The results of total hip replacement in 39 patients 30 years old or younger were evaluated including 67 Charnley low-friction arthroplasties and one McKee total hip arthroplasty. The average age was 25.9 years with an average follow-up of 39 months, the longest being 8 years, 3 months. The diagnoses included rheumatoid arthritis, ankylosing spondylitis, CDH, Still's disease, and a miscellaneous group. Over-all function did not improve as greatly as in the 9 to 10 year series, but this was due to a higher proportion of patients with severely disabling rheumatoid polyarthritis, hemiplegia, severe obesity, or cardiovascular disease. Wear did not seem to be any greater in this younger age group as compared to previous studies in older patients.

Joint effect of diabetes mellitus (DM) and extreme natural conditions of the Far North on the clinical course of coronary heart disease (CHD) remains poorly known. Specific features of DM2 concomitant with CHD were studied in 243 of the 8573 subjects living in Far North areas of the Tyumen region and examined by coronarography (mean age 53.4 +/- 0.4 years). The control group comprised 139 patients (56.9 +/- 0.58 years) with CHD and DM2 living in the south of Tyumen region. Clinical and ECG observations were supplemented by measurement of total cholesterol and selective coronarography. Patients of the study group were younger (p < 0.001), more frequently consumed tobacco (p < 0.001) and alcohol (p = 0.001) and had obesity (p = 0.008). Most patients in both groups had a history of myocardial infarction, suffered functional class II or higher angina and circulatory insufficiency (according to NYHA), high cholesterolemia, and arterial hypertension further deteriorating effects of CHD and DM2. However, the two groups were not significantly different in terms of these characteristics. It is concluded that combination of CDH and DM2 has unfavourable effect in patients living in the Far North although severity of clinical and functional manifestations of CDH in DM2 patients is not significantly different between residents of northern and southern areas of Tyumen region.

Adiponectin is secreted exclusively by adipocytes. Adiponectin is regulated by obesity, smoking, and genetic factors, including CDH13, which may contribute to development of diseases such as cardiovascular disease. Therefore, we aim to explore the joint effect of smoking and obesity on the association between CDH13 (rs3865188) and adiponectin among Korean men. This study included 1,570 Korean men aged 40-69 years who participated in the KARE cohort study (community-based cohorts, South Korea) from 2001. Hypoadiponectinemia was defined as the lowest quartile of adiponectin. In this study, individuals with at least one at-risk allele, the T allele, had an increased risk for hypoadiponectinemia, particularly current smokers with at least one T allele together with obesity when compared to those without the T allele. In addition, individuals with the TT genotype of CDH13 rs3865188, as well as obesity, were significantly associated with a 1.8-fold (odds ratio (OR) = 9.4, 95% confidence interval (CI) = 4.4-19.6, P < 0.001) increased risk for hypoadiponectinemia compared to individuals with the TT genotype of CDH 13 rs3865188 and normal waist circumference (WC) (OR = 5.1, 95% CI = 3.5-7.6, P < 0.001). However, in the joint effect of smoking and obesity, current smokers with the TT genotype of CDH13 rs3865188, as well as obesity, were significantly associated with a 6.2-fold (OR = 24.2, 95% CI = 3.0-196.6, P < 0.001) increased risk for hypoadiponectinemia compared to nonsmokers with the TT genotype of CDH 13 rs3865188 with normal WC (OR = 3.9, 95% CI = 1.7-9.3, P < 0.001). This study suggested that the association between CDH13 and adiponectin can be modified by lifestyle factors, such as smoking and obesity, among Korean men.
