochondria, and increased glycogen stores outside the mitochondria.

The authors report the results of electronystagmographic investigations of the eyes in patients with damage to the motor nerves of the eyes, their neuromuscular junctions in cases of internuclear ophthalmoplegia, and lesions of the cerebellar and extrapyramidal systems. In all these cases pathological records of these movements were found which were fairly characteristic of a given type of damage.

The authors describe a 43-year-old man with ophthalmoplegia, ataxia and areflexia and with 0.42 g/l of protein in the cerebrospinal fluid. EMG disclosed some slowing down of the conduction velocity in the motor fibres of the extremities. Treatment with synacthen depot, cocarboxylase and vitamins gave a good effect. The described case corresponds closely to Fisher's syndrome and may be a special form of brain-stem encephalitis involving the mesencephalon.

In the years 1963-1981 internuclear ophthalmoplegia syndromes were observed 106 times in 94 patients treated in hospital for multiple sclerosis. Besides the typical syndromes of anterior and posterior internuclear ophthalmoplegia, Webino syndrome and the one-and-a-half syndrome in 4 cases syndromes were observed with superimposition of bilateral syndromes of anterior and posterior internuclear ophthalmoplegia. For these cases the author suggests the term--complete internuclear ophthalmoplegia.

This study presents a case of a 73 year old woman who has suffered from diabetes mellitus for 8 years and has had 6 isolated cranial nerves palsies within a period of 7 years. The last are-because of which she was treated in Department of Neurology in July 1992--concerned left oculomotor nerve. The coincidence of metabolic and vascular disorders provoke authors to take a wide diagnostics and discussion on diabetic ophthalmoplegia problem.
