To elucidate the nature of lipid defects in patients with diabetes mellitus (DM) concurrent with acute myocardial infarction (MI), the study was undertaken to examine the serum concentrations of total cholesterol, triglycerides, alpha- and beta-lipoproteins with DM in the presence of acute MI. 40 non-diabetic patients with acute MI, 23 diabetics with postinfarct
cardiosclerosis
, and 17 non-insulin-dependent diabetics without signs of coronary atherosclerosis. Urinary epinephrine and norepinephrine excretion was additionally determined in the acute period and 3-4 weeks after therapy. Homogeneous lipid metabolic parameters were found in CHD patients with and without DM and when transient hyperglycemia developed. The patients with acute MI exhibited some increase in lipid consumption to satisfy the energy need for the cardiovascular system, this being true for triglycerides in DM patients. The DM patients who showed low triglyceride levels had more frequently transmural MI and MI complicated with heart failure.
Obesity
and familial histories of DM and CHD in DM patients with acute MI were ascertained to be accompanied by reduced serum alpha-lipoprotein concentrations.

Ninety-eight subjects with a diagnosis of primary rheumocarditis made at a pediatric clinic in childhood were examined for the central hemodynamics by tetrapolar chest rheography. Based on a clinical and instrumental study rheumatic heart disease was diagnosed in 3 subjects; in 37 subjects, no heart pathology was recognized; myocarditic
cardiosclerosis
was diagnosed in 22,
obesity
in 20, and neurocirculatory dystonia of the cardial type in 16 subjects. The data obtained point to the overdiagnosis of rheumatic fever.

The immediate and late results of Weinberg's operation have been studied in 56 patients with chronic ischaemic heart disease (CIHD) with diffuse coronary sclerosis. Two control groups were compared; those with CIHD who were not operated on, and those who were subjected to pericardocardiopexy. The follow-up periods lasted from 6 months to 10 years. It was established that Weinberg's operation produced good effect in 76% of patients with chronic ischaemic heart disease. The operation is indicated in angina refractory to conservative therapy, with diffuse atherosclerotic lesions of the coronary bed, confirmed angiographically. Concomitant hypertensive disease,
obesity
, marked
cardiosclerosis
, recent myocardial infarction decrease the efficacy of Weinberg's operation.

Data on lipid metabolism in 109 cases of endometrial carcinoma and 33 patients in control are presented. Relevant disorders were detected in 72.5% of the study group [stage I
obesity
--23 (21.1%); II--29 (26.6%); III--25 (22.9%); IV--2 (1.9%)] which was 1.5 times the mean level in controls (51.5%). The abdominal pattern of
obesity
was predominant (77.7%). Symptoms of
cardiosclerosis
were identified in 93 patients with endometrial tumors (85.3%), 93.5% of the latter group presenting with concomitant hyperglyceridemia. In 68.8% of endometrial carcinoma patients, incidence of arterial hypertension was higher than in controls (54.5%). Lipoproteins played a major role in dyslipoproteid pathogenesis involved in
obesity
and high blood pressure. A study of the insertion-deletion (I/D) polymorphism of apolipoprotein AI genes showed two deletion alleles (DD--6%) and one heterozygote (ID--3%) in control group; no deletion alleles were identified in endometrial tumor patients (0.1 (p(0.05). An investigation of the I/D polymorphism of angiotensin-converting enzyme genes identified deletion homozygotes in 30, insertion homozygotes (II)--27, and heterozygotes--41% (control). In endometrial cancer group, the deletion allele distribution was: DD--29; II--28 and ID--45%. Deletion allele frequency in control was 0.485 while in endometrial carcinoma--0.484 (p(0.05), i.e. with out significant difference.

The authors analyzed the medical records of 1414 patients aged 60 +/- 14 years, who were examined in a specialized cardiological clinic within a one-year period. Among the patients, 41.2% complained of dyspnea; the number of women with dyspnea prevailed over the number of men. Dyspnea was caused by chronic heart failure in 42.2% of patients, by transient myocardial ischemia in 12.3% of patients, and by paroxysmal tachyarrhythmia in 6.3% of patients. In 45.6% of the patients, mostly in women, significant non-cardial factors were revealed: obstructive or restrictive respiratory failure (20.6%),
obesity
(14.7%), thyroid gland dysfunction (3.9%), pulmonary arterial thromboembolism, anemia etc. A combination of two or more etiological factors took place in 22.6% of cases. The reason for respiratory discomfort remained unclear in 21.3% of the patients, mostly women. Symptom-limited load test with gas analysis (ergospirometry) was performed in 70 patients with dyspnea of unclear origin. According to its results, in 75% of elderly patients with essential hypertension and postinfarction
cardiosclerosis
, who did not have significant systolic dysfunction, restrictive diastolic dysfunction, valvular disorder, or atrial fibrillation, dyspnea was caused by hyperventilation,
obesity
, and respiratory pathology.

Currently identified a large number of biomarkers that are closely linked with the development of chronic heart failure, some of which are clusterin and fractalkine. Accordingly, the purpose of our study was - to evaluate the role of clusterin and fractalkine in progression of chronic heart failure in patients with postinfarction
cardiosclerosis
, type 2 diabetes and
obesity
. We investigated 71 patients with postinfarction
cardiosclerosis
, type 2 diabetes and
obesity
. All patients with postinfarction
cardiosclerosis
, diabetes and
obesity
were divided into groups according to the functional class of chronic heart failure (CHF). It was found that an increase the level of fractalkine and reduced clusterin leads due to the development of systolic dysfunction and heart failure progression in patients with postinfarction
cardiosclerosis
, type 2 diabetes and
obesity
. Fractalkine and clusterin play an important role in progression of the heart failure in patients with postinfarction
cardiosclerosis
, type 2 diabetes and
obesity
, and this gives them the right to be considered indicators of the severity of CHF.
